Sony Recalls Computer AC Adapters Due to Shock Hazard

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Sony VAIO Computer AC Adapters

Units: About 69,000

Manufacturer: Sony Electronics Inc. of San Diego, Calif.

Hazard: Insulation inside the AC adapter can fail over time, posing an electrical shock hazard to consumers.

Incidents/Injuries: Sony has received four reports of adapters short circuiting. No incidents occurred in the United States. No injuries have been reported.

Description: The recalled AC adapter model is the Sony VGP-AC19V17, which was supplied for use with the following Sony products:

Near Strangulation of Children Prompts Recall of Roman Shades by Louis Hornick & Co.; Sold Exclusively at Bed Bath & Beyond

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Dublin Energy Solution Roman Shades

Units: About 364,000

Importer: Louis Hornick & Co. Inc., of New York, N.Y.

Retailer: Bed Bath & Beyond Inc., of Union, N.J.

Hazard: Strangulations can occur when a child places his/her neck between the exposed inner cord and the fabric on the backside of the blind or when a child pulls the cord out and wraps it around his/her neck.

Incidents/Injuries: There have been two reports of children becoming entangled in the exposed inner cord on the back of the shade. In July 2008, while in his crib, a 20-month-old boy became entangled in the inner cord from a roman shade. His grandfather responded to the child's cries and removed the cord that left a red mark on the right side of his neck. In April 2009, the mother of a 3-year-old boy found her crying son with a cord mark that ran ear to ear on the front of his neck. The boy was able to free himself after becoming entangled in the inner cord. No permanent injuries were sustained in these incidents.

Description: This recall includes all sizes and colors of Dublin Energy Solution Roman Shades sold by Bed Bath & Beyond with exposed inner cords on the back. Colors include navy, chocolate, khaki and cream. On the backside of the head rail, a small orange, black and white warning label sticker in English and Spanish which read in part, "Cords and bead chains can loop around child's neck and STRANGLE. Always keep cords ...to get to cords."

Halloween Flashlights Sold Exclusively at Target Recalled Due to Burn Hazard

Hazard: The flashlights can overheat and melt, posing a burn hazard to consumers.

Incidents/Injuries: The firm has received eight reports of flashlights overheating and melting, including one report of burns to the hand.

Description: This recall involves two types of Halloween-themed flashlights: the mini flashlights and flashlights sold with stencils.

Mini Flashlights - The mini flashlights have a key ring extending from the bottom and were sold in a pack of three colors: orange, green and black. The orange and purple packaging has "Mini Flashlights (3 Pack)" printed on the front and "DGI", "Made in China" and "DPCI# 234-02-1813" printed on the back.

Standard Size Flashlights - Also included in the recall are standard sized flashlights with a black handle and an orange top. The flashlights were sold with six stencils in various colors and images: a pumpkin, ghost, spider, cat, witch and skull & cross-bones. "Flashlight with Stencil" is printed on the front and "Tien Hsing," "Made in China" and" 234 02 1838" is printed on the back of the packaging.

Incidents/Injuries: In March 2008, a 2-year-old boy from Ocean View, Delaware climbed up on a toy chest to look out of a window and became entangled in the inner cords of the Roman shade. His parents removed the cord which left a temporary red mark around his neck. No permanent injuries were sustained.

Description: This recall involves all Faux Suede room darkening Roman shades with style number 12810A. The shades were sold in Ivory, Taupe, Gold, Plum, Wine Red, Midnight Blue, Moss Green and Dark Brown and have a head rail that measures 23, 27, 31, 35, 39 43 or 48 inches. The shades have a sticker on the head rail with the letters "WS" and "style 12810A" is printed on the packaging box.

Risk of Strangulation Prompts Recall to Repair IKEA Roller Blinds

Name of Product: ISDANS, TUPPLUR, and ENJE Roller Blinds

Distributor: IKEA Home Furnishings, of Conshohocken, Pa.

