Chef Pierre® is voluntarily recalling a partial production run of the following foodservice product as a precautionary measure as the product may contain milk, an undeclared allergen, posing a potential serious and life-threatening health risk to milk-allergic individuals. This recall affects 4,380 cases of Chef Pierre Gourmet Lemon Meringue Pie labeled product that actually contained chocolate meringue pie.
The item included in the voluntary recall is part of a partial production run only for Chef Pierre Lemon Meringue Pie with the UPC CODE 3210009293 AND a code of 9159 printed on the top of the pie label directly under the Chef Pierre logo.
No other Chef Pierre branded products are affected by this voluntary recall. Only packages labeled as Chef Pierre Lemon Meringue Pie with the UPC CODE 3210009293 AND a code of 9159 printed on the top of the pie label directly under the Chef Pierre logo is part of this recall.
This product was sold and distributed nationally to foodservice companies and sold to consumers through foodservice retail outlets and other foodservice establishments.
Chef Pierre has received no reports of illnesses or allergic reactions.
This recall is being conducted in conjunction with the Food and Drug Administration (FDA).
Consumers may return affected product to the store or operator where it was purchased for a full refund. Hazard: The cotton hoodie has a waist drawstring that could pose an entrapment hazard to children. In February 1996, CPSC issued guidelines (pdf) to help prevent children from strangling or getting entangled on the neck and waist drawstrings in upper garments, such as jackets or sweatshirts.
Incidents/Injuries: No injuries have been reported.
Description: The cotton hoodie, which zips in front and has a drawstring at the waist, came in gray with blue lining and cherry with lavender lining. They were sold in girls' sizes SML-XL. ROXY is printed on the front of the gray hoodie and on the neck label on both colors. The style number, B487857, is printed on the inside care label near the waist.

Risk of Strangulation Prompts Recall to Repair Roller Shades by Lutron Shading Solutions

Hazard: Strangulations can occur if the shade's looped bead chain is not attached to the wall or the floor and a child's neck becomes entangled on the free-standing loop.

Description: This recall involves custom-ordered made-to-measure manual roller shades. The shades were sold in a variety of colors, fabrics, and sizes. They have a label on an adhesive strip that connects the shade fabric to the roller tube that reads "Lutron Shading Solutions" and "800-446-1503." The shade needs to be removed from the brackets and unrolled completely to access the label. The shade was sold with an orange, black and white hang tag warning consumers that "young children can STRANGLE in cord and bead chain loops." However, the hang tag may have been discarded upon installation. Nearly all shades were professionally installed.
Sold at: Specialty dealers and Expo Design Centers nationwide from January 2000 through April 2009 for between $200 and $2000 per shade.

Strangulation Death of a Child Prompts Recall To Repair Window Blinds By Vertical Land

Hazard: Horizontal Blinds: The blinds do not have inner cord stop devices to prevent the accessible inner cords from being pulled out. If an inner cord is pulled out, a child can become entangled in the loop and strangle.
Vertical Blinds: Strangulations can occur if a child's neck becomes entangled on the free-standing loop formed by the bead chain or by the cord with a weighted device.
Cellular Shades: Strangulations can occur if a child's neck becomes entangled on the shade's free-standing looped cord.
Incidents/Injuries: In May 2006, CPSC received a report of a 4-year-old girl from Pensacola, Fla., who strangled in the loop of a vertical blind cord that was not attached to the wall or floor.
Description: This recall involves all Vertical Land custom-made vertical and cellular window coverings that do not have a cord tensioning device and horizontal blinds that do not have inner cord stop devices. A sticker identification label can be found on the head rail of some of the window coverings which lists the company's name, address and telephone number.
Sold at: Vertical Land stores in Panama City and Pensacola, Fla. from January 1992 through December 2006 for between $60 and $200 (depending on the size of the blind).

The widths of the Matchstick roman shades are 23, 27, 31, 35 and 36 inches and are brown.

