Walmart Recalls GE Food Processors Due to Laceration and Fire Hazard

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: General Electric(r) Food Processors

Units: About 255,000

Importer: Walmart Stores Inc., of Bentonville, Ark.

Hazard: The safety interlock system on the recalled food processor can fail; allowing operation without the lid secured which poses a laceration hazard. In addition, the product can emit smoke, or catch fire, posing a fire hazard.

Incidents/Injuries: Walmart has received a total of 58 incident reports: 24 reports of the food processor operating without the lid in place, of which 21 resulted in injuries to fingertips; and 34 reports of the unit smoking, including 3 reports of fires.

Description: This recall involves GE-branded digital, 14-cup food processors. The food processors are black with stainless steel trim, and model number 169203 is imprinted on the underside of the unit.

Sold exclusively at: Walmart stores nationwide and Walmart.com from September 2009 through February 2011 for a retail price of about $50.

Manufactured in: China

Remedy: Consumers should immediately stop using the recalled food processor and return the product to any Walmart for a full refund.

Gorilla Inc. Recalls EXO-Tech Safety Harness Due to Fall Hazard

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: EXO-Tech Safety Harness

Units: About 90

Manufacturer: Gorilla Inc., of Flushing, Mich.

Hazard: The webbing of the waist belt on the safety harness is not routed through the lineman's loop located on the front of the harness near waist level. Since the loops are not properly anchored to the harness webbing but are attached only through stitching not intended to restrain a user during a fall, they that can pull away from the harness when force is applied, leaving the user unrestrained. In addition, the manufacturer of the harness used a previously untested carabiner connector located at the end of the tether at the back of harness, which is the portion of the tether that attaches to the tree.

Incidents/Injuries: No incidents were reported.

Description: This safety harness, used as a fall restraint for hunting, is comprised of two leg straps and two shoulder straps, which connect to a waist belt and padded back support. There is a long, black tether strap at the top rear of the safety harness, which has grey and red accents. The name EXO-Tech is located on the right front shoulder strap and the name Gorilla is located on the left front should strap both in white lettering. Similar to a shirt tag, there is a white manufacturing label on the inside of the back of the harness with the model number 45111 and manufacturing dates, 4/22/2009 or 6/26/2009. These are the only harnesses recalled.

Sold at: Cabelas, Bass Pro and at various sporting goods stores nationwide from May 2009 to August 2009 for about $200.

Remedy: Consumers should immediately stop using the harness and contact Gorilla Inc. to receive a refund.

Yamaha Motor Corp. Offers Free Repair For 450, 660, and 700 Model Rhino Vehicles

Yamaha Motor Corp. Offers Free Repair For 450, 660, and 700 Model Rhino Vehicles: CPSC advises consumers not to use the off-road vehicles until repaired


The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Yamaha Motor Corp. U.S.A., of Cypress, Calif., is announcing a free repair program to address safety issues with all Rhino 450, 660, and 700 model off-highway recreational vehicles. Yamaha has also agreed to voluntarily suspend sale of these models immediately until repaired. Consumers should immediately stop using these popular recreational vehicles until the repair is installed by a dealer.

CPSC staff has investigated more than 50 incidents involving 46 driver and passenger deaths in these two Rhino models. More than two-thirds of the cases involved rollovers and many involved unbelted occupants. Of the rollover-related deaths and hundreds of reported injuries, some of which were serious, many appear to involve turns at relatively low speeds and on level terrain.

About 120,000 of the 450 and 660 model Rhinos have been distributed nationwide since Fall 2003. Some units have been equipped by Yamaha with half doors and additional passenger handholds, either before or after sale.

Yamaha's repair includes the installation of a spacer on the rear wheels as well as the removal of the rear anti-sway bar to help reduce the chance of rollover and improve vehicle handling, and continued installation of half doors and additional passenger handholds where these features have not been previously installed to help keep occupants' arms and legs inside the vehicle during a rollover and reduce injuries. Owners of the affected Rhinos should stop using them and call their dealer to schedule an appointment to have repairs made once they are available and to take advantage of a free helmet offer.

