Pampers(r) Natural Stages Pacifiers Recalled by Key Baby Due to Choking Hazard

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Pampers(r) Natural Stages Infant Ortho and Bulb Pacifiers

Units: About 29,000

Importer: Key Baby LLC, of Lutz, Fla. (Note: Pampers(r) licensed their brand name to Key Baby)

Manufacturer: Tahoe Enterprises, of China

Hazard: The pacifiers fail to meet federal safety standards and pose a choking hazard to young children.

Incidents/Injuries: None reported

Description: The pacifier comes in yellow, pink or blue colors and is made of silicone. Only "Stage 1" pacifiers are recalled. "Stage 1" and "Ortho" or "Bulb" are printed on the package. The recalled pacifiers have an oval-shaped mouth guard and "Pampers" molded on to the handle side of the mouth guard. The product comes two per package.

Sold at: Retail stores nationwide from April 2010 through February 2011 for about $6.

Manufactured in: China

Remedy: Consumers should immediately take the recalled pacifiers away from infants and contact Key Baby for instructions on returning the product for a full refund or $10 coupon toward the purchase of any Pampers(r) Natural Stages products.

Manhattan Group Recalls Parents Wooden Activity Toys Due to Choking Hazard

The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Parents(r) Busy Time Activity Centers(tm)

Units: About 400

Manufacturer: Manhattan Group LLC, of Minneapolis, Minn.

Hazard: Wooden pegs on the xylophone activity can come loose, posing a choking hazard to young children.

Incidents/Injuries: CPSC has received one report of a child putting a peg in his mouth. No injuries have been reported.

Description: The Parents(r) Busy Time Activity Centers(tm) is a wooden activity center cube-shaped toy that has a multi-colored bead run on the top. Activities on each side include a 4-tone xylophone and rasp, butterfly and caterpillar spinners, zig-zag drum, castanets, and pockets with "alligator," "bear," and "cat" pillow characters. Only items with wooden pegs bearing date code 400090GC are involved in this recall. The date code can be found on the bottom of the product packaging near the UPC code and on the tags on the bear character.

Sold at: Specialty and gift stores nationwide and through mail order catalogs and online and in Canada from December 2010 through February 2011 for about $90.

Remedy: Consumers should take these recalled toys away from young children immediately and return it to the store where purchased for a full refund. Consumers can also contact Manhattan Group for instructions on receiving a replacement item or a full refund.

Units: About 14,000 in the United States and 14,000 in Canada

Importer: Rhino Toys Inc., of Santa Cruz, Calif.

Hazard: The hard plastic C-links on both ends of the rattles' soft plastic chain can break, posing a choking hazard to young children.

Incidents/Injuries: Rhino Toys has received one report of a broken C-link. No injuries have been reported.

Description: This recall involves infant rattles called OBall Links & Mini Rattles™. The soft plastic OBall is 2 1/2 inches in diameter and has beads inside a small plastic bubble that make the rattle sound. The OBall is attached to a 7 1/2 inch soft plastic chain by C-shaped plastic links on both ends. "OBall" is stamped into the top of the plastic chain.

Sold at: Toys R Us and other specialty toy stores nationwide from April 2010 through December 2010 for about $8.

Hazard: Decorations on the mittens can be pulled off, posing a choking hazard.

Incidents/Injuries: The company has received one report of a decoration falling off a mitten. No injuries have been reported.

Description: The infant mittens are made of white cotton. They have candy cane, teddy bear and "2011" decorations glued on one side.

Sold at: The recalled products were distributed by hospitals nationwide for free to newborns from October 2010 to January 2011.

Kid O Products Recalls Baby Rattles Due To Choking Hazard

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Baby rattles

Units: About 1,500 units

Importer: Kid O Products, LLC of Perth Amboy, N.J.

Hazard: The baby rattle's beads can come off when the rattle is twisted or pulled forcefully, posing a choking hazard to young children.

Incidents/Injuries: None reported

Description: This recall involves the Prisma and Duo style wooden baby rattles with multi-colored beads. The Prisma rattle is shaped as a bell with a loop handle and has style number 10331. The rattle measures about 3 ¾- inches long. "Prisma" and the style number can be found on the packaging. The Duo rattle is two interlocked rings with red and blue bead accents. "Duo" and style number 10333 is printed on the packaging.