Hazard: Strangulations can occur when a child places his/her neck between the exposed inner cord and the fabric on the backside of the blind or when a child pulls the cord out and wraps it around his/her neck.
Incidents/Injuries: There have been six reports of children becoming entangled in the exposed inner cord on the back of the shade between March 2006 and July 2008. Four children were found with cords entangled around their necks. Scissors were used to release two of these children. Two children were found with red marks around their necks after having freed themselves. No permanent injuries were sustained.
Description: This recall involves all sizes and colors of Pottery Barn Kids roman shades with exposed inner cords on the back. Two sewn-in labels are located at the bottom edge of the shade. The smaller label identifies the manufacturer as "pottery barn kids" while the second larger orange, black and white label warns that "Cords and bead chains can loop around child's neck and STRANGLE." Roman shades with a fabric covering over the cords are not being recalled.

The Roman blind has a continuous looped bead chain that when not attached to the wall or floor, hangs loosely by the blind, posing a strangulation hazard to children.
Incidents/Injuries: CPSC and IKEA received a report of a 2-year-old boy who suffered a near strangulation. His mother found him hanging from the looped bead chain.
Description: This recall includes all sizes/colors of MELINA Roman blinds. These blinds have a sewn-in label at the top edge of the blind with the IKEA logotype, article name (MELINA), 5-digit supplier number 19395, four digit date stamp (YYWW) and the words "Made in Taiwan". On the bottom edge of the blind there is a sewn-in orange/white safety warning label. The blinds are made from 100% polyester.
Sold at: IKEA stores nationwide from August 2006 through June 2008 for between $20 and $40.

Strangulation Deaths of Children Prompt Recall to Repair of Roll-Up Blinds and Roman Shades by Lewis Hyman Inc.
The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Product: ¼" Oval Roll-up Blinds and Woolrich Roman Shades
Units: About 4.2 million roll-up blinds and 600,000 Roman shades
Importer: Lewis Hyman Inc., of Carson, Calif.
Hazard: Roll-up Blinds: Strangulations can occur if the lifting loops slide off the side of the blind and a child's neck becomes entangled on the free-standing loop or if a child places his/her neck between the lifting loop and the roll-up blind material.
Roman Shades: Strangulations can occur when a child places his/her neck between the exposed inner cord and the fabric on the backside of the blind or when a child pulls the cord out and wraps it around his/her neck.
Incidents/Injuries: In November 2007, a 1-year-old boy from Norridgewock, Maine became entangled and strangled in the lift cord loop of a roll-up blind that had fallen into his portable crib. In October 2008, a 13-month-old boy from Conway, Ark. was found with his head between the exposed inner cord and the cloth on the backside of a Roman shade. The cord was not looped around the boy's neck but rather ran from ear to ear and strangled the child.
Description: This recall involves roll-up blinds without release clips (see picture below) and all Woolrich Roman shades. The roll-up blinds have plastic oval-shaped slats that measure about ¼ inch tall. The blinds measure either 72" or 96" long. The bottom rail has a WARNING label advising that "Young children can become entangled and strangle in cord or bead loops" and a label that reads "Lewis Hyman, Inc." and the year of manufacture. Roll-up blinds that have release clips right below the head rail on the backside of the blind are not included in this recall.
The Woolrich Roman shades come in twill fabric and micro-suede fabric and measure 72" long. The head rail has two labels that read "Lewis Hyman, Inc., www.lewishymaninc.com" and "LHI, 005301, Made in China" respectively.
Sold at: The Roman shades were sold exclusively at Target stores nationwide and on Target.com from March 2006 through December 2008 for between $25 and $43. The roll-up blinds were sold at retail stores nationwide from January 1999 through December 2003 for between $6 and $20.
Manufactured in: China
Remedy: Consumers should immediately check the backside of the roll-up blinds to determine if they have release clips. If the roll-up blind does not have release clips, stop using it immediately and contact Lewis Hyman for a free repair kit.

