Alpine Slicing and Cheese Conversion Company Issues Voluntary Recall of...

“HEB Queso Cotija (Grated)”, “El Viajero Queso Quesadilla (Shredded)”, “El Viajero Queso Cotija (Grated)”, “BMF Queso Quesadilla (Shredded)” Because of Possible Health Risk

Alpine Slicing and Cheese Conversion Company in Wisconsin is voluntarily recalling “HEB Queso Cotija (Grated)”, “El Viajero Queso Quesadilla (Shredded)”, “El Viajero Queso Cotija (Grated)”, and “BMF Queso Quesadilla (Shredded)” because it has the potential to be contaminated with Listeria monocytogenes, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, Listeria infection can cause miscarriages and stillbirths among pregnant women.

No illnesses have been reported.

  • “HEB Queso Cotija (Grated)” – 8 oz. packages, Distributed in Texas. UPC code 0 4122024846 9 and sell-by dates of 08FEB12 and 09MAR12, which are located on the back of the package. Product was distributed between October 27, 2011 and December 23, 2011.
  • “El Viajero Queso Quesadilla (Shredded)” -- 8 oz packages, Distributed in Wisconsin, Illinois, Tennessee and Florida UPC code 0 7282014045 7 (located on the bottom left of front label) and sell by dates of 24FEB12, 09MAR12 or 15MAR12, which are located on the back of the package. Product was distributed between November 1, 2011 and January 3, 2012.
  • “El Viajero Queso Cotija (Grated)” -- 4 lb. packages, Distributed in Indiana and Minnesota. Use-by code 09MAR12, located on the back of the package. Product was distributed between November 11, 2011 and January 3, 2012.
  • “BMF Queso Quesadilla (Shredded)” -- 5 lb. packages, Distributed in New York. Sell by date of 16FEB12, 01MAR12 or 15MAR12, on product cases. Product was distributed between October 21, 2011 and January 3, 2012.
Alpine Slicing and Cheese Conversion discovered the issue during testing of its products by an independent laboratory. Laboratory testing has confirmed that certain products processed in the shred line contained Listeria monocytogenes. Therefore, other products produced on the same line, even if they have been tested and found to be negative, have the potential to be contaminated.

Alpine Slicing and Cheese Conversion has contacted the FDA and issued the recall. Alpine Slicing and Cheese Conversion has ceased the production and distribution of the products as FDA and Alpine Slicing continue their investigation into the matter. All remaining inventory has been quarantined.

Green Valley Food Corp. is recalling approximately 35,159 cases of a variety of products because they have the potential to be contaminated with Listeria monocytogenes. Random samples tested positive for Listeria monocytogenes, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, Listeria infection can cause miscarriages and stillbirths among pregnant women.

Any customer who received any of the products on 12/07/2011 to 1/1/2012 are affected in this recall and/or if the items have a use by date ranging from 12/22/2011 to 1/17/2012. The original recall was initiated on 12/23/11 and 12/24/11, we are adding the additional items mentioned in this recall.

The sprouts affected in this recall were distributed via truck deliveries to all customers in Texas. Our customers consist on grocery store distribution centers and food service customers

Till this present day there has bee n no related i l l nesses CONFIRMED because of this recall . This i s a cautionary measure taken by G reen Vall ey Food C orp. to assure safe and quali ty products are being distributed by our facility.

On 12/12/2011 a random sample was taken from a customer the product tested positive for Salmonella. Green Valley Food Corp. voluntarily recalled the alfalfa-based products when we were notified of the positive result on 12/23/2011 and 12/24/2011. On 12/21/2011 random samples were taken of our facility, several samples had a positive result for Listeria monocytogenes. To assure our customers of safe products, Green Valley Food Corp., is voluntarily recalling the additional items listed, which consist of “Let’s Grow Healthy Together”, “Green Valley Food Corp.” and “BroccoSprouts” branded products. This is a voluntary cautionary measure taken by Green Valley Food Corp. to assure all customers and consumers of any issues with our products. We are voluntarily requesting this recall and are working closely with the FDA to assure all consumers our products are safe.

If you are a direct customer affected by this recall you will be receiving a letter asking for the quantities you were shipped and their whereabouts. Consumers who purchased these product at your local grocery store should dispose of the products immediately in a trash receptacle.

HP Expands Recall of Notebook Computer Batteries Due to Fire Hazard Consumers urged to recheck notebook models and batteries

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Lithium-ion batteries used in HP and Compaq notebook computers

Units: About 162,600 additional batteries (54,000 and 70,000 batteries were previously recalled in May 2010 and May 2009, respectively)

Importer: Hewlett-Packard Company, of Palo Alto, Calif.

Hazard: The recalled lithium-ion batteries can overheat and rupture, posing fire and burn hazards to consumers.

