The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.
Name of Product: General Electric(r) Food Processors
Units: About 255,000
Importer: Walmart Stores Inc., of Bentonville, Ark.
Hazard: The safety interlock system on the recalled food processor can fail; allowing operation without the lid secured which poses a laceration hazard. In addition, the product can emit smoke, or catch fire, posing a fire hazard.
Incidents/Injuries: Walmart has received a total of 58 incident reports: 24 reports of the food processor operating without the lid in place, of which 21 resulted in injuries to fingertips; and 34 reports of the unit smoking, including 3 reports of fires.
Description: This recall involves GE-branded digital, 14-cup food processors. The food processors are black with stainless steel trim, and model number 169203 is imprinted on the underside of the unit.
Sold exclusively at: Walmart stores nationwide and Walmart.com from September 2009 through February 2011 for a retail price of about $50.
Manufactured in: China
Remedy: Consumers should immediately stop using the recalled food processor and return the product to any Walmart for a full refund.

Taylor Farms Pacific, Inc. Recalls Grape Tomatoes Due to Salmonella Risk
Taylor Farms Pacific, Inc. of Tracy, CA has been notified by grower Six L’s that a specific lot of grape tomatoes supplied to Taylor Farms Pacific may be contaminated with Salmonella. This product has been recalled by Six L's.
This lot of grape tomatoes was used in the following products made by Taylor Farms Pacific for Albertsons, Raley's, Safeway, Savemart, Sam's Club, & Walmart and is being voluntarily recalled as a precautionary measure. No illnesses have been reported.
Salmonella is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy people may experience fever, nausea, vomiting, diarrhea (which may be bloody), and abdominal pain. GE and Professional Series Brand Dehumidifiers Recalled Due to Fire Hazard
The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.
Name of Product: GE and Professional Series Brand Dehumidifiers
Units: About 198,000
Importer: GEA Products L.P., of Louisville, Ky., and CEM Global LLC (Professional Series) of China
Manufacturer: GD Midea Air Conditioning Equipment Ltd., of China
Hazard: A component in the dehumidifier's compressor can short circuit, posing a fire hazard to consumers.
Incidents/Injuries: Midea and GE have received a total of 14 reports of incidents involving smoke and fumes emitting from the unit and eight reports of fires. In six of the reported fire incidents, property damage extended beyond the unit. No injuries have been reported.
Description: This recall involves 30-pint and 40-pint portable dehumidifiers manufactured between November 2006 and August 2007, and during April 2008. The dehumidifiers are white with a front-loading water bucket. "GE" or "Professional Series" and digital controls are located on top of the dehumidifier. Model and serial numbers are located on the back of the dehumidifiers. Model and serial numbers included in the recall are:

Wal-Mart Recalls Electric Heaters Due to Fire and Burn Hazard
The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.
Name of Product: Flow Pro, Airtech, Aloha Breeze & Comfort Essentials Heaters
Units: About 2.2 million
Importer: Wal-Mart Stores Inc., of Bentonville, Arkansas
Hazard: The heaters can malfunction resulting in overheating, smoking, burning, melting and fire.
Incidents/Injuries: Wal-Mart has received 21 reports of incidents, which included 11 reports of property damage beyond the heater. Injuries were reported in four incidents, three of which required medical attention for minor burns and smoke inhalation. The remaining incidents included smoke irritation, sparking or property damage beyond the heater.
Description: This recall involves Flow Pro, Airtech, Aloha Breeze and Comfort Essentials 1500 watt heaters. The heaters are grey with a metal handle on the top with vents and grey control knobs on the front. The model number is 1013 and can be found on a label on the lower left corner of the back panel of the heater.
Sold Exclusively at: Walmart stores nationwide from December 2001 through October 2009 for about $18.
Manufactured in: China
Remedy: Consumers should immediately stop using the recalled heater and return the product to any Walmart store for a full refund.