Maytag Expands Recall of Refrigerators Due to Fire Hazard
/PRNewswire/ -- The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product.
Name of Product: Maytag(R), Magic Chef(R), Performa by Maytag(R) and Crosley(R) brand refrigerators
Units: About 46,000 (1.6 million units were previously recalled in March 2009)
Manufacturer: Maytag Corp., of Newton, Iowa
Hazard: An electrical failure in the relay, the component that turns on the refrigerator's compressor, can cause overheating and pose a serious fire hazard.
Incidents/Injuries: Maytag has received 23 additional reports of refrigerator relay ignition, including 4 reports of property damage ranging from smoke damage to extensive kitchen damage.
Description: The recall includes certain Maytag(R), Magic Chef(R), Performa by Maytag(R) and Crosley(R) brand side-by-side and top freezer refrigerators. The affected refrigerators were manufactured in black, bisque, white and stainless steel. They have model and serial numbers printed on a label located on the top middle or left upper side of the refrigerator liner and have the following model and serial number combinations:

Kroger Recalls Super Kids Enriched White Sandwich Bread Due to Undeclared Milk in the Product
The Kroger Co. said today it is recalling Kroger brand Super Kids Enriched White Sandwich Bread sold in 13 states because it may contain milk not listed on the label. Customers should return the product to stores for a refund or replacement.
People who are allergic to milk could have a serious or life-threatening reaction if they consume this product. For consumers who are not allergic to milk, there is no safety issue with the product.
Kroger stores in the following states are included in this recall: Alabama, Georgia, Illinois, Indiana, Kentucky, Michigan, Missouri, North Carolina, Ohio, South Carolina, Tennessee, Virginia and West Virginia. Kroger stores in Arkansas, Louisiana, Mississippi and Texas are not included in the recall.
The recall also includes Jay C Food stores in Indiana and Food 4 Less stores in Illinois and Indiana. Other stores Kroger operates under different banner names are not included in this recall.

Nutracoastal Trading LLC Expands Its voluntary nationwide recall of Steam Dietary supplement
Nutracoastal Trading LLC announced today that it is expanding its July 28th, 2009 voluntary nationwide recall of the company's dietary supplement product sold under the following name: STEAM.
The Company has found by lab analysis that Lot 90260 contains Tadalafil, an active ingredient of an FDA-approved drug for erectile dysfunction (ED), making STEAM DIETARY SUPPLEMENT an unapproved drug. The active drug ingredient is not listed on the product label. The undeclared ingredient may interact with nitrates found in some prescription drugs such as nitroglycerin and may lower blood pressure to dangerous levels. Consumers with diabetes, high blood pressure, high cholesterol, or heart disease often take nitrates. Additionally, the product may cause side effects, such as headaches and flushing.
The recalled product listed below was distributed in white plastic bottles to retail stores nationwide.

Two Additional Infant Deaths Prompt Re-announcement of Simplicity "Close-Sleeper/Bedside Sleeper" Bassinets
Consumers urged to immediately stop using the 3-in-1 and 4-in-1 bassinets, including those with Graco logo and "Winnie the Pooh" motifs
The U.S. Consumer Product Safety Commission (CPSC) is once again urging all parents and caregivers to immediately stop using convertible "close-sleeper/bedside sleeper" bassinets manufactured by Simplicity Inc., of Reading, Pa. CPSC has learned of two additional infant deaths since the August 2008 safety alert and recall announcements that notified consumers of two infants who had strangled in their Simplicity bassinets. To date, CPSC is aware of four infants who have died in the close-sleeper bassinets.
CPSC learned that in September 2008, a two-month old female in Demorest, Georgia suffocated when she rolled into the adjustable fabric siding of the bassinet, pressing her face into a "pocket" which formed near the Velcro© fasteners. In another incident in January 2009, a six-month old female in Fort Worth, Texas appears to have been fatally entrapped in the bassinet's bar opening created when the Velcro© fasteners were not secured. An investigation into this infant's death is ongoing.
In addition, CPSC has received two separate reports involving a 10-week old and a 3-month old whose heads became entrapped between the lower bassinet bar and the mattress support. The infants were freed by their caregivers without injury.
The Simplicity 3-in-1 and 4-in-1 convertible bassinets, including bassinets with the Graco logo and "Winnie the Pooh" motif licensed by Disney Consumer Products, contain metal bars that are covered by an adjustable fabric flap. This fabric flap is secured by Velcro© fasteners, which are intended to be partially removed when the bassinet is converted into the bedside/close-sleeper mode. If the Velcro© fasteners are not properly re-secured when the fabric flap is adjusted, or if the consumer entirely removes the fabric flap exposing the metal bars, an infant can slip through the opening between the lower metal bar and the mattress support and become entrapped and suffocate or strangle. Consumers are urged to check their bassinets to ensure they are not using one of the recalled Simplicity bassinets.
Parents and caregivers are advised to return these bassinets to the retail store where they were purchased. All of the recalled bassinets can pose a danger to infants.
To determine whether your bassinet has been recalled, please check for the model number and the manufacture date code pictured below. Bassinets manufactured prior to 21st week of 2008, "2108" (before May 18) are recalled. The model numbers for recalled bassinets manufactured prior to "2108" are:

Hazard: The bicycle stem can crack and cause the rider to lose control, posing a risk of serious injury if the rider falls.
Incidents/Injuries: Nirve Sports has received two reports of cracked stems. In both cases, the cracks occurred during assembly of the bicycle. No injuries have been reported.
Description: This recall involves Cannibal "Chopper" model bicycles with graphic designs by artist Corey Miller. The bicycles have a black matte finish. The affected bicycles have a serial number within the following range: L8E0300001 - L8E1001050. Serial numbers are located on the bottom of the bicycle between the pedals.

Hazard: The amplifiers were designed to operate at a temperature warm to the touch. However, a component input device can fail and cause the amplifiers to overheat, posing burn and fire hazards to consumers.
Incidents/Injuries: Krell has received 50 reports of the amplifiers overheating including reports of smoke and electrical fire. No injuries have been reported.
Description: This recall involves Krell power amplifiers with model numbers KAV-250a, KAV-250a/3, KAV-500i and KAV-1500. The amplifiers are used to provide power to a loudspeaker in a home audio playback system. The Krell logo is printed on the front of the amplifier.

Ridgid Table Saws Sold Exclusively at Home Depot Recalled by One World Technologies Due to Laceration Hazard

Name of Product: Ridgid 10-inch Table Saws
Units: About 3,000
Distributor: One World Technologies Inc., of Anderson, S.C.
Hazard: The table saw's arbor shaft can fail when used with a stacked blade set (commonly known as a "stacked dado set"), which is used to cut grooves. The stacked blade set can be ejected from the saw, posing a potential laceration hazard to consumers.
Incidents/Injuries: One World Technologies has received three reports of shafts failing when used with a stacked dado set. No injuries have been reported.
Description: This recall involves Ridgid 10-inch table saws, model R4511. The recalled saws have manufacturing date codes between CD0829 and CD0837. The model number and manufacturing date code are located on a metal plate on the rear of the cabinet. Products with an "Arbor Inspected" sticker directly above the plate or an orange square on the outside of the package are not included in the recall.

Burley Design Recalls Child Trailers Due to Risk of Injury
The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Products: 2009 d'lite ST and Solo ST Child Trailers
Units: About 2,700
Manufacturer: Burley Design LLC, of Eugene, Ore.
Hazard: The axle assembly's internal sleeve can loosen, causing one wheel to separate from the trailer. This poses a risk of injury to the child occupant or bike rider.
Incidents/Injuries: No incidents or injuries have been reported.
Description: The recalled products are two-wheeled child trailers designed to carry one or two children behind a bicycle. Only the 2009 d'lite ST and 2009 Solo ST trailers with serial numbers that begin with D939 or D948 are included in the recall. The serial number is located in the rear cargo area behind the seat on the lower left rear frame tube. The trailers have "d'lite ST" or "solo ST" screen printed on the cover.
Sold by: Independent bicycle retailers and specialty outdoor retailers nationwide and on various web sites from November 2008 through June 2009 for between $550 and $600.
Manufactured in: Philippines
Remedy: Consumers should stop using the trailer immediately and contact Burley to receive a free repair kit or to locate a retailer to assist with the free repair.