Incidents/Injuries: Since the May 2010 recall expansion, HP has received 40 additional reports of batteries that overheated and ruptured, resulting in seven burn injuries, one smoke inhalation injury, and 36 instances of property damage.

Description: The recalled lithium-ion rechargeable batteries are used with various model series of HP and Compaq notebook computers and include batteries that consumers were informed were not included in previous recalls. The chart below includes all notebook model numbers associated with batteries recalled to date. The computer model number is located at the top of the service label on the bottom of the notebook computer. Not all batteries matching the bar codes are being recalled.

Walmart Recalls GE Food Processors Due to Laceration and Fire Hazard

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: General Electric(r) Food Processors

Units: About 255,000

Importer: Walmart Stores Inc., of Bentonville, Ark.

Hazard: The safety interlock system on the recalled food processor can fail; allowing operation without the lid secured which poses a laceration hazard. In addition, the product can emit smoke, or catch fire, posing a fire hazard.

Incidents/Injuries: Walmart has received a total of 58 incident reports: 24 reports of the food processor operating without the lid in place, of which 21 resulted in injuries to fingertips; and 34 reports of the unit smoking, including 3 reports of fires.

Description: This recall involves GE-branded digital, 14-cup food processors. The food processors are black with stainless steel trim, and model number 169203 is imprinted on the underside of the unit.

Sold exclusively at: Walmart stores nationwide and Walmart.com from September 2009 through February 2011 for a retail price of about $50.

Manufactured in: China

Remedy: Consumers should immediately stop using the recalled food processor and return the product to any Walmart for a full refund.

STIHL Recalls Yard Power Products Due to Burn and Fire Hazards

Additional Fingertip Amputations and Lacerations Prompt Reannouncement of November 2009 Recall of Strollers by Maclaren USA

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today reannounced a voluntary recall of the following consumer product. Consumers who have not yet obtained the repair should do so immediately. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Maclaren strollers (sold prior to November 2009)

Units: About one million (this product was recalled in November 2009)

Distributor: Maclaren USA, Inc., of South Norwalk, Conn.

Hazard: The stroller's hinge mechanism poses a fingertip amputation and laceration hazard to the child when the consumer is unfolding/opening the stroller.

Incidents/Injuries: Maclaren has received a total of 149 reported incidents with the strollers, including 37 reported injuries that occurred after the stroller was recalled in November 2009. These reported injuries include five additional fingertip amputations, 16 additional lacerations and 16 additional fingertip entrapments/bruising. At the time of the original recall, there were 15 incidents, including 12 reports of fingertip amputations in the United States.

Description: This reannouncement involves all Maclaren single and double umbrella strollers sold prior to November 2009. The word "Maclaren" is printed on the stroller. Maclaren strollers sold after May 2010 have a different hinge design and are not affected by this announcement.

Sold at: Juvenile product and mass merchandise retailers nationwide from 1999 through November 2009 for between $100 and $360.

Remedy: Consumers who have not installed the hinge covers should immediately contact Maclaren USA to receive the free repair kit.

Six L's Voluntarily Recalls Grape Tomatoes Because of Possible Health Risk

Six L's of Immokalee, Fla. is voluntarily recalling a single lot of grape tomatoes, because they have the potential to be contaminated with Salmonella, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. The strain of Salmonella has not been determined and no illnesses have been reported.

The specific lot was packed on April 11 and was comprised of grape tomatoes that can be identified by Cherry Berry lot code DW-H in either in clam shells or 20 lbs. cardboard containers. The product was distributed to North Carolina, South Carolina, Alabama, Florida, New Jersey, New York, Michigan, Pennsylvania, California, Georgia and Canada, and reached consumers through retail stores and restaurant distribution.

The contamination was detected through a random sample obtained by the USDA at a distributor in New York. The product is from a farm in Estero, Fla. which has since ceased production of that commodity.

The recall does not include any other tomatoes or produce distributed by Six L's. Consumers who have purchased the specific lot of grape tomatoes mentioned above are urged to return it to the place of purchase for a full refund. Taylor Farms Pacific, Inc. Recalls Grape Tomatoes Due to Salmonella Risk

Taylor Farms Pacific, Inc. of Tracy, CA has been notified by grower Six L’s that a specific lot of grape tomatoes supplied to Taylor Farms Pacific may be contaminated with Salmonella. This product has been recalled by Six L's.

This lot of grape tomatoes was used in the following products made by Taylor Farms Pacific for Albertsons, Raley's, Safeway, Savemart, Sam's Club, & Walmart and is being voluntarily recalled as a precautionary measure. No illnesses have been reported.