Hazard: Strangulations can occur if the blind's looped bead chain is not attached to the wall or the floor with the tension device provided and a child's neck becomes entangled in the free-standing loop.

Incidents/Injuries: None reported.

Description: This recall involves all ISDANS, TUPPLUR and ENJE roller blinds. These blinds are made of a solid or sheer fabric, measuring 24" to 79" wide, and have a metal top rail and a bead chain. ENJE has a glued label on the wall fitting showing the IKEA logotype, article number, 5-digit supplier number, four digit date stamp (YYWW) and "Made in Taiwan". The roller blinds have an orange warning label attached at the base of the blind that is marked with AA-136800-3.

Sold at: IKEA stores nationwide from July 2005 through July 2009 for between $10 and $55.

Manufactured in: Taiwan and France

Remedy: Consumers should immediately check the recalled roller blinds to make sure the tension device provided is attached to the bead chain and installed into the wall or floor. If not attached, consumers should immediately stop using the roller blinds and contact IKEA or visit their local IKEA store to receive a free repair kit. The repair kits for the roller blinds will be available the second week of November. If the consumer has difficulty installing the tension device, contact IKEA for additional information.

Qualitest Pharmaceuticals Issues a Voluntary Nationwide Recall of All Accusure® Insulin Syringes

Huntsville AL-Qualitest Pharmaceuticals today issued a voluntary nationwide recall of all Accusure® Insulin Syringes. The distributed syringes are of the following descriptions and NDC numbers: 28G 1/2cc, NDC 0603-6995-21;28G 1cc, NDC 0603-6996-21; 29G 1/2cc NDC 0603-6997-21, 29G 1cc, NDC 0603-6998-21, 30G 1/2cc, NDC 0603-999-21, 30G 1cc, NDC 0603-7000-21, 31G 1/2cc, NDC 0603-7001-21; and 31G 1cc, NDC 0603-7002-21. All Accusure® Insulin Syringes regardless of lot number are subject to this recall. These syringes were distributed between January 2002 and October 2009 to wholesale and retail pharmacies nationwide (including Puerto Rico). The syringes in these lots may have needles which detach from the syringe.

If the needle becomes detached from the syringe during use, it can become stuck in the insulin vial, push back into to the syringe, or remain in the skin after injection.

Consumers who have any Accusure® Insulin Syringes should stop using them and contact Qualitest at 1-800-444-4011 for reimbursement. You can find the lot number on the white paper backing of each individual syringe.

Qualitest is notifying all customers who received these syringes and arranging for the return of any affected product.

Mars Snackfood US ISSUES ALLERGY ALERT ON UNDECLARED PEANUTS In Dove Caramel Pecan Perfection Ice Cream

Mars Snackfood US announced a voluntary recall of its Dove Caramel Pecan Perfection ice cream with the lot number 931AB5YN07 because it may contain undeclared peanuts. People who have an allergy or severe sensitivity to peanuts run the risk of serious or life-threatening allergic reaction if they consume these products. No related illnesses have been reported to date. For consumers who do not suffer from a peanut allergy or sensitivity, this product is safe to eat. The lot number is found on the bottom of the containers.

This voluntary recall is limited to this specific lot number (931AB5YN07) and was distributed only in the United States, and specifically to the following states: AL, CO, FL, GA, IA, IL, IN, KS, KY, LA, MI, MN, NC, OH, OR, TX, UT, WA and WI.

Dove Caramel Pecan Perfection is sold in a brown and gold 15.10 fl. oz. pint container and pictures a ribbon of caramel, pecans and a square of Dove® Brand Chocolate on the front.

The voluntary recall was initiated after Dove Caramel Pecan Perfection containing peanuts in lot number 931AB5YN07 was discovered. Mars Snackfood US was able to quickly confirm that only this lot is affected and, out of an abundance of caution, issued a voluntary recall for potentially affected containers of the ice cream.