Haier America Recalls Chest Freezers Due to Fire Hazard

Name of Product: Chest Freezers
Units: About 67,500
Importer: Haier America Trading LLC, New York, N.Y.
Hazard: A capacitor in the freezer's circuitry can overheat, posing a fire hazard.
Incidents/Injuries: Haier America and CPSC have received reports of 18 incidents, including four reports of fires with minor property damage, consisting of smoke damage, damage to a wall, and food spoilage. There have been no reports of injuries.
Description: This recall involves the Black &Decker(r) Model BFE53 and Haier(r) Model ESNCM053E 5.3 cubic foot capacity white chest freezers. "Black & Decker" is printed at the front upper-right corner or "Haier" is printed on the front upper-left corner of the freezer. "Black &Decker" or "Haier," the model number, the unit's serial number and other information are printed on a rating label at the top center of the back of the freezer. Only Model BFE53 and Model ESNCM053E freezers with serial numbers beginning as follows are included in this recall:

NBTY Acquisition LLC Dba Leiner Health Products Issues Allergy Alert on Undeclared Soy in Air Shield Effervescent Tablets, Air Protector Effervescent Tablets, Immune System Support Effervescent, and Airhealth Effervescent Tablets And Stick Packs
NBTY Acquisition LLC, dba Leiner Health Products of Ronkonkoma, New York is recalling Air Shield, Air Protector, Immune System Support, and AirHealth Effervescent 10 Tablet containers and Stick Packs, 8 stick containers, because they may contain undeclared Soy. People who have an allergy or severe sensitivity to Soy run the risk of serious or life-threatening allergic reaction if they consume these products.
This recall was initiated after it was discovered that the products contain soy but were distributed in packaging that did not list the presence of soy. These products were manufactured and packaged for NBTY Acquisition LLC dba Leiner Health Products by a contract manufacturer who did not disclose the presence of soy in the products.
The Effervescent Tablet product was sold under the names Air Shield, Air Protector, Immune System Support, and AirHealth, under various brands shown in the table below. The Stick Packs were sold only under Wal-born and Safeway brands.
The products listed below were distributed throughout the United States via retail stores.

Child's Death Prompts Recall to Repair Portable Playard Tent by Tots in Mind Due to Strangulation Hazard
/PRNewswire-/ -- The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Tots in Mind Inc., of Salem, N.H., is announcing the voluntary recall today of about 20,000 (and 85 in Canada) Cozy Indoor Outdoor Portable Playard Tents Plus Cabana Kits.
Clips that attach the tent to the top of the playard can break or be removed by a child. A child can lift the tent and become entrapped at the neck between the rigid playard frame and the metal base rod of the tent, posing a strangulation hazard.
CPSC is aware of a death of a 2-year-old boy in December 2008 in Vinalhaven, Maine. The boy was found hanging with his neck entrapped between the playard frame and the metal base rod of the tent that had been partially tied by pieces of nylon rope and partially attached by clips. The tent was tied to the playard because the child was able to pop off the clips. Apparently, the child became entrapped while attempting to climb out of the playard. In three other incidents, children were able to remove one or more clips and place their necks between the tent and the playard. The children were not injured.
The dome-shaped white-colored mesh tent is designed to fit over playards as small as 28 inches by 40 inches or as large as 31 inches by 44 inches to contain a child. There are 12 plastic clips to secure the base of the tent to the top rail of the playard through button holes along the bottom of the tent. The tent has a zippered side for putting in and taking out the child.
The cribs were made in China and sold at Walmart, Amazon.com and various baby and children's stores nationwide from January 2005 through February 2010 for about $60.
Consumers should immediately stop using the playard tents and contact Tots in Mind to get free replacement clips. Replacement clips will be available in late August or early September 2010. Walmart Recalls General Electric(r) Coffee Makers Due to Fire Hazard
The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.
Name of Product: General Electric(r)-branded 12-cup digital coffee makers
Units: About 900,000
Importer: Walmart Stores Inc., of Bentonville, Ark.
Hazard: The coffee maker can overheat, posing fire and burn hazards to consumers.
Incidents/Injuries: Walmart has received 83 reports of overheating, smoking, melting, burning and fire, including three reports of minor burn injuries to consumer's hands, feet and torso. Reports of property damage include a significant kitchen fire and damage to countertops, cabinets and a wall.
Description: This recall involves General Electric(r)-brand 12-cup coffee makers sold in white or black. The digital coffee maker has programmable functions and plastic housing. The GE logo is printed on the base of the coffee maker and the model number is printed on the bottom of the base. Model numbers included in the recall are 169164 (Black) and 169165 (White). No other models are included in this recall.
Sold exclusively at: Walmart stores nationwide from March 2008 through January 2010 for about $30.
Manufactured in: China
Remedy: Consumers should immediately stop using the recalled coffee makers and return the product to any Walmart for a full refund.