Salmonella is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy people may experience fever, nausea, vomiting, diarrhea (which may be bloody), and abdominal pain. Defibtech Announces a Voluntary Recall of DDU-100 series AEDs

Defibtech, LLC is initiating a worldwide voluntary recall of certain DDU-100 series semi-automatic external defibrillators (AEDs) sold under the Lifeline AED and ReviveR AED brand names, including 65,885 AEDs distributed in the United States. This recall affects only DDU-100 Series AEDs shipped with 2.004 software or earlier. This corrective action addresses two possible conditions, which in rare cases may cause an affected AED to cancel shock during the charging process and not provide therapy which may result in failure to resuscitate the patient. Both conditions are not detectable by the periodic self test.

Condition 1: In rare instances, the AED may cancel charge in preparation for a shock. Based on field data, the odds of an affected AED having this happen are less than a 1 in 400,000 chance per month for any given AED.

A subset of AEDs (less than 11%) that are affected by Condition 1 are also affected by Condition 2: In rare instances, the AED may cancel charge in preparation for a shock in very high humidity conditions. The only reported cases were in environments of greater than 95% relative humidity or condensing conditions. Based on field data, the odds of an affected AED having this happen are less than a 1 in 250,000 chance per month for any given affected AED.

Defibtech will provide customers with a free software upgrade to address these issues. The correction to the AED will be able to be performed at the location where the AED is deployed.

Because both of these conditions occur very rarely, it is recommended that customers keep their AEDs in service until they have performed the software upgrade. Full instructions and recommendations are being mailed to affected customers. Organic Food Bar, Inc. Expands its Voluntary Recall on Chocolatey Chocolate Chip Bars to Include Additional Products and Lots

Organic Food Bar, Inc. of Fullerton, CA is issuing a voluntary recall on the specific listed lots of RAW Organic Food Bars listed below, because they may contain undeclared peanut proteins from cashew butter supplied by one of the vendors. These bars are being recalled because they are made with the same lot of cashew butter that was used in previously tested chocolatey chocolate chips bars. People who have an allergy or severe sensitivity to peanuts should return these lots to the place of purchase.

RAW Organic Food Bar lots mentioned below were distributed nationally in USA. The product reached consumers through retail stores, mail order, direct delivery etc. The affected products listed below were shipped in 12 pack cartons to each outlet or direct to consumers.

The voluntary recall is being initiated since the product may contain peanut allergen through cross contamination of cashew butter provided to us by our former supplier.

Quinault Tribal Enterprise Recalls All Canned Seafood Products Because of Possible Health Risk

Quinault Tribal Enterprise of Taholah, WA is recalling all canned seafood products including all Salmons, smoked and non-smoked; Tuna, smoked and non-smoked; Smoked Sturgeon; Minced Razor Clams; smoked Razor Clams; and smoked Steelhead. The canned seafood products are being recalled because they were not adequately processed according to the U.S. Food and Drug Administration's low-acid canned food regulations. Canned seafood that is not processed following those food safety requirements may be contaminated with harmful microorganisms that can cause serious and possibly life-threatening illness. Consumers are warned not to use the products even if it does not look or smell spoiled.

No illnesses or injuries have been reported to date.

The recalled products were distributed nationwide through distributors and retail stores.

The products are packaged in metal cans in various sizes (5oz, 6oz, 6.5oz, and 7oz) and labeled under the Quinault Pride or Quinault Tribal Enterprises brand. All manufacturing codes are subject to this recall.

This issue was found during FDA inspection at Quinault Tribal Enterprises and Quinault Tribal Enterprises is cooperating with FDA in conducting this voluntary recall.

Consumers who have purchased Quinault Pride Brand are urged to return it to the place of purchase for a full refund. Consumers with questions may contact the company at 1-800-982-8650 Monday through Friday between 8:00am and 3:00pm Pacific Time.

Toro Recalls Power Clear Snowblowers and Recycler Mowers Due to Fire or Burn Hazard

The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Toro Power Clear Snowblower and the Toro 20" Recycler Mower

Units:
Snowblowers: About 18,000 in the U.S. and 5,000 in Canada
Mowers: About 6,000 in the U.S. and 200 in Canada

Manufacturer: The Toro Company, of Bloomington, Minn.

Hazard: The carburetors on both products develop fuel leaks and can ignite when exposed to an ignition source, posing a fire or burn hazard.

Incidents/Injuries: There have been about 500 reports of carburetor leaks. There were no reports of fire or injury.