Mars Snackfood US will work with retail customers to ensure that the recalled products are not on store shelves. In the event that consumers believe they have purchased products affected by this voluntary recall, they should return the product to the store where they purchased it for a full refund. CPSC Urges Parents and Caregivers to Stop Using "My Baby Soother" Pacifiers Due to Choking Hazard

The U.S. Consumer Product Safety Commission (CPSC) is urging parents and caregivers to immediately stop providing "My Baby Soother" pacifiers to their children. The pacifiers were distributed by T & L Trading Corp., of Brooklyn, N.Y. The pacifiers failed to meet federal safety standards because the nipples can separate from the base easily, posing a choking hazard to infants and toddlers.

CPSC is issuing this warning because T & L Trading has refused to recall these pacifiers.

About 16,500 "My Baby Soother" pacifiers were sold at grocery stores, delis, and discount stores in Bronx, Brooklyn, Broadway, and Coney Island of New York from August 2007 through July 2009 for about $1.

The "My Baby Soother" pacifier has a ring-shaped handle and a blue, pink, red, white or yellow heart-shaped mouth guard with two ventilation holes. The nipple is made of either silicon or latex. The pacifier package has the words "My Baby Soother" printed on the top and a picture of an infant on the background.

Distributors and retailers who purchased the "My Baby Soother" pacifiers from T & L Trading should immediately stop distributing the pacifier and call CPSC. Consumers should immediately take the recalled pacifiers away from infants and toddlers and discard them.

Unilever Issues Allergy Alert on Wheat in a Limited Number of Tubs of Breyers® Ice Cream

Unilever United States, Inc., of Englewood Cliffs, NJ, in cooperation with the U.S. Food and Drug Administration (FDA), is voluntarily recalling a limited number of tubs of Breyers® ice cream because it was mispackaged and may contain undeclared wheat. People who have an allergy or severe sensitivity to wheat run the risk of allergic reaction if they consume this product.

On a limited number of tubs, the lid describes the product as "Breyers® All Natural Cookies & Cream" ice cream, and the tub portion of the package describes the product as "Breyers® All Natural Mint Chocolate Chip" ice cream. The product inside of the tub is "All Natural Cookies & Cream." The ingredient statement on the tub does not declare wheat, which is an ingredient of the Cookies & Cream product.

The mispackaged ice cream product was distributed in Alabama, Arizona, Delaware, Florida, Georgia, Maryland, New Jersey, New York, Ohio, Pennsylvania, Puerto Rico, South Carolina, Tennessee, Trinidad and Virginia through retail stores in these locations and adjoining areas. The affected product is being recalled from consumers and retailer store shelves, back rooms and warehouses.

The affected mispackaged product was distributed in 1.5-quart (1.41L) paperboard tubs marked with UPC # 077567254245 located on the side of the tub and in combination with "Best If Used By" dates of FEB1711GH, FEB1811GH and FEB1911GH located on the bottom of the tub.

The allergy alert and recall relates only to the above mentioned mispackaged Breyers® ice cream. No other Breyers® ice cream products are affected.

No adverse reactions have been reported to date. In addition to working in cooperation with the FDA, the company is also issuing an alert through the Food Allergy & Anaphylaxis Network.

The recall was initiated after the company received a few consumer complaints about the mispackaged product. It was determined that product containing wheat was distributed in packaging that did not reveal the presence of wheat in the ingredient list on the tub.

Consumers who have purchased the mispackaged Breyers® ice cream with UPC # 077567254245 and in combination with "Best If Used By" dates of FEB1711GH, FEB1811GH and FEB1911GH are urged to discard it immediately and contact the company at 1-877-270-7402 for a full refund. Hazard: The plastic decorative inserts on the flatware's handles can detach during dishwashing, posing a choking hazard to children.

Incidents/Injuries: The company has received 28 reports of a plastic insert detaching from the flatware. One incident of injury reported, no specifics provided.