Black & Decker Coffeemakers Recalled By Applica Consumer Products Due to Burn Hazard

Name of Product: Black & Decker(r) Thermal Coffeemakers
Units: About 9,800
Distributor: Applica Consumer Products Inc., of Miramar, Fla.
Hazard: The coffeemakers can overheat and melt, posing a burn hazard to consumers.
Incidents/Injuries: The firm has received one report of a coffeemaker melting. No injuries reported.
Description: This recall involves Black & Decker 8-cup programmable thermal coffeemakers. Model number TCM1000IKT is printed on the rating plate on the bottom of the coffeemaker.
Sold at: Walmart and small retail stores nationwide from April 2008 through July 2009 for between $50 and $65.

Remote Switches Recalled by Woodstock International Due to Fire Hazard

Name of Product: Shop Fox Dust Collection Remote Switches
Units: About 450
Manufacturer: Woodstock International, of Bellingham, Wash.
Hazard: An incorrectly sized wire inside the remote switch can overheat, posing a fire hazard to consumers.
Incidents/Injuries: Woodstock International has received 10 reports of the remote switch overheating. No injuries were reported.
Description: The recalled product is a remote dust collector switch, which remotely turns on and off a woodworking dust collector. The product is a black electrical outlet box that comes with two hand-held remote controls. The electrical outlet box has a label that reads "Remote Dust Collector Switch". Both the electrical box and remote controls have "Shop Fox" molded into the plastic on the top side of the product. Model numbers D3038 & D3346 were printed on the carton but not on the remote control unit. The recalled switches measure 4 and 13/16" long by 3 and 7/16" wide.
Sold by: Woodworking stores nationwide from July 2008 through June 2009 for about $50.

McDonough company recalling batch of pickles
Curry & Echols, Inc. of McDonough, Georgia, is recalling some Curry & Echols “Sweeties” Bread and Butter Pickles due to jars not sealing properly.
The “Sweeties” Bread and Butter Pickles are packaged in 16-ounce glass jars with batch code 206092. The batch number is found on the bottom of the jar. This is the only batch included in this recall. The batch was only distributed in Georgia.
The recall was initiated when the company discovered the jars were not sealing properly. There have been no consumer complaints or reported problems at this time.
Consumers who have any products in this batch should contact Curry & Echols, Inc. at 678-898-3730 for a full refund.
The company is working with the Georgia Department of Agriculture to find the cause of this problem and take corrective actions.

Hospira Issues Urgent Device Recall For AC Power Cords
Hospira, Inc. (NYSE: HSP), is initiating a nationwide recall of certain Hospira devices that have defective AC power cords manufactured by Electri-Cord Manufacturing Corporation. This recall is being issued in response to customer reports of sparking, charring and fires on the plug of the power cord. Hospira's investigation of these reports determined that the power cord's prongs may crack and fail at or inside the plug. The potential risks from this power cord failure include electrical shock, delay in setup and therapy, interruption of therapy, device failure, and fires which may also occur in an oxygen-rich environment. Depending on the device and therapy, these failures may lead to potential serious injury or death. The products affected by this recall include:

Hospira will instruct users of these products to inspect and identify affected power cords on their infusion pumps, compounders, monitoring devices and printers to determine whether their product contains an affected Electri-Cord AC power cord.
Hospira is working with its customers to replace all affected power cords regardless of their condition. Hospira will begin service activity the week of Aug. 16, 2009, and will continue until all replacement activity is complete.
Users with affected power cords that have bent or cracked prongs, burnt plastic or excessive wear and tear should discontinue use immediately and contact their Hospira sales representative or Hospira Technical Support Operations at 1-800-241-4002 (available from 6 a.m. to 4 p.m., Pacific time) for instructions on receiving replacement parts or devices. Users with affected power cords that are not exhibiting any of these characteristics should monitor the power cords regularly and be mindful of excessive wear and tear, misuse or abuse until all affected cords can be replaced.
This recall is limited to device power cords with a prong and ground-pin insert design, which can be identified by a black plastic bridge connecting the terminal prongs on the plug ( purple plug with a black plastic bridge connecting the terminal prongs on the plug.)
Devices with power cords that do not have a black bridge connecting the terminal prongs on the plug are not affected (purple plug that does not have a black bridge connecting the terminal prongs on the plug.)
Electri-Cord has identified the root cause of the issue and has implemented necessary design changes to reduce the likelihood of recurrence.
Hospira has not received any reports of serious patient harm related to the situation.

