Name of product: Handlebar Stems Used on Salsa Bicycles
Units: About 8,600
Importer: Salsa Bicycles, a subsidiary of Quality Bicycle Products, of Bloomington, Minn.
Hazard: The handlebar stems can crack or break, posing a fall hazard to the consumer.
Incidents/Injuries: Salsa Bicycles has received three reports of handlebar stems breaking. One incident resulted in a rider suffering a broken wrist.
Description: This recall involves all CroMoto S.U.L. stems sold as individual aftermarket units and on these models of complete Salsa bicycles: Ala Carte, El Mariachi, Casseroll Triple, Casseroll Single and La Cruz. The aftermarket stems are black and have the word "Salsa" painted on the extension. The complete bike stems are painted to match the bike model color and have the word "Salsa" painted on the extension. The recalled stems range from 75 through 105-degree rise and extension length from 90 to 120mm. Tri-Union Seafoods Issues Precautionary Recall of Select 5-Ounce Solid Light Tuna in Olive Oil
Tri-Union Seafoods LLC is issuing a precautionary recall of the 5-ounce variety of Genova solid light tuna in olive oil.
Due to a production error, the product is mislabeled and contains albacore tuna in soybean oil with vegetable broth that contains soy. People who have an allergy or severe sensitivity to soybean run the risk of serious allergic reaction if they consume these products. There have been no reported illnesses, and Tri-Union Seafoods is issuing the precautionary recall to protect public health.
Tri-Union Seafoods' trace-back measures determined distribution of the product at retail was limited to 900 cases – the vast majority of which were distributed to the Northeast (two cases were distributed to Publix stores in the Southeast). The 5-ounce variety of Genova solid light tuna in olive oil was distributed in January and February. Consumers who purchased the 5-ounce variety of Genova solid light tuna in olive oil with the UPC code 4800013265, "Best By Date 10/17/12" and product code DEHAH 1A225 or DEHBH 1A225 are asked to return it to the store where they purchased it for a full refund. The UPC code (also known as bar code) is found on the side of the can and the best by date and product code are ink jet printed on the bottom of the can.
"We have notified all of our customers who have received the mislabeled product and asked them to remove it from their shelves," said Shue Wing Chan, president, Tri-Union Seafoods. "This precautionary recall is the right and prudent action in the interest of protecting public health. Further, we are taking corrective steps to our production process to ensure this error does not repeat itself."

Arrowhead Mills, Inc. Voluntarily Recalls One Lot Of Organic Stone Ground Whole Wheat Flour Because Of The Possible Presence Of An Undeclared Allergen
Arrowhead Mills, Inc. announced that it is taking the precautionary measure of recalling Arrowhead Mills® Organic Stone Ground Whole Wheat Flour (2 lbs.) with a lot code of 06OCT09 because of the possibility of the presence of an undeclared dairy allergen in a limited number of bags within this lot. The lot code is located on the right side of the upper front panel of the individual bag of flour. No other lot codes of this product or any other Arrowhead Mills products are affected in any way and there have been no reported illnesses.
Consumers who have an allergy or severe sensitivity to milk protein run the potential risk of a health problem or illness if they consume this product.
The recalled product was distributed to natural food stores nationwide. Arrowhead Mills is working with distributors to actively recover any affected product remaining on store shelves.

Compound Bows Recalled by BowTech Archery; Bow Can Unexpectedly Break Apart

Name of Product: "The General" Compound Bows
Units: About 8,000
Manufacturer: BowTech Archery, of Eugene, Ore.
Hazard: The ends of the bow's limbs can unexpectedly break during use and send fragments of the bow in the direction of the user or bystanders, posing a risk of injury.
Incidents/Injuries: The firm has received 255 reports of the bow's limbs breaking, including 11 reports of lacerations, abrasions, or contusions when pieces from the breaking bow struck the user.
Description: This recall involves the "The General" compound bows hunting and target models with draw weights of 60 and 70 pounds that do not have a collet (see illustration below) in the limb. The bows measure about 31 inches long and have draw lengths of 26-30 inches. Hunting models were sold in real tree and mossy oak camouflage patterns. Target models were sold in gray, black, and wood-grain. The bows have laminated wood grips engraved with the word "BowTech" and a label with "The General" on the center pivot point of the bottom limb.