Sold at: Specialty children's stores nationwide from November 2010 through December 2010 for about $12.

Units: About 8,300 units in the United States and 2,000 in Canada

Hazard: The overalls have snaps that could come loose and pose a choking hazard to young children.

Incidents/Injuries: None reported.

Description: This recall involves infants' Carhartt(r) overalls with style numbers GG8500 (100% cotton canvas) and GG8501 (100% cotton knit). The overalls were sold in brown or red in infant sizes 3, 6, 9, 12, 18 and 24 months. The Carhartt logo is sewn on the front of the garment and the style numbers can be found on the sewn-in label hanging from the sleeve.

Magnetic Maze Boards Recalled by Lakeshore Learning Materials Due to Choking, Intestinal and Aspiration Hazards

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Magnetic maze boards

Units: About 18,500

Manufacturer: Lakeshore Learning Materials, of Carson, Calif.

Hazard: The magnetic maze board's plastic wand can separate and expose a magnet that can be a choking hazard to children. Also, if a child has more than one of these toys and the magnets detach and are swallowed, the magnets can attract each other and cause intestinal perforations or blockages, which can be fatal.

Incidents/Injuries: Lakeshore has received ten reports of wands separating and magnets detaching. No injuries have been reported.

Description: This recall involves wooden magnetic maze boards. The wooden boards have a clear plastic sheet that covers small wooden or plastic pieces located inside a maze. A plastic wand containing a magnet in its tip is attached to the board by a cord. The wand is used to move items inside the maze. The top of the board reads: "Who's Hiding in the Garden?," "Who's Hiding in the Ocean?," "Magnetic Counting Maze," "Magnetic Alphabet Board" or "My Community Magnetic Board."

Units: About 96,000 in the United States (14,000 were sold in Canada)
Importer: Fisher-Price, of East Aurora, N.Y.

Hazard: The plastic Sonya Lee figure in the play set can break at the waist, exposing small parts that pose a choking hazard to young children.

Incidents/Injuries: The firm has received eight reports of the Sonya Lee figure breaking. No injuries have been reported.

Description: This recall involves the Little People Play 'n Go Campsite. The seven-piece plastic play set includes Sonya Lee, a tent and other accessories. Product number R6935 is printed on the toy's packaging. The name Sonya Lee is printed on the underside of the figure. Only Sonya Lee figures that bend at the waist, have a green sweater and purple camera around the neck are included in this recall. No other Sonya Lee figure is affected. The remaining pieces of the Little People Play 'n Go Campsite are not affected.

Sold at: Major retailers including mass merchandisers, discount stores, department stores and toy stores nationwide and in Puerto Rico, and by online retailers from October 2009 through August 2010 for about $15.

Voluntary Recall of PreserVision® Eye Vitamin AREDS 2 Formula in the United States

Bausch + Lomb is conducting a voluntary recall of its PreserVision® Eye Vitamin AREDS 2 Formula with Omega 3 soft gels, only available within the United States.

Bausch + Lomb chose to initiate this recall based on a small number of reports predominantly within a specific age group, age 70 and older, who reported difficulty swallowing or a choking sensation when taking the soft gel.

The voluntary recall is limited only to the United States; it does not affect locations in Europe, the Middle East or Asia-Pacific regions. The PreserVision Eye Vitamin AREDS 2 Formula with Omega 3 is the only supplement affected in the recall; all other PreserVision and Ocuvite® supplements, soft gels and tablets, remain on the market.

To clarify, the formulation of PreserVision Eye Vitamin AREDS 2 Formula with Omega 3 is safe. While many of our customers can comfortably swallow the supplement, we believe the design of the soft gel requires further consideration. Our customers are our top priority, and we want to ensure they have a supplement which is comfortable to use on a daily basis.

Bausch + Lomb expects to release an AREDS 2 formulation in a smaller soft gel which will be dosed twice per day, two pills per dose. This immediate redesign is expected to be available to customers by later this year.

We have directly contacted U.S. retailers who have been shipped this product to initiate the recall and inform them of the steps they should take to return the product to us. We have also contacted eye care professionals to alert them of the recall.