Hazard: The recalled necklaces contain high levels of cadmium. Cadmium is toxic if ingested by young children and can cause adverse health effects.

Strangulation Death of a Child Prompts Recall to Repair Roman Shades by All Strong Industry

Name of Product: Roman Shades
Units: About 290,000
Distributor: All Strong Industry (USA) Inc., of Ontario, Calif.
Hazard: Strangulations can occur when a child places his/her neck between the exposed inner cord and the fabric on the backside of the blind or when a child pulls the cord out and wraps it around his/her neck.
Incidents/Injuries: In February 2007, a 16-month-old boy in Painesville, Ohio died after becoming entangled in the exposed cord on the backside of a Roman shade and strangled. The child was found face down under a corner of the shade pulled into his crib.
Description: This recall involves Roman shades distributed by All Strong Industry that are made of a white-on-white striped fabric and measure about 64 inches long and the width ranges between 23 and 39 inches. The head rail has a strangulation warning and a label stating "CF" (Ching Feng Fashion Home Fashions Co., Ltd.), "52% Cotton/48% Polyester", and made in either Taiwan, Thailand or China. "RN 103984" is printed on another sewn-in tag near the bottom back side of the fabric.
Sold at: Walmart stores nationwide and other retailers from January 2003 through May 2005 for between $10 and $20.
Manufactured in: Taiwan, Thailand, and China
Remedy: Consumers should stop using the recalled Roman shades immediately and contact the Window Covering Safety Council for a free repair kit.

Hazard:
Roll-Up Blinds: Strangulations can occur if the lifting loops slide off the side of the blind and a child's neck becomes entangled on the free-standing loop or if a child places his/her neck between the lifting loop and the roll-up blind material.
Roman Shades: Strangulations can occur when a child places his/her neck between the exposed inner cord and the fabric on the backside of the blind or when a child pulls the cord out and wraps it around his/her neck.
Incidents/Injuries: No incidents or injuries have been reported involving Roman shades directly sold by Walmart. However, CPSC is aware of the death of a child from one of these Roman shades sold by another retailer. No incidents or injuries have been reported in roll-up shades.
Description: This recall involves all styles and sizes of fabric, bamboo and jute roll-up blinds and Roman shades.

Units: About 26,000 in the United States and 2,100 in Canada

Hazard: The control handle of the log splitter could fail to automatically return to the neutral position as it should and could fail to stop the splitting wedge from moving forward, posing a risk of amputation to consumers' hands and fingers.
Incidents/Injuries: None reported.
Description: The following log splitter brand names and model numbers are included in this recall. They were sold in red, black and yellow in 21, 25, 27 and 33 ton models. Adventure Playsets Recall to Repair Backyard Swing Sets Due to Fall Hazard
The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following products. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Product: Adventure Playsets Wooden Play Sets
Units: About 275,000 in the United States and 6,800 in Canada
Distributor: Adventure Playsets, of Amarillo, Texas
Hazard: The plastic coated lumber on the horizontal ladder (monkey bar/swing beam) can weaken over time due to rotting of the whitewood (spruce, pine and fir species), resulting in a fall hazard.
Incidents/Injuries: Adventure Playsets has received more than 1,400 reports of rotting ladders involving 16 injuries that resulted in nine emergency room visits. The injuries include two arm fractures, lacerations, scrapes, and bruises to children younger than 8 years old. Most of the reported injuries occurred when the swing came out of the monkey bar/ swing beam that had rotted.
Description: This recall involves wooden play sets with swings, slides and ladders. Each set has an overhead monkey bar ladder that acts as both the monkey bar and swing beam, and an end ladder coated with cranberry or green plastic. The instruction manual has the name "Adventure Playsets" and one of the following model numbers printed on the cover.