Evenflo(r) Recalls Children's Activity Centers Due to Fall Hazard

Hazard: When used as an activity table, the cap on one end of the product can loosen and fall off, posing a fall hazard to a young child.
Incidents/Injuries: Evenflo has received 11 reports of dislodged end caps which resulted in nine minor injuries, including bumps and bruises. Evenflo has also received a report of a dislodged end cap that resulted in a child suffering a broken collarbone in Canada.
Description: The recall involves Evenflo ExerSaucer Triple Fun stationary activity centers made between October 2006 and December 2008. The model number is 6231711. Stage 3 position involves converting the product into an activity table. No other ExerSaucer models use end caps and, therefore, no other models are included in this recall.

Strangulation Death of a Child Prompts Hill Sportswear To Recall Hooded Sweatshirts with Drawstrings
The U.S. Consumer Product Safety Commission (CPSC) is announcing today a voluntary recall by Hill Sportswear Inc., of Paramount, Calif., of 300,000 hooded drawstring sweatshirts. CPSC received one report of a death involving a 3-year-old boy in Fresno, Calif. He was strangled when the drawstring on the hooded sweatshirt that he was wearing became stuck on a play ground set.
The sweatshirts, Kid Pullover Hood Sweatshirt and Kid Zipper Sweatshirt with Hood, have drawstrings through the hood. The drawstrings should be removed immediately since they can become caught on objects and strangle a child. In February 1996, CPSC issued guidelines (pdf) to help prevent children from strangling or getting entangled on the neck and waist drawstrings in upper garments, such as jackets or sweatshirts.
The recall includes the Kid Pullover Hood and Kid Zipper with Hood. The Kid Pullover Hood has a flat style drawstring at the neck, one middle front pocket, and fleece inside lining. Top sewn in label reads "HILL/ Made in USA." The hooded Zipper children's sweatshirt has a rounded style drawstring at the neck, two front pockets, and fleece inside lining. Top sewn in label reads "HILL/ Made in USA". The recalled sweatshirts were sold at various small retailers in California and Texas from August 1999 through December 2008 for approximately $8.
Due to the serious nature of this hazard, parents are urged to immediately remove the drawstrings from the sweatshirts or return the garment to either the place of purchase or to Hill Sportswear for a full refund.

Marzocchi Recalls Bicycle Forks Due to Crash Hazard and Risk of Injury during Repair

Hazard: The suspension system on bikes with the recalled forks could become completely compressed and fail to return to its original position after a forceful landing. This can cause riders to lose control of the bicycle and crash. The forks can also expand forcefully if repair is attempted by the consumer, posing a risk of serious injury.
Incidents/Injuries: The firm has eight reports of damaged forks outside the U.S., including a report of an injury from a fall.
Description: The recalled bicycle forks were sold for installation on mountain bikes. The recall involves certain 2008 model year 888RC3 and 888RC3WC suspensions with date codes beginning with T07D07, T07D08 and T07D09. The forks are white or black with red and black artwork on the legs.

Peanut Corporation of America Provides Additional Information about Expanded Nationwide Recall of Peanut Products
In connection with the January 28, 2009 expanded recall announcement issued by Peanut Corporation of America (PCA), the company wishes to highlight important information regarding several of its products.
On January 28, 2009, PCA announced it was voluntarily recalling all peanuts and peanut products processed in its Blakely, Georgia facility since January 1, 2007 because they have the potential to be contaminated with Salmonella.
Salmonella is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting, and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurisms), endocarditis and arthritis. PCA has not manufactured any products in cans and jars since 2007. However, because the January 28th recall notice indicated that PCA does not sell products directly to consumers in stores, and because the expanded recall includes PCA products made since the beginning of 2007, the potential for confusion may exist. Therefore, the company is highlighting information about those former retail offerings to assist consumers who may have purchased those products and who may still have them in their possession. These products were part of the January 28th recall, but here, they are individually listed. Consumers should be aware that the list below includes only PCA's own brands that were sold to consumers at retail. The list does not include brands made by PCA for other firms. FDA continues to update its website to alert the public about any new recalls.
PCA sold its Casey’s, Parnell’s Pride, Reggie, and Robinson Crusoe labels to 99 Cent Stuff, 99 Cents Only Stores, Dollar General, and Dollar Tree Stores. Listeria Contamination in Queso Fresco, Fresh Cheese
New York State Agriculture Commissioner Patrick Hooker today warned consumers not to consume certain “Queso Fresco, Fresh Cheese,” made by Peregrina Cheese Corp., 342 Ten Eyck St., Brooklyn, New York 11206 due to possible Listeria contamination.
The product is contained in a foil wrapped, 14 oz. net weight package with a code of 4461. The consumer warning affects all packages with this code.
A routine sample of the cheese, taken by an inspector from the Department’s Division of Milk Control and Dairy Services on January 26, 2009, was subsequently tested by the New York State Food Laboratory and discovered to be contaminated with Listeria monocytogenes. On February 3, 2009, the manufacturer was notified of the presence of Listeria monocytogenes and the company voluntarily recalled the product.
Listeria contaminated product could cause Listeriosis, a disease that usually causes mild flu-like symptoms in healthy individuals; however in immune-compromised individuals, meningitis and blood poisoning can occur. Pregnant women are also considered a high-risk group, as Listeriosis can also result in stillbirths.
While Peregrina has most of the affected product in its possession, a small quantity of the product was distributed to retail outlets.
To date, no illnesses are known by the Department to be associated with this product.