Hazard: The recalled workshop sets and trucks have oversized, plastic toy nails that can pose a choking hazard to young children.
Incidents/Injuries: The firm has received one report of an 11-month old boy from Goose Creek, S.C. who choked when the toy nail become forcefully lodged in his throat. The child was hospitalized and made a full recovery.
Description: The oversized, plastic toy nail is about 3 ¼ inches long by 1 ¼ inch in diameter and comes in red or blue colors. It has a large round head; about 1 ¼ inches below the head is a plastic ridge, slightly smaller than the nail head and about 1 inch in diameter. Limited Recall of 772 Cartons of Iced Jumbo Green Onions due to Possible Health Risk
A precautionary voluntary recall of 772 cartons of iced jumbo green onions under the brand names Omo and Fu Choy distributed from three locations, R.A.M. Produce Distributors of Detroit, Michigan, Franzella Distributing of San Francisco, CA, and United Food Service of East Point, GA has been issued by California-based NewStar Fresh Foods due to the possible presence of Salmonella.
No illnesses have been reported. The product is packaged in a 48-count iced carton, item numbers 02487403 and 02487452, product lot numbers 40550707 and 40510707, with production dates of July 20th and July 21st 2009. No other food service distributors or products are involved in this recall.
All affected product within the foodservice channel has already been accounted for and destroyed.
58 cartons were shipped from two distributors to small grocery outlets in the Detroit and San Francisco areas. Consumers in California may have purchased the product from Larry’s Produce in Fairfield, La Loma Produce, Good Life Grocery and El Grande, all San Francisco, and Valley Produce & Meat Market, San Pablo. Consumers in Michigan may have purchased the product from Confers Supermarket in New Lothrop, Ryan’s Food in Detroit, Brothers Supermarket and Al Jazeera Market in Dearborn and Sahara Market in Warren.
Although it is likely that the jumbo green onions are now beyond their usable life, the recall is being issued by the company out of an abundance of caution for consumer well-being in the possible event some consumers might have purchased from the listed retail outlets and still have the product in their home refrigerators.
The company is working closely with regulatory authorities at the state and federal levels to inform consumers in Detroit and San Francisco areas who may have purchased the product.
Salmonella is an organism that can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstance, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis.

Liberté Yogourt Issues Allergy Alert on Undeclared Walnuts and Plums in Its Méditerranée Wild Blackberry Yogourt 6oz Product Distributed in the USA
Liberté Brand Products, St Hubert, Quebec, Canada announced that it is recalling all Liberté Méditerranée Wild Blackberry Yogourt 6 oz code dated Sept 02 distributed in the USA because it may contain undeclared walnuts and plums. People who have an allergy or severe sensitivity to walnuts run the risk of serious or life-threatening allergic reaction if they consume the product.
Liberté Méditerranée Wild Blackberry yogourt was distributed nationwide. The product is packaged in a 6oz retail plastic container with the expiry code date of September 02 printed on the foil lid. The UPC Bar code is 0 65684 65434 5.
Liberte initiated the recall on August 11th 2009, when the company received an email from a consumer that found that containers labelled as Liberté Wild Blackberry yogourt actually contained the Liberté Méditerranée Plum and Walnut fruit preparation. Notwithstanding our internal quality safeguards, the Plum and Walnut filling was inadvertently put into the Wild Blackberry containers for some subset of this production run. The company has notified its distributors and retailers where the product was distributed and has asked that the products be removed from the shelves.
To date no illnesses have been reported from this product.
Consumers who have Liberté Méditerranée Wild BlackBerry Yogourt 6oz should destroy the product and contact Liberté at 1-888-340-9306 x 0 (Monday-Friday 8am-5pm EST) or via email at info@liberte.ca for a full refund.
The company is working cooperatively with the U.S. Food and Drug Administration until the recall is complete. We continue to evaluate the root cause of this problem and are making corrective actions.

