Goodman Company Reannounces Recall of Air Conditioner/Heat Pump Units Due to Fire Hazard

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product.

Name of Product: Packaged Terminal Air Conditioner/Heat Pump (PTACs) Units

Units: About 30,000; previously recalled in August 2008

Manufacturer: Goodman Company, LP, of Houston, Texas

Hazard: The power cords on the PTACs can overheat, posing a burn or fire hazard.

Incidents/Injuries: Goodman has received eleven reports of smoke or fire associated with the PTAC's power cords. No injuries have been reported.

Description: The recall includes 5.0 kW Amana-brand, Comfort-Aire-brand and Century-brand Packaged Terminal Air Conditioner units with model numbers PTxxx3x50xx (Amana) and EKTxxx-150x (Comfort-Aire and Century) and serial numbers 0702112056 through 0804237539. The model and serial numbers are located on the control board plate found under the PTAC unit's front cover.

Sold at: Goodman and heating and cooling equipment dealers nationwide from February 2007 through June 2008 for between $700 and $1,000.

Evenflo Recalls Cake Toys on Children's Activity Centers Due to Choking Hazard

The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following products. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Evenflo ExerSaucer(r) 1-2-3 Tea for Me(tm) Activity Learning Centers

Units: About 66,000 in the United States and 13,660 in Canada

Manufacturer: Evenflo Co. Inc., of Miamisburg, Ohio

Hazard: The candle flame attached to the top of the cake toy can detach, posing a choking hazard to young children.

Incidents/Injuries: Evenflo has received 11 reports of the toy flames detaching. Five of the incidents occurred in the United States and six in Canada. No injuries have been reported.

Description: This recall involves Evenflo ExerSaucer(r) 1-2-3 Tea for Me(tm) activity learning centers. The tea party themed toys have a two-tier cake that is light blue, dark pink and yellow. Models included in the recall are 6161834 and 6161920. The model numbers are printed on the packaging and on a label located on the underside of the base of the product. The recommended age for use of this product is 4 months to walking.

Sold at: Toys "R" Us and juvenile product stores nationwide from December 2007 through March 2009 for about $70.

Remedy: Consumers should immediately remove the cake toy from the product and contact Evenflo to receive a free replacement toy. The ExerSaucer(r) may continue to be used without the cake toy.

Infant Suffocation Deaths Prompt Recall of Amby Baby Motion Beds/Hammocks

Hazard: The side-to-side shifting or tilting of the hammock can cause the infant to roll and become entrapped or wedged against the hammock's fabric and/or mattress pad, resulting in a suffocation hazard.

Incidents/Injuries: Amby Baby is aware of two infant suffocation deaths in the Amby Baby hammock. In June 2009, a 4-month-old girl in Lawrenceville, Ga. died in a baby hammock and in August 2009, a 5-month-old boy from Gresham, Ore.

Description: The Amby Baby Motion Bed consists of a steel frame and a fabric hammock which are connected by a large spring and metal crossbar. There is only one model of the hammock available which can be identified by a label sewn onto the hammock stating: "Amby - Babies Love It, Naturally."

Sold online at: Ambybaby.com and other Internet retailers from January 2003 through October 2009 for about $250.

Bayer Consumer Care Voluntarily Recalls One Lot of Combination Package of Alka-Seltzer Plus(R) Day & Night Cold Formula Liquid Gels

/PRNewswire/ -- In consultation with the U.S. Food and Drug Administration (FDA), Bayer's Consumer Care division has begun a voluntary recall of a single product lot of the combination package of Alka-Seltzer Plus® Day & Night Cold Formula Liquid Gels. Bayer initiated the recall after identifying that the labeling on the foil blister card of certain packages within the lot (less than 4 percent) were printed with the label reversed. All individual liquid filled capsules are imprinted correctly.

The affected Alka-Seltzer Plus product lot number can be found on both the interior blister package (in black text adjacent to the expiration date) as well as on the exterior carton containing the blister packaging (embossed on the side panel under the Bayer logo).

The company initiated an investigation following consumer reports and found that in a limited number of combination packages of Alka-Seltzer Plus Day and Night Cold Formula Liquid Gels from a single lot, the information on the underside of the blister package was reversed. Therefore, the label for the green Night product appears under some of the blue Day product and vice versa. As such, there is a risk that consumers may not be aware of the warnings of an antihistamine in the product that could cause drowsiness.

Unilever Conducts Nationwide Voluntary Recall of Slim-Fast® Ready-to-Drink Products in Cans Due to Possible Health Risk

Unilever United States, Inc., in cooperation with the U.S. Food and Drug Administration (FDA), is conducting a nationwide voluntary recall of all Slim-Fast® ready-to-drink (RTD) products in cans, due to the possibility of contamination with Bacillus cereus, a micro-organism, which may cause diarrhea and possibly nausea and/or vomiting. The probability of serious adverse health consequences is remote.

The products were sold in stores nationwide.

Product Description:

The products are packaged in paperboard cartons and contain four, six or 12 steel cans that are 11 FL OZ (325 mL) each. Individual cans are also sold in certain retail outlets. The recall involves all Slim-Fast® RTD products in cans, regardless of flavor, Best-By date, lot code or UPC number. A listing of all RTD recalled products is attached to this press release.

No other Slim-Fast® products are affected by this recall. No Slim-Fast® powdered shakes, meal bars, or snack bars are affected by this recall.

The recall was initiated after the company conducted quality testing on Slim-Fast® RTD products in cans. Out of an abundance of caution, the company is recalling all RTD products in cans that are currently in distribution centers, on-shelf or in back rooms in retail outlets or in consumers’ homes. The company is in the process of identifying and correcting the production issue, and will resume production and shipment of the product when the issue has been addressed and corrected.