Genisoy Food Company Announces an Expanded Voluntary Recall on Protein Bars Due to Potential Health Risk
Genisoy Food Company today expanded its voluntary recall because of concerns that the recalled bars contain peanut products that were manufactured by Peanut Corporation of America (PCA), which is the focus of an on-going Salmonella investigation. Genisoy took this action in cooperation with the FDA in response to PCA's recent expansion of its peanut products recall to include all peanut products produced in PCA's Blakely, Georgia plant since January 1, 2007.
Salmonella is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis. The recalled bars were sold in all types of retail stores as individual bars in 12-count caddies throughout the United States. No 6-count caddies of Genisoy or MLO Bio Protein were affected by this recall. All MLO Bio Protein Bars and Genisoy Soy Protein Bars with codes after 5/27/09 are not a part of this recall because they were produced at a manufacturer that did not use PCA peanut butter products. Genisoy Organic Apple Cinnamon, which is the focus of the original Genisoy Food Company Voluntary Bar Recall is the exception – still subject to voluntary recall. Genisoy Organic Soy Protein Rich Chocolate and Mixed Berry are not a part of this expanded recall because the manufacturer did not use PCA peanut products in these bars.

This product is being recalled because of the expanded recall notification from Peanut Corporation of America (“PCA”) and the possibility of contamination with Salmonella, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis.
The product was sold directly to consumers from retail stores located nationwide.
The recalled product is packed in a 7.5 oz bag with the UPC Code 0 70844 00112 9 with the above mentioned Best Before Dates. The Best Before Date is located on the back panel in black text.
PANOS’ voluntary recall is in response to the widening recall involving Peanut products supplied by PCA due to possible contamination with Salmonella. Products distributed by PANOS under the Kame brand have NOT been linked to the nationwide Salmonella outbreak.
This recall is being conducted with the knowledge of the Food and Drug Administration.

Rich Ice Cream Company, Inc. voluntarily recalled its Rich’s Nutty Sundae Cone on January 30, 2009, because the product has the potential to be contaminated with Salmonella. This action is the result of the Peanut Corporation of America’s expanded recall on January 28, 2009, for all products manufactured in their Blakely, Georgia facility since January 1, 2007.
Salmonella is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis. Rich’s Nutty Sundae Cone is distributed in AL, CA, DE, DC, FL, GA, IL, IN, KS, KY, LA, MD, MA, MI, MS, MO, NV, NH, NJ, NY, NC, OH, OK, PA, PR, SC, TN, TX, VI, VA, Bahamas, and Barbados in bulk packages of 24, and sold to consumers in a 3 fl. oz., cone-shaped package with UPC 75455-42000 printed on the individual package.
Rich Ice Cream has received no reports of illnesses related to our product to date. No other products manufactured by Rich Ice Cream are involved in this recall.
Because Rich’s Nutty Sundae Cone is typically consumed at the point of sale, it is unlikely that consumers will have the product on hand in their homes. However, if for some reason consumers have the product at home, they are instructed to dispose of it.

As a result of the expansion of Peanut Corporation of America’s (PCA), Blakely, Georgia facility, recall to include all peanut ingredients produced since January 1, 2007, Farley's & Sathers Candy Company, Inc. has issued a voluntary nationwide recall for products containing PCA supplied peanuts. This PCA facililty is the subject of an U.S. Food and Drug Administration investigation into potential Salmonella contamination of peanut butter, peanut paste, and whole peanuts.