Description: This recall involves the Fiesta Masquerade and the Home Olympic 20- piece flatware sets. The Fiesta Masquerade flatware set is silver-colored and the handles have plastic decorative inserts in red, orange, yellow, green and three different shades of blue. The Home Olympic flatware sets are charcoal and the handles have decorative black inserts.

Blair Expands Recall To All Women's Chenille Apparel Due to Burn Hazard; Additional Reported Deaths Prompt Re-Announcement of Robe Recall; 9 Reported Deaths in Total; Consumers Urged to Stop Use of Flammable Wearing Apparel


The U.S. Consumer Product Safety Commission (CPSC) and Blair LLC, of Warren, Pa., are expanding Blair's voluntary recall of women's full length chenille robes to include additional chenille robes and three other chenille products all manufactured by A-One Textile & Towel. CPSC and Blair also are re-announcing the earlier recall of women's robes.

In April 2009, Blair recalled 162,000 chenille robes after it learned of three robes catching on fire, including one report of second degree burns. Blair then received several reports of deaths allegedly due to robes catching fire. This prompted a second news release in June 2009 to again alert consumers to the burn hazard for the chenille robes. After this re-announcement, Blair received four additional reports of deaths allegedly due to the robes catching fire. Three of these victims were cooking and a fourth was tending a pellet stove; three of the four victims were in their 80s and one was in her 70s. All nine reported deaths occurred prior to the April 2009 announcement of the recall.

Blair has received one report involving one of the additional garments described below catching fire; no injuries have been reported related to these garments. Blair decided to expand the recall to include all women's chenille wearing apparel manufactured by A-One Textile & Towel.

Units: About 138,000; 162,000 units were previously recalled in April 2009

Manufacturer: CA-One Textile & Towel, of Karachi, Pakistan. A-One Textile & Towel manufactured all of the products recalled in April 2009. Blair is expanding its recall to all women's chenille wearing apparel manufactured by A-One.

Retailer: Blair LLC, of Warren, Pa.

Hazard: Some of these products fail to meet the federal flammability standard and present a risk of serious burns to consumers if the garments are exposed to an open flame.

Incidents/Injuries: Since announcing the recall in April 2009, Blair has received nine reports of deaths allegedly due to the originally recalled robes catching fire. Blair has received one report on one of the additional garments catching fire; no injuries have been reported.

Description: The recall involves the following items:

Description: The recall involves Coby DVD/CD/MP3 players with an 8 1/2 inch screen and product number TF-DVD- 8501. The rechargeable batteries sold with the DVD players have serial numbers that begin with "HY." The serial number can be found on a white sticker affixed to the battery. "Coby" is printed on the front cover and the product number is on the bottom of the unit.

Description: This recall involves Bad Boy Buggy Standard model off-road utility vehicles. The Bad Boy Buggy Standard model has one row seat that allows two persons (the driver and passenger) to sit side-by-side while the vehicle is operated. The Standard has an open air design (no doors or windows), but has a roof. The Bad Boy Buggy Standard models come in Realtree camouflage pattern, Hardwoods camouflage pattern, hunter green, red and black colors. Urgent Food Recall Due to Undeclared Peanuts

/PRNewswire/ -- International Bazaar Inc. a Dayton, Ohio firm is voluntarily recalling a single lot of "Kroger Hazelnut Spread", packed in Italy by Nutkao S.r.L. The product has a March 2011 sell by date, is packaged in a plastic jar, contains 13 ounces of product and has a the UPC code 11110-87825. Consumers who have an allergy to peanuts may run the risk of a serious allergic reaction if they consume product containing such ingredients.

The recall applies only to the "Kroger Hazelnut Spread" with a "Sell By March 2011" date code. Approximately 65 cases were distributed to Kroger stores in as many as 16 states (Alabama, Arkansas, Georgia, Illinois, Indiana, Kentucky, Louisiana, Mississippi, Missouri, North Carolina, Ohio, South Carolina, Tennessee, Texas, Virginia, and West Virginia).