No illnesses have been reported to date.

This recall was initiated after it was discovered that the labels on these particular products did not reveal that there is MILK in the product. Further investigation has revealed the problem was caused by a temporary breakdown in the company’s packaging process.

Production of these products has been suspended until Snax In Pax, Inc. is certain the problem has been corrected.

Blenders Recalled by Haier America Due to Laceration Hazard

Hazard: The blade assemblies of the blenders may come apart or break, posing a laceration risk.

Incidents/Injuries: Haier America has received approximately 60 reports of blade assemblies coming apart or breaking. One consumer reported receiving a minor cut to his hand.

Description: This recall involves commercial-style 500-watt blenders with model number HB500BSS. The units are stainless steel and black plastic. "Haier" is printed on the front below the power switch, and the model number is printed on a label on the bottom of the blenders.

Sold at: Mass merchandisers and specialty retailers nationwide from November 2006 through October 2009 for between $26 and $60.

CPSC, LaJobi Reannounce 2001 Recall of Cribs Due to Entrapment and Strangulation Hazards Concern Cribs Could Now Be At Second Hand or Thrift Stores

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: "Molly" and "Betsy" Cribs

Units: About 400 (This recall was first announced in December 2001

Manufacturer: LaJobi Inc., of Cranbury, N.J.

Hazard: The cribs have cut-outs in the end panels that can allow young children to get their heads entrapped, which can pose a strangulation hazard to infants and toddlers.

Incidents/Injuries: CPSC is aware of the death of a one-year old girl whose head and neck became entrapped in the headboard cut-out of the "Betsy" style crib in 2003 in Mississippi, following the 2001 recall. No additional incidents or injuries have been reported.

Description: This recall involves both "Molly" and "Betsy" style wooden cribs. The end panels on the "Molly" style cribs are made of solid wood with openings on both sides. The end panels on the "Betsy" style cribs are constructed with wood slats. The model numbers are printed inside the headboard at the bottom.

Diamond Pet Foods Announces Recall of Premium Edge Adult Cat and Premium Edge Hairball Cat Food

On September 23, Diamond Pet Foods issued a voluntary recall for Premium Edge Finicky Adult Cat and Premium Edge Hairball cat because they have the potential to produce Thiamine Deficiency. Today’s announcement provides additional information from the company’s posted announcement of September 23 when the initial recall information was provided.

Thiamine is essential for cats. Symptoms of deficiency displayed by an affected cat can be gastrointestinal or neurological in nature. At the first stage the cat may show decreased appetite, salivation, vomiting, and weight loss. Later, neurologic signs can develop, which may include ventriflexion (bending towards the floor) of the neck, wobbly walking, circling, falling, and seizures. These ultimately may result in the death of the animal if left untreated. If your cat has consumed the recalled product and has these symptoms, please contact your veterinarian.

The affected products were distributed in Maine, New Hampshire, Vermont, Massachusetts, Connecticut, Rhode Island, New Jersey, Maryland, Delaware, New York, Pennsylvania, Virginia, Alabama, Tennessee, North Carolina, South Carolina, Georgia, Florida.

The affected date codes were RAF0501A22X 18lb. (BB28NOV10), RAF0501A2X 6 lb. (BB28NOV10), RAF0802B12X 18lb (BB30FEB11), RAH0501A22X 18 lb. (BB28NOV10), RAH0501A2X 6lb. (BB28NOV10, BB30NOV10, BB08DEC10)

To date, 21 cases of thiamine deficiency in cats have been reported and confirmed by Diamond. The reports have been confined to the New York and Pennsylvania areas and none have been received since October 19.

Diamond has tested the product and found the cat foods were deficient in thiamine. Samples taken by the FDA indicated that there were additional lots with insufficient levels of thiamine. No other complaints have been reported on any other product manufactured by Diamond Pet Foods.

Vita Food Products, Inc. Issues an Alert Announcing Undeclared Wheat in ELF Branded Herring in Sour Cream‏

Vita Food Products, Inc. of Chicago, IL is voluntarily recalling its Elf Branded Herring in Sour Cream product distributed in retail stores in California, Texas, Massachusetts, Maryland, Georgia, Florida, Michigan, Minnesota, North Dakota, and Wisconsin because their labels do not disclose that the product contains wheat. The following product, in the two sizes listed below, is affected by the recall:

Description: This recall involves girl's velour hooded sweatshirts with a zip front. The sweatshirts were sold as a part of a 2-piece set. "Major Diva" is printed on the front of the sweatshirts. The tag on the inside of the sweatshirts reads, "2b REAL." The sweatshirts were sold in hot pink, light pink, ivory and khaki, and in sizes 4, 5/6 and 6X.

Sold exclusively at: Burlington Coat Factory stores nationwide from October 2008 through July 2009 for about $11.

Team Work Trading Expands Recall of Children's Metal Pendants Due to Risk of Lead Exposure

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Children's Metal Pendants

Units: About 1,500 (an additional 1,500 pendants were previously recalled in September 2009)

Importer/Distributor/Retailer: Team Work Trading, of Los Angeles, Calif.

Hazard: The recalled children's pendants contain high levels of lead. Lead is toxic if ingested by young children and can cause adverse health effects.

Incidents/Injuries: None reported.

Description: This recall involves eleven types of metal pendants sold with silver-colored chains. The pendants feature symbols from the following animations/cartoons: "Bleach," "Death Note," "Naruto" and "One Piece."

Sold at: The Teamwork Trading Store in Los Angeles, Calif. and other toys stores and gift shops nationwide from November 2008 through March 2009 for between $3 and $4.

Manufactured in: China

Remedy: Consumer should immediately take the recalled pendants away from children and contact Team Work Trading to receive a full refund or replacement product.