We are asking consumers who currently have the PreserVision Eye Vitamin AREDS 2 Formula with Omega 3 to return the product to Bausch + Lomb. Even if consumers are comfortable swallowing the soft gel, we urge them to return the product.

Consumers who have this product in their home should call our customer service center for instructions on returning and reimbursement: 1-800-553-5340. Bausch + Lomb’s customer service line is open to consumers 9am-5pm EST Monday through Friday.

Units: About 1.2 million in the United States and 102,000 in Canada

Manufacturer: Klean Kanteen Inc., of Chico, Calif.

Hazard: The water bottle spouts can break or crack, posing a choking hazard to children and adults.

Incidents/Injuries: The firm has received six reports of incidents of the water bottle spouts breaking or cracking in children's mouths as they were drinking from the bottles. No injuries have been reported.

Description: The Sport Cap 2.0 water bottle spouts are made of black plastic and have an attachment loop. "Klean Kanteen" is stamped into the side of the cap.

Sold at: Outdoor sports stores, including LLBean, and grocery and specialty stores nationwide, and on the Web at www.llbean.com from April 2009 through April 2010 for $5 for the spout and between $15 and $28 when sold with a stainless steel water bottle.

Gund Recalls to Replace Baby Books Due to Choking Hazard

The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following products. Consumers should stop using products immediately unless otherwise instructed.

Name of Product: Gund Baby Paperboard Books

Units: About 15,100 in the United States and 865 in Canada

Importer: Gund, of Edison, N.J.

Hazard: The styrofoam used to fill the book binding can detach, posing a choking/aspiration hazard to infants and young children.

Incidents/Injuries: Gund has received three reports of children mouthing the styrofoam that detached from the books. No injuries have been reported.

Description: This recall involves three miniature children's paperboard books with plastic handles designed as baby rattles. They include "Animals" (item number 059174), "Numbers" (item number 059175) and "Colors" (item number 059176). The books were also sold as a three-book set (item number 059173). The name "Gund" and the item number are located on the back of the books. The books measure 4 1/2 inches by 7 inches.

Children's Fork and Spoon Sets Recalled by Peachtree Playthings Due to Choking Hazard; Sold at Dollar Tree and Deals

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Scooby Doo, Tweety and Batman Fork and Spoon Sets

Units: About 127,000

Manufacturer: Peachtree Playthings, of Atlanta, Ga.

Hazard: The middle two prongs of the plastic fork can detach, posing a choking hazard to children.

Incidents/Injuries: Peachtree Playthings has received one report of a middle prong detaching from the fork. No injuries have been reported.

Description: This recall involves the Scooby Doo, Tweety and Batman plastic fork and spoon sets. The two-piece set is white plastic with cartoon images on the handle.

Sold at: Dollar Tree Stores and Deals during January 2010 for about $1 per set.

Remedy: Consumers should immediately take the recalled fork and spoon sets away from children and return them to Dollar Tree or Deals for a full refund.

Pull Toys Recalled by Manhattan Group LLC Due to Choking and Aspiration Hazards

The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following products. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Pull-A-Long Friends Toucan(tm), Pull-A-Long Friends Alligator(tm), and Pull-A-Long Friends Sharky(tm)

Units: About 2,400 in the United States and 400 in Canada

Manufacturer: Manhattan Group LLC, of Minneapolis, Minn.

Hazard: The toy has wooden components that can break or come loose, posing a choking/aspiration hazard to young children.

Incidents/Injuries: None reported.

Description: This recall involves three types of pull-toys: Pull-A-Long Friends Toucan(tm) with lot code 210720GB, Pull-A-Long Friends Alligator(tm) with lot code 210750GB, and Pull-A-Long Friends Sharky(tm) with lot code 210530GB. The Toucan has a large yellow and black beak and blue striped wings. The crocodile is green and has a red ridge on its back and red wheels with yellow polka dots. The shark has blue swirls painted on its wheels and an orange wooden fish on the pull string. The lot code is printed on the bottom of the toy.

Sold at: Gift and specialty stores nationwide, online and through catalogs from September 2009 through January 2010 for about $22.

Manufactured in: Thailand

Remedy: Consumers should take the toys away from young children immediately and return them to the store where purchased for a refund or a replacement toy.