Wal-Mart Announces Recall Expansion of Durabrand DVD Players Due to Fire Hazard

Name of product: Durabrand DVD Players. (The August 20 recall of the silver Durabrand DVD player has been expanded to include the pink- and purple-colored versions of the same DVD player.)

Hazard: The DVD player's circuit board can overheat, posing a fire and burn hazard to consumers.

Kolcraft Recalls 1 Million Play Yards Due to Fall Hazard

Name of Product: Kolcraft, Carter's, Sesame Street, Jeep, Contours, Care Bear and Eric Carle Play Yards

Distributor: Kolcraft Enterprises Inc. of Chicago, Ill.
Hazard: The play yard's side rail can fail to latch properly and when a child pushes against the rail it can unlatch unexpectedly, posing a fall hazard to children.
Incidents/Injuries: The firm has received 347 reports of sides of the play yard collapsing unexpectedly, resulting in 21 injuries to young children, including bumps, scrapes, bruises and one concussion.
Description: This recall involves the Kolcraft Travelin' Tot play yards, as well as other similar play yards manufactured for Carter's, Sesame Street, Jeep, Contours, Care Bare and Eric Carle, with model numbers listed below. Some of the units have a bassinet and/or changing table. Some models have a mobile, vibrating unit or a parent organizer. Model numbers are printed on a white sticker on one of the feet of the play yard.

Ralcorp Frozen Bakery Products Recalls Wal-Mart Bakery Brand Peanut Butter Cookies Because Of Possible Health Risk
Ralcorp Frozen Bakery Products, Inc. announced that it has taken the precautionary measure of voluntarily recalling all Wal-Mart Bakery brands of PEANUT BUTTER COOKIES, PEANUT BUTTER NO-BAKE COOKIES and PEANUT BUTTER FUDGE NO-BAKE COOKIES because they have the potential to be contaminated with Salmonella. The cookies contain peanut butter supplied by Peanut Corporation of America which is the subject of an FDA investigation concerning recent Salmonella outbreaks. No illnesses have been reported in connection with the recalled cookies and no other type of Wal-Mart Bakery brand cookies are being recalled.
The recalled peanut butter cookies are distributed nationally under the Wal-Mart Bakery cookies brand. The recalled Wal-Mart Bakery no-bake peanut butter cookies and no-bake fudge peanut butter cookies were distributed nationally with the exception of the following states: Wisconsin, Michigan, Minnesota, Illinois, Indiana, Ohio, Pennsylvania, Delaware, New Jersey, New York, Rhode Island, Connecticut, Massachusetts, Vermont, New Hamshire and Maine. The cookies are sold in the in-store bakery sections of only Wal-Mart stores and have a code date of 9200 or less.

Salmonella is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. CSI USA, Inc. Issues Voluntary National Recall of Topical Acne Cream
CSI, USA, Inc. issued a voluntary nationwide consumer product recall of all lots of 1 ounce (28 g) tubes of 10% Benzoyl Peroxide Acne Cream with the following names: "DG Maximum Strength Acne Medicated Gel" (sold at Dollar General); "Kroger Acne Gel 10% Benzoyl Peroxide Acne Medication" (sold at Kroger); and "Equate: Medicated Acne Gel" (sold at Wal-Mart).
These products are being recalled because samples of the products were found to contain bacteria identified as "Burkholderia Cepacia," formerly known as Pseudomonas Cepacia.
After conducting a thorough assessment, the company concluded that the risk of illness in healthy individuals following use of this product is very low. However, there could be an increased health risk of infections for individuals with cuts, scrapes, rashes or other compromised skin conditions; or those with weakened or suppressed immune systems.
No consumer adverse health events related to this issue have been reported.
No other acne medications sold by Dollar General, Kroger or Wal-Mart are affected by this recall.
Consumers should discontinue using the product and may obtain a full refund by mailing the tube, or proof of purchase, to:

FDA Reports Nationwide Recall of Mislabeled ReliOn Insulin Syringes
The U.S. Food and Drug Administration is notifying health care professionals and patients that Tyco Healthcare Group LP (Covidien) is recalling one lot of ReliOn sterile, single-use, disposable, hypodermic syringes with permanently affixed hypodermic needles due to possible mislabeling. The use of these syringes may lead to patients receiving an overdose of as much as 2.5 times the intended dose, which may lead to hypoglycemia, serious health consequences, and even death.
The recall applies to the following lot number and product information:
-- Lot Number 813900
-- ReliOn 1cc, 31-gauge, 100 units for use with U-100 insulin
Only ReliOn syringes from this lot number and labeled as 100 units for use with U-100 insulin are the subject of the recall.
These syringes are distributed by Can-Am Care Corp and sold only by Wal-Mart at Wal-Mart stores and Sam's Clubs under the ReliOn name. Wal-Mart requests that all users of ReliOn 31-gauge, 1cc syringes return those labeled as 100 units for use with U-100 insulin from Lot Number 813900 to their local Wal-Mart store or Sam's Club pharmacy. Customers will be provided with replacement product.
The FDA urges patients and health care professionals to check their syringe packaging carefully for syringes labeled as 100 units for use with U-100 insulin from Lot Number 813900.
Consumers and health care professionals who suspect they have the recalled product may also contact Covidien at 866-780-5436 or www.relion.com/recall for more information.
ReliOn Insulin Syringes consist of a syringe barrel, a plunger rod, and a hypodermic needle attached to the tip of the syringe.
During the packaging process for this lot, some syringes labeled for use with U-40 insulin were mixed with syringes labeled for use with U-100 insulin, then all packaged individually and in boxes as 100 units for use with U-100 insulin.
The manufacturer has distributed 4,710 boxes in the recalled lot, which equals 471,000 individual syringes. Wal-Mart sold the syringes at Wal-Mart stores and Sam's Clubs from Aug. 1, 2008, until Oct. 8, 2008.
Tyco Healthcare Group LP (Covidien) voluntarily recalled this lot of syringes on Oct. 9, 2008, asking that any units of the affected product be removed from inventory and placed in quarantine. Wal-Mart posted the recall announcement in Wal-Mart stores and Sam's Clubs, as well as on its Web site, and sent letters to more than 16,500 customers notifying them of the recall.
The manufacturer has received one adverse report related to a syringe from this product lot.
Health care professionals and consumers may report serious adverse events (side effects) or product quality problems with the use of this product to the FDA's MedWatch Adverse Event Reporting program either online, by regular mail, fax or phone.

Mars Petcare US Issues Voluntary Recall of a Limited Number of Bags of SPECIAL KITTY® Gourmet Sold at Wal-Mart locations in Fifteen States due to Pote
Mars Petcare US today announced a voluntary recall of a limited number of bags of SPECIAL KITTY® Gourmet Blend dry cat food sold at Wal-Mart locations in Connecticut, Delaware, Massachusetts, Maryland, Maine, North Carolina, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode Island, Virginia, Vermont, and West Virginia. The pet food is being voluntarily recalled following a positive test result indicating a potential contamination with Salmonella.
This product should not be sold or fed to pets. Pet owners should dispose of product in a safe manner (example, a securely covered trash receptacle) and return the empty bag to the store where purchased for a full refund.
Salmonella can cause serious infections in dogs and cats, and, if there is cross contamination caused by handling of the pet food, in people as well, especially children, the aged, and people with compromised immune systems. Healthy people potentially infected with Salmonella should monitor themselves for some or all of the following symptoms: nausea, vomiting, diarrhea or bloody diarrhea, abdominal cramping and fever. On rare occasions, Salmonella can result in more serious ailments, including arterial infections, endocarditis, arthritis, muscle pain, eye irritation, and urinary tract symptoms. Consumers exhibiting these signs after having contact with this product should contact their healthcare providers.
Pets with Salmonella infections may be lethargic and have diarrhea or bloody diarrhea, fever, and vomiting. Some pets will have only decreased appetite, fever and abdominal pain. Animals can be carriers with no visible symptoms and potentially infect other animals or humans. If your pet has consumed the recalled product and has these symptoms, please contact your veterinarian.
There have been no complaints or reports of illness resulting from consumption or handling of the recalled product.