Nutkao received a complaint from 2 consumers in France that had an allergic reaction after consuming product from the same batch that was distributed under a different label. Testing revealed the product was found to contain trace amounts of peanuts. The Company has not received any reports of illness or allergic reactions caused by consuming Kroger Hazelnut Spread and has issued this recall as a precautionary measure. Consumers who are not allergic to peanuts face no safety risk.

Plum Organics Voluntarily Recalls Select Batch of Apple & Carrot Portable Pouches Due to Potential Health Risk

Plum Organics ™ announced today that it is taking the precautionary measure of voluntarily recalling one particular batch of its 4.22oz. Apple & Carrot Portable Pouch baby food with the best by date May 21, 2010 and marked with the following universal product code (UPC) #890180001221 located on the bottom of the package. These pouches are sold individually at Toys-R-Us and Babies-R-Us locations nationally.

No illnesses have been reported in connection with this product and no other Plum Organics products are affected. As a further precaution, samples from every Plum Organics product manufactured before and after this batch were tested and found to be within quality standards.

The recall was undertaken as a precaution due to the risk of potential contamination with Clostridium botulinum, which can cause botulism, a serious and sometimes lifethreatening condition. Consumers should not use these products, even if they appear to be normal, because of the possible health risk. Symptoms of botulism poisoning in humans include general weakness, dizziness, double-vision and trouble with speaking or swallowing. People experiencing these problems should seek immediate medical attention.

"The product did not meet the FDA guidelines for proper acidity level," said Dr. Paul Gerhardt, a Food Science Ph.D. and member of the Plum Organics action team. TGF PRODUCTION LLC Issues An Alert On Uneviscerated Herring Salted

TGF PRODUCTION LLC 191 CENTRE STREET BROOKLYN NY 11231 is recalling Herring Salted because the product was found to be uneviscerated.

The recalled Herring Salted was distributed nationwide in 1.3kg ( 2.86lb) round plastic containers with the code numbers 24.08.09 and 13.07.09. The Herring Salted is a product of Russia.

The Herring Salted was sampled by a New York State Department of Agriculture and Markets Food Inspector during a routine inspection. Subsequent analysis of the product by New York State Food Laboratory personnel confirmed that the Herring Salted was not properly eviscerated prior to processing.

The sale of uneviscerated fish is prohibited under the New York State Agriculture and Markets regulations because Clostridium botulinum spores are more likely to be concentrated in the viscera than any other portion of the fish. Uneviscerated fish have been linked to outbreaks of botulism poisoning.

This product may be contaminated with Clostridium Botulinum spores, which can cause Botulism, a serious and potentially fatal food-borne illness. Symptoms of botulism include blurred or double vision, general weakness, poor reflexes, difficulty in swallowing and respiratory paralysis.

Single Meter Sockets Recalled by Milbank Manufacturing Due to Fire and Electrocution Hazards

Manufacturer: Milbank Manufacturing Co., of Kansas City, Mo.

Hazard: A short may occur while in use due to an incorrect bridge installed in the product, to which the meter clips are attached. If the manufacturing defect exists, all metal parts of the meter could create a shock or burns can occur if the cover is off and the meter socket is energized.

Incidents/Injuries: Three incidents have been reported of the unit shorting out. No injuries were reported.

Description: The single meter 200 amp/4 terminal sockets are used for underground utility meter installations. The meter socket is 20 3/4 inches high by 9 inches wide and 4 1/2 inches deep with 1 meter position. It is used by the approving utility to mount their electric meter to measure how much electricity a residence uses. It can be used for an underground residential application. The recalled model number 9090 appears on the right or left side of the installed meter socket. If not installed, model numbers U9090-O or R9090-O will appear on the label on the outside of the box.

Sold at: Electrical supply distributors in N.Y., Pa., Vt. and Mass. and at local hardware stores in N.Y. from May 2009 through August 2009 for about $70.

Incidents/Injuries: Electra Bicycle Co. has received 15 reports of the front tray or the basket coming loose including two reports of minor cuts and bruises.

