Defibtech Announces a Voluntary Recall of DDU-100 series AEDs

Defibtech, LLC is initiating a worldwide voluntary recall of certain DDU-100 series semi-automatic external defibrillators (AEDs) sold under the Lifeline AED and ReviveR AED brand names, including 65,885 AEDs distributed in the United States. This recall affects only DDU-100 Series AEDs shipped with 2.004 software or earlier. This corrective action addresses two possible conditions, which in rare cases may cause an affected AED to cancel shock during the charging process and not provide therapy which may result in failure to resuscitate the patient. Both conditions are not detectable by the periodic self test.

Condition 1: In rare instances, the AED may cancel charge in preparation for a shock. Based on field data, the odds of an affected AED having this happen are less than a 1 in 400,000 chance per month for any given AED.

A subset of AEDs (less than 11%) that are affected by Condition 1 are also affected by Condition 2: In rare instances, the AED may cancel charge in preparation for a shock in very high humidity conditions. The only reported cases were in environments of greater than 95% relative humidity or condensing conditions. Based on field data, the odds of an affected AED having this happen are less than a 1 in 250,000 chance per month for any given affected AED.

Defibtech will provide customers with a free software upgrade to address these issues. The correction to the AED will be able to be performed at the location where the AED is deployed.

Because both of these conditions occur very rarely, it is recommended that customers keep their AEDs in service until they have performed the software upgrade. The AEDs affected by this recall have been distributed globally to fire departments, EMS, health clubs, schools, and other organizations. Hoover Recalls WindTunnel Canister Vacuums Due to Fire and Shock Hazards

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Hoover® WindTunnel Canister Vacuums

Units: About 142,000

Importer: Hoover Inc., of Glenwillow, Ohio

Hazard: The power cord between the power nozzle and the wand connector can short-circuit posing fire and shock hazards to consumers. This condition can occur even if the vacuum has been turned off but left plugged in.

Incidents/Injuries: Hoover has received 69 reports of overheating or electrical malfunction, including one report of fire and smoke damage, and two reports of carpet damage. There has been one report of a minor injury.

Description: This recall involves the Hoover WindTunnel Bagless Canister Vacuum model S3755. The vacuum is silver and black in color, and comes with a power nozzle. The model number can be found on a label on the bottom of the canister.

Sold at: Mass merchandisers, department stores and independent vacuum retailers nationwide and online from March 2003 to December 2008 for between $250 and $280.

Manufactured in: China

Remedy: Consumers should immediately stop using the recalled vacuum cleaners and contact Hoover for a free repair.

Pottery Barn Recalls Lamps Due to Shock Hazard

Name of Product: Pottery Barn Lamps

Units: About 7,300

Importer: Pottery Barn, of San Francisco, Calif.

Hazard: The electrical wire that runs through the lamps can be pinched or severed at the lamp's adjustable joint, posing a risk of electric shock to consumers.

Incidents/Injuries: Pottery Barn has received one report of a consumer who received an electrical shock when she touched the lamp.

Description: This recall involves Pottery Barn's Clay Task Lamp (Model #2467553), the Montgomery Task Lamp (Model #9691783) and the Montgomery Floor Lamp (Model #9691775). All of the lamps have bronze-colored finishes. "Pottery Barn" and the model number are printed on the bottom of the lamp.

Sold at: Pottery Barn stores nationwide, in Pottery Barn's catalog and on the firm's website www.potterybarn.com from July 2009 through August 2010 for between $60 and $300.

Remedy: Consumers should immediately stop using the recalled lamps, unplug them and contact Pottery Barn for instructions on how to return the lamp for a full refund.

Hazard: The product label can lose adhesion, exposing the circuitry. This could pose a shock hazard to consumers.

Description: This recall involves the Pumptec electronic motor controller for 115v/230v submersible motors rated 1/3 to 1 1/2 HP with model number 5800020600 and date codes of 06F45 through 09L45. OSRAM SYLVANIA Recalls Portable Nightlights Due to Electric Shock Hazard

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: LED Rocketship PalPODzzz(tm) Portable Nightlights

Units: About 26,000

Importer: OSRAM SYLVANIA Products Inc., of Danvers, Mass.