Kolcraft Recalls 1 Million Play Yards Due to Fall Hazard

Distributor: Kolcraft Enterprises Inc. of Chicago, Ill.
Hazard: The play yard's side rail can fail to latch properly and when a child pushes against the rail it can unlatch unexpectedly, posing a fall hazard to children.
Incidents/Injuries: The firm has received 347 reports of sides of the play yard collapsing unexpectedly, resulting in 21 injuries to young children, including bumps, scrapes, bruises and one concussion.
Description: This recall involves the Kolcraft Travelin' Tot play yards, as well as other similar play yards manufactured for Carter's, Sesame Street, Jeep, Contours, Care Bare and Eric Carle, with model numbers listed below. Some of the units have a bassinet and/or changing table. Some models have a mobile, vibrating unit or a parent organizer. CPSC Launches Resale Round-up Campaign to Keep Dangerous Recalled Products Out of Resale Stores and Off the Internet
The U.S. Consumer Product Safety Commission (CPSC) today announced its Top Ten list of recalled children's products as part of the agency's "Resale Round-up" campaign. CPSC's goal is to protect consumers by getting dangerous recalled products out of resale stores and off the Internet. Re-selling recalled products is a violation of federal law.
Before re-selling products, CPSC encourages re-sellers to check its Web site, www.cpsc.gov, to determine if the product has been recalled.
"Those who re-sell recalled children's products are not only breaking the law, they are putting children's lives at risk," said CPSC Chairman Inez Tenenbaum. "Resale stores should make safety their business and check for recalled products and hazards to children."
A CPSC study conducted in 1999 found that nearly 70 percent of resale stores sold at least one recalled or otherwise hazardous product.
CPSC is partnering with the National Association of Resale & Thrift Shops (NARTS), the Salvation Army, state agencies, and others to alert re-sellers about recalled products, product hazards including cribs that do not meet current safety standards, drawstrings in children's clothing, and the Consumer Product Safety Improvement Act (CPSIA).
"NARTS fully supports CPSC's outreach efforts to resale stores," said Adele Meyer, NARTS' Executive Director. "Our members are resale professionals committed to product safety who believe good value and selling safe products go hand in hand."
The agency's education campaign, which will include safety seminars for resale store owners and employees, is part of CPSC's commitment to protecting consumers, especially children.
CPSC's Internet surveillance team is monitoring online retailers and auction sites for sales of recalled and hazardous products. Re-sellers are reminded that they must comply with the federal law which will be enforced for in-store and online sales.
CPSC has released a new "Handbook for Resale Stores and Product Re-sellers" to help resellers comply with the law. It can be found at www.cpsc.gov/cpscpub/pubs/thrift/thrguid.pdf or can be ordered by calling CPSC's Hotline at (800) 638-2772.