Yamaha is also voluntarily implementing the same repair program and suspension of sale for the Rhino 700 model, in order to ensure customer satisfaction. Consumers should stop riding the 700 model until it is repaired. About 25,000 Rhino 700s are part of this repair program.

Once these repairs have been made to their vehicles, Rhino users should always wear their helmet and seatbelt and follow the safety instructions and warnings in the on-product labels, owner's manuals and other safety materials. The Rhino is only recommended for operators 16 and older with a valid driver's license. All passengers must be tall enough to place both feet on the floorboard with their back against the seat back.

Watson Announces a Nationwide Voluntary Recall of Propafenone HCL Tablets Due to Oversized Tablets

Watson Pharmaceuticals, Inc. (NYSE: WPI), a leading specialty pharmaceutical company, announced today that one lot of Propafenone HCL 225 mg Tablets sold in 100 count bottles in the United States is being voluntarily recalled to the consumer level as a precautionary measure. The lot is being recalled because some tablets may contain slightly higher levels of the active ingredient than specified. The recalled tablets were manufactured by Watson Laboratories, Inc. The affected lot of Propafenone HCL tablets was shipped to customers between October 15, 2008 and November 26, 2008. No other strengths or lots were affected and the Company does not anticipate any product shortages as a result of this recall. The Company has notified the U.S. Food and Drug Administration (FDA) of the recall.

Propafenone is a drug product used to treat cardiac arrhythmias (irregular heartbeats). It has a narrow therapeutic index, so some patients who are particularly sensitive to small variations in dose may experience potentially serious side effects, including arrhythmias (irregular heartbeat) or low blood pressure. Consequently, as a precautionary measure, Watson is recalling this one lot to the consumer level to minimize any potential risk to patients.

Anyone who has 225 mg Propafenone HCL tablets should check the bottle for the lot number and expiration date to see if they have tablets being recalled. Anyone with 225 mg Propafenone HCL tablets being recalled should call (888) 352-9616, Monday through Friday, 8:00 a.m.-5:00 p.m. EDT, for instructions on how to return affected product.

Patients using 225 mg Propafenone HCL tablets who have medical questions should contact their health-care provider.

Infant Death Prompts Recall of Convertible Cribs by Playkids USA; Crib Poses Entrapment and Suffocation Hazards

The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Playkids U.S.A. of Brooklyn, N.Y. is announcing today a voluntary recall of about 2,000 convertible cribs. The sides of the convertible crib are made of a mesh that expands, creating a gap between the mattress and the side through which an infant can slip. This poses suffocation and entrapment hazards for young children.

CPSC is aware of the death of a 5-month-old child on August 31, 2008 in Brooklyn, N.Y. The child became entrapped between the mattress and the drop side rail of the convertible crib and suffocated.

This recall involves the Playkids U.S.A. convertible crib/playpen/bassinet/bed with model number PLK-909. "Playkids U.S.A." can be found on the packaging and on a label sewn into the side of the crib. The model number can be found on the packaging. The convertible cribs have a drop side rail, a stationary side rail, a canopy assembly, and a bassinet. The sides of the convertible crib, the mattress support, the bassinet, the canopy and the bedskirt are covered in fabric and mesh. The fabric and the mesh come in a variety of colors and patterns.

The convertible cribs, which were made in China, were sold in juvenile product retailers in New York from March 2007 through September 2008 for about $100.

Consumers should stop using the recalled convertible cribs immediately and contact Playkids USA to receive a full refund.

The Wick & Petal Co. Recalls Candles Due to Fire Hazard

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Jar candles
Units: About 1,300 Manufacturer: The Wick & Petal Co. of Indianapolis, Ind.
Hazard: The candles can burn with a high flame, posing a fire hazard.
Incidents/Injuries: Wick & Petal has received seven reports of the candles burning with high flame, some resulting in minor property damage. No injuries have been reported.
Description: This recall involves all Wick & Petal jar candles. The candles have a cylindrical shape and a chrome lid. The lid has a white label with a picture of the fragrance, the fragrance name, and "Wick &Petal Co." printed on it.
Sold at: Select candle retailers and craft fairs in Indiana from October 2007 through December 2007 for between $8 and $18.
Manufactured in: United States
Remedy: Consumers should immediately stop using the recalled candles and return them to the place of purchase for a full refund.