Doctor's CarbRite Diet Issues Allergy Alert on Certain Chocolate Brownie Flavored Bars

Doctor’s CarbRite Diet of New Brunswick, NJ is voluntarily recalling all lots of the 2 oz. Chocolate Brownie Doctor’s CarbRite Diet Bar that are missing the allergen statement, “Allergy Alert: Made on equipment that also processes soy, milk, egg, peanuts and tree nuts”, due to possible peanut allergen cross-contamination. This allergy statement would be found at the end of the Ingredients list. People who have an allergy or severe sensitivity to a specific type of allergen (e.g., peanuts, tree nuts {chestnuts, brazil nuts, walnuts, hazelnuts, pecans, pine nuts, cashews}, eggs, and sulfites) run the risk of serious or life-threatening allergic reaction if they consume these products.

The recalled lots of the Chocolate Brownie Doctor’s CarbRite Diet Bar extend to all fifty states and the bar was offered through a variety of distribution channels including health food stores, gyms, and internet-based e-commerce sites.

This recall affects only the Chocolate Brownie flavor for Doctor’s CarbRite Diet Bars, those marked with the UPC # 0 39442 08112 4.

Chocolate Brownie Doctor’s CarbRite Diet Bars have been produced for over 10 years for sale without any serious adverse events or allergic reactions reported in the United States.

The recall was initiated after it was discovered that the certain Chocolate Brownie flavored bars were distributed in packaging that omitted the appropriate allergen warnings: “Allergy Alert: Made on equipment that also processes soy, milk, egg, peanuts and tree nuts.” Examination of current lots of Chocolate Brownie revealed that the correct allergen warnings do appear.

This recall is being conducted with the knowledge of the Food and Drug Administration.

Consumers who purchased any one of the affected lot numbers in question are asked to return the product to the place of purchase for a full refund or credit. June Deadline Set for End to Globe Fire Sprinkler Model J Sprinkler Recall Property Owners Should Act Now To Request Replacement of Sprinkler Heads

The U.S. Consumer Product Safety Commission (CPSC) and Globe Fire Sprinkler Corp. are urging residential and commercial property owners who have model J dry-type series fire sprinklers covered under the previously-announced recall and have not yet filed a claim to replace them, to contact Globe Fire Sprinkler immediately. Home and commercial property owners should immediately submit their claim, as the program for receiving replacement heads will end on June 1, 2011.

The voluntary replacement program, which was first announced in June 2007, included about 300,000 model J series dry fire sprinklers manufactured by Globe Fire Sprinkler due to a concern that these sprinklers can deteriorate over time and can fail to operate in a fire situation.

To participate in the replacement sprinkler program, property owners must submit "Proof of Claim" and "Waiver and Release of Claims" forms to Globe Fire Sprinkler Corporation postmarked by June 1, 2011. Valid claims received prior to the deadline will qualify for replacement sprinkler heads.

Consumers can obtain the claim forms, additional information on identifying the sprinklers covered by this recall and instructions on filing a claim by visiting www.globesprinkler.com or by calling Globe Fire Sprinkler Corporation at (800) 248-0278 between 8 a.m. and 5 p.m. ET Monday through Friday.

This deadline relates only to the Globe model J dry type sprinklers recalled on June 12, 2007. It does not involve additional models of sprinklers other than those named in the June 2007 recall announcement. Building owners need not take any further action if they have: (1) had replacement sprinklers installed by Globe Fire Sprinkler; (2) submitted a claim to Globe Fire Sprinkler that has been deemed complete and installation of the replacement heads is pending; or (3) confirmed that their sprinkler heads are not covered by the Globe Fire Sprinkler model J dry type recall. Building owners who have previously submitted an incomplete claim will be sent a reminder notice reminding them to send in any missing information or materials by the June 1, 2011 deadline or else their claim will be void.

Globe Fire Sprinkler Corp. urges all other building owners to consult with their fire sprinkler installer and/or check their system records as soon as possible to determine whether their sprinkler heads are covered by this program.

Aquarium Heaters Recalled by United Pet Group Due to Fire and Laceration Hazards

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Marineland Stealth and Stealth Pro Aquarium Heaters

Units: About 1.2 million

Importer: United Pet Group, of Cincinnati, Ohio

Hazard: A wiring problem can cause the aquarium heaters to overheat or break during normal use, damaging the aquarium and posing fire and laceration hazards to consumers. Overheating can cause the heater to shatter or the acquarium glass to break.

Incidents/Injuries: United Pet Group has received 38 reports of fires resulting in property damage and 45 reports of broken aquarium glass. United Pet Group has received one report of a consumer who suffered an eye injury when the aquarium heater forcefully broke while he held it.

Description: The recall involves the following Marineland Stealth and Stealth Pro acquarium heaters. The heaters are black plastic tubes and have a temperature adjustment knob at the top. The model name "Stealth" (in white letters) or "Stealth Pro" (in red letters) is printed on the side of the heater. The model number and the wattage rating are printed below the model name. Stealth Pro heaters were also sold as part of aquarium starter kits.