Dollar General Recalls Toy Guns Due to Choking Hazard

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Special Forces and Police SWAT Toy Gun Sets

Units: About 9,600

Retailer: Dollar General, of Goodlettsville, Tenn.

Importer: Dolgencorp LLC, of Goodlettsville, Tenn.

Hazard: The orange tips located at the end of the toy guns' barrels, which are designed to distinguish them from real guns, can easily be removed from the barrels, posing a choking hazard to children.

Incidents/Injuries: None reported.

Description: This recall includes a Special Forces Weapons and Accessories Set and a SWAT Police Play Equipment Set. Each set includes two toy guns and related accessories. The number 48JQH09 is printed on the right side of the larger gun included in the recalled sets.

Sold at: Dollar General stores in 35 states from September 2009 through October 2009 for about $5.

Remedy: Consumers should immediately take the recalled toy guns away from children and return them to any Dollar General for a full refund or a replacement product.

Hazard: Loose buttons on the suit's jacket may easily detach, posing a choking hazard to young children.

Incidents/Injuries: No injuries reported.

Description: This recall includes First Impressions boy's three-piece Santa Sets in sizes 0-24 months, including a jacket, pants and hat. For units sold at Macy's and on macys.com the UPCs are: 0689439416622, 0689439417131, 0689439417148, 0689439417155, 0689439417162, and 0689439417179. For units sold at the Navy Exchange stores the UPCs are: 0706254537346, 0706254537353, 0706254537360, 0706254537377, 0706254537384, and 0706254537391. For all products the tracking label information sewn into the side seam of the top and bottom includes: Style 9131, Supplier 22155, Mfg 34837, Aug. 2009, Macy's Inc.

Sold at: Macy's and macys.com, including locations in Puerto Rico, from September to December 2009, and at U.S. Navy Exchange stores from November through December 2009 for about $20 to $40.

Child's Asphyxiation Death Prompts Recall of Toy Dart Gun Play Set by OKK Trading

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: "Action Team" Toy Dart Gun Set

Units: About 22,000

Importer: OKK Trading Inc., of Los Angeles, Calif.

Manufacturer: Kong Hoi Industrial Co., of Hong Kong, China

Hazard: If a child places the soft, pliable plastic dart in his/her mouth, he/she is likely to choke/aspirate the dart into her/her throat impairing the child's ability to breathe. If the dart is not immediately removed, brain damage or death can occur.

Incidents/Injuries: CPSC and OKK Trading have received one report of the November 2007 death of an 8-year-old boy in Port Arthur, Texas. The child reportedly was chewing on the toy dart when he inadvertently swallowed it and it became lodged in his throat blocking his ability to breathe.

Description: The "ACTION TEAM" play set has a toy gun with three soft rubber darts, a S.W.A.T. watch, a baton, walkie-talkie, a whistle, and a badge with a clip and an identification card. The soft, pliable orange plastic darts have a nearly 2 ½ inch long shaft and an approximately ¾ inch diameter suction cup. The toy gun is black with an orange nose and trigger and red spring release mechanism. "Made in China" is printed on one side of the barrel.

Sold at: Discount department stores nationwide from December 2006 through March 2008 for about $1.

Remedy: Consumers should immediately take the recalled dart gun sets away from children and contact OKK Trading to return the toy for a $3 bounty. OKK Trading will provide a free postage paid envelope for consumers to return the toy.

Name of Product: Monday the Bullfrog Plush Books

Units: About 142,000

Manufacturer: Simon & Schuster Inc., of New York, N.Y.

Hazard: The plastic eye on the frog can detach, which may pose a choking hazard to young children.

Incidents/Injuries: Simon & Schuster has received reports of three plastic eyes that detached after use and one unit that was packaged with a detached eye. No injuries have been reported.

Description: Monday the Bullfrog is a plush puppet bullfrog with a board book attached to the frog's mouth. It was sold in clear plastic packaging, and the book is marked with ISBN numbers 10: 1416912312 and 13: 9781416912316 printed inside the mouth of the bullfrog.

Sold at: Bookstores and retailers nationwide and online from January 2006 through November 2009 for about $18.

Remedy: Consumers should immediately place the product out of a child's reach and contact Simon & Schuster for a free replacement product.