Description: This recall involves the 2009 Delivery 3i, Delivery 8D, Holiday 3i, Holiday 8i and Surf 3i bicycles with front-mounted trays or baskets. The trays have an alloy frame with wooden slats. The baskets are wicker with a removable tote bag. The trays were sold as original equipment on the bicycles and as aftermarket items. The baskets were sold only as aftermarket items.

Sold by: Authorized Electra Bicycle dealers nationwide from October 2008 through August 2009 for between $600 and $750 for the bicycles and about $100 for after-market trays and baskets.

Description: This recall involves a dry suit inflation hose that connects a diver's dry suit to the air supply and allows for the pumping of air into the suit to set up a positive pressure arrangement to help keep it watertight. The hose contains an air flow restricting insert that may be either black, blue or green in color. The batch code is stamped on the threaded metal end of the hose. On the air compressors the brand, model number and manufacturing date are located on the front of the motor housing. Only pressure washers and compressors with pneumatic tires with plastic tire hubs are affected; pressure washers and compressors with solid tires or metal tire hubs are not affected.

Sold at: Both products were sold at home centers and hardware stores nationwide. Recalled pressure washers were sold from January 2004 through November 2005 for between $300 and $1,400. The recalled air compressors were sold between December 2004 and October 2006 for between $300 and $500.

Hazard: The chair back supports can break, posing a fall hazard to consumers.

Incidents/Injuries: Lowe's and White Tiger have received 10 reports of chairs breaking. No injuries have been reported.

Description: This recall conducted by White Tiger involves red and blue folding director's chairs with white frames. "Garden Treasures" is printed on the chair's packaging.

Sold exclusively at: Lowe's stores nationwide from June 2009 through July 2009 for about $40.

Manufactured in: Indonesia

Remedy: Consumers should immediately stop using the recalled chairs and return them to any Lowe's store for a full refund.

Guardian Full-Face Diving Masks Recalled by Ocean Technology Due to Visor Separation Hazard

Manufacturer: Undersea Systems International Inc., dba Ocean Technology Systems, of Santa Ana, Calif.

Hazard: If significant pressure is applied vertically to the top and bottom of the visor clamp, the clear plastic visor may dislodge causing the mask to flood.

Incidents/Injuries: Ocean Technology Systems has received three reports of visors dislodging. No injuries have been reported.

Description: This recall involves the Guardian full-face mask which is a scuba diving mask that incorporates the second stage regulator into the mask allowing it to cover the diver's full face.

Sold by: Direct sales and diving equipment retailers nationwide from March 2009 through August 2009 for $800.

Manufactured in: United States

Remedy: Consumers should immediately stop using the diving masks and contact Ocean Technology Systems to receive a free repair. Ocean Technology Systems is providing consumers stainless steel clamps to secure the plastic visor.

Name of Product: Halcyon Diving Equipment

Units: About 20,300

Manufacturer: Halcyon Manufacturing Inc., of High Springs, Fla.

Hazard: The over pressure valves (OPVs) in the diving equipment could fail allowing the buoyancy compensator devices (BCDs) and the diver lift inflatable devices to leak, posing a drowning hazard to divers.

Incidents/Injuries: None reported.

Description: This recall involves Halcyon diving equipment including the Halcyon Explorer, Eclipse, CCR35, Evolve and Pioneer Buoyancy Compensator Devices (BCDs) and Halcyon Surface Marker Buoys (SMBs), Lift Bags, Diver Alert Markers (DAMs) Surf Shuttle and Diver Lift Raft Inflatable Devices. "Halcyon" is printed on the diving equipment.

Sold at: Diving equipment retailers and distributors from January 2006 through December 2008 for between $350 and $450 for the buoyancy compensator devices (BCDs) and between $50 and $275 for the inflatable devices.

Remedy: Consumers should immediately stop using recalled diving equipment and return it to an authorized Halcyon distributor or dealer for a free inspection and, if necessary, free replacement of the overpressure valve spring.