Hazard: The bottom plastic cover on the recharging base of the portable nightlight can break, exposing internal electrical components. This poses an electric shock hazard to consumers.

Incidents/Injuries: The firm has received three reports of the bottom of the recharging base breaking and consumers touching internal electrical components that resulted in minor electric shocks.

Description: This recall involves LED Rocketship PalPODzzz(tm) portable nightlights with model number 72174. The product can also be used as an emergency light or a flashlight. The nightlight is shaped as a rocket ship and sits in a plastic recharging base that plugs into the wall. The model number and date codes "0808" or "0908" are printed on the label attached to the bottom cover of the recharging base.

Sold at: The Home Depot, Stop & Shop, and other retailers nationwide and online at Amazon.com, Smarthome.com, and Sylvaniaonlinestore.com from October 2008 through November 2009 for between $15 and $20.

Incidents/Injuries: Sony has received four reports of adapters short circuiting. No incidents occurred in the United States. No injuries have been reported.

Single Meter Sockets Recalled by Milbank Manufacturing Due to Fire and Electrocution Hazards

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Single Meter Sockets

Units: About 1,400

Manufacturer: Milbank Manufacturing Co., of Kansas City, Mo.

Hazard: A short may occur while in use due to an incorrect bridge installed in the product, to which the meter clips are attached. If the manufacturing defect exists, all metal parts of the meter could create a shock or burns can occur if the cover is off and the meter socket is energized.

Incidents/Injuries: Three incidents have been reported of the unit shorting out. No injuries were reported.

Description: The single meter 200 amp/4 terminal sockets are used for underground utility meter installations. The meter socket is 20 3/4 inches high by 9 inches wide and 4 1/2 inches deep with 1 meter position. It is used by the approving utility to mount their electric meter to measure how much electricity a residence uses. It can be used for an underground residential application. The recalled model number 9090 appears on the right or left side of the installed meter socket. If not installed, model numbers U9090-O or R9090-O will appear on the label on the outside of the box.

Sold at: Electrical supply distributors in N.Y., Pa., Vt. and Mass. and at local hardware stores in N.Y. from May 2009 through August 2009 for about $70.

Name of Product: Intermatic DT17 Heavy Duty Digital Timers

Hazard: The recalled timers can have a faulty ground connection, posing a shock hazard to consumers.

Incidents/Injuries: None reported.

Description: The recalled Intermatic DT17 heavy-duty digital timers are lamp and appliance timers. They were packaged as the "DT17C Heavy Duty Digital Timer" and the "DT17C8 Heavy Duty Digital Timer." The timer is white-colored and measures 3 3/8" tall x 2 5/8" wide x 1 5/8" deep. Only products with date codes of "40Z" through "52Z" or date codes ending in "A," "B" or "C" are included in the recall. The brand name "Intermatic" is molded on the front of the timer, and the model number ("DT17") and date code are printed on the back of the timer.

Sold at: Retailers and electrical distributors nationwide from September 2005 through December 2008 for between $15 and $25.

Fujifilm Recalls Battery Chargers Due to Shock Hazard

Name of Product: BC-50 Battery Chargers

Hazard: The plastic portion of the unit's power plug can crack or detach, posing an electrical shock hazard to consumers.

Description: The recalled BC-50 battery chargers were included with the Fujifilm FinePix F100fd digital cameras and are identified by the production lot codes "WCAA" and "WCAB", which can be located on the back of the battery charger unit, in the lower, right-hand corner. The battery chargers were also sold separately as an optional accessory for the FinePix F100fd and F50fd digital cameras and have production lot codes of "WBAD" and "WFBA". Battery chargers packaged with the Fujifilm Finepix F50d digital cameras are not included in the recall.

Sold at: Camera and photo supply retailers nationwide from March 2008 through September 2008 for about $60.