If the side rails of the portable crib fold during use, an infant can become entrapped and suffocate. Three deaths have been reported.
Evenflo Happy Camper Play Yards (June 25, 1997)
The product can collapse, trapping the child in the "V" formed by the folded top rails. The rotating plastic hinges can crack or break, presenting a sharp edge or possibly allowing the child to escape. Evenflo and CPSC are aware of three deaths involving the play yards.
Baby Trend Home and Roam and Baby Express Portable Cribs and Play Yards (December 19, 1994 & February 28, 2001)
These cribs/play yards can collapse and entrap an infant. In January 2001, a 9-month-old baby in Longview, Wash., died of asphyxiation when her neck was caught in the V-shape created by the collapsed sides of her crib/play yard. There have been three other deaths and three reports of babies found not breathing (who were revived) associated with these products.
Magnetix Magnetic Building Sets (March 31, 2006 & April 19, 2007)
Tiny magnets inside the plastic building pieces and rods can fall out. Magnets found by young children can be swallowed or aspirated. If more than one magnet is swallowed, the magnets can attract to each other and cause intestinal perforation or blockage, which can be fatal. This product is unsuitable for young children. CPSC is aware of one death and at least 27 serious injuries.
Easy Bake Ovens (July 19, 2007)
Young children can insert their hands into the oven's front opening, and get their hands or fingers caught, posing entrapment and burn hazards.
Polly Pocket Dolls with Magnets (August 14, 2007 & November 21, 2006)
Small magnets inside the dolls and accessories can come loose. The magnets can be found by young children and swallowed or aspirated. If more than one magnet is swallowed, the magnets can attract to each other and cause intestinal perforation or blockage, which can be fatal. CPSC is aware of 3 serious injuries.
Simplicity Drop Side Cribs (July 2, 2009, September 17, 2008 & September 21, 2007)
The drop side can detach. When the drop side detaches, it creates space between the drop side and the crib mattress. Infants and toddlers can roll into this space and become entrapped which can lead to suffocation. CPSC is aware of 10 deaths associated with Simplicity drop side cribs.
Simplicity Bassinets (also includes bassinets with Graco or Winnie the Pooh motif) (September 11, 2008, August 27, 2008 & August 28, 2008)
The Simplicity 3-in-1 and 4-in-1 convertible bassinets contain metal bars that are covered by an adjustable fabric flap which is attached by velcro. The fabric is folded down when the bassinet is converted into a bed-side co-sleeping position. If the velcro is not properly re-secured when the flap is adjusted, an infant can slip through the opening and become entrapped between the metal bars and suffocate. CPSC is aware of at least three deaths involving Simplicity bassinets.
Hill Sportswear hooded drawstring sweatshirts (February 12, 2009)
CPSC received one report of a death involving a 3-year-old boy in Fresno, Calif. He was strangled when the drawstring on the hooded sweatshirt that he was wearing became stuck on a playground set.
Evenflo Envision High Chairs (April 2, 2009)
Recline fasteners and metal screws on both sides of the high chair can loosen and fall out, allowing the seatback to detach or recline unexpectedly. Children can fall backwards or fall out of the high chair and suffer bumps and bruises to the head, abrasions, cuts and bruises. Detached hardware also poses a choking hazard to children.

Target Recalls Circo Booster Seats Due to Fall Hazard

Hazard: The booster seat restraint buckle can open unexpectedly, allowing a child to fall from the chair and be injured.
Incidents/Injuries: Target has received eight reports of the booster seat buckles opening unexpectedly, including three reports of bruises.
Description: This recall involves Circo booster seats with manufacture date codes XJ0811, XJ0812, XJ0901, and XJ0902. The plastic booster seat is blue with green trim and has white straps. The manufacture date code is printed on the backside of the seat next to the consumer warning information. "Circo" can be found on a label located in the front of the seat.
Sold exclusively at: Target stores nationwide from December 2008 through June 2009 for about $13.

Jump 'n Jive Doorway Jumpers Recalled by Graco Due to Choking Hazard

Name of Product: Jump 'n Jive(tm) Doorway Jumpers
Units: About 2,400
Manufacturer: Graco Children's Products Inc., of Atlanta, Ga.
Hazard: The recalled doorway jumpers include detachable toys that are attached to the jumper straps with strips of hook and loop fabric. The strips of fabric are not permanently attached to the toys and can become detached during use, posing a choking hazard.
Incidents/Injuries: The company has received one report of a child removing the fabric strip and placing it in his/her mouth. No injuries have been reported.
Description: The recalled doorway jumper includes an interactive musical dance mat and two detachable toys. The model number, 1755544, appears on the carton UPC label and on the underside of the jumper tray. The doorway jumpers were sold from April 2009 through July 2009.
Sold at: Babies R Us and other children's speciality retail stores nationwide and online from Amazon.com for about $55.

Remedy: Consumers should immediately stop using the recalled product and remove and discard the detachable toys and the attaching fabric strips and call the manufacturer for a replacement set of toys. Dorel Juvenile Group Expands Recall of Safety 1st Stair Gates Due to Fall Hazard

Name of Product: Safety 1st SmartLight Stair Gates

Hazard: The hinges that hold the stair gate in place can break, posing a fall hazard to children if the gate is placed at the top of the stairs.

Description: The recall involves the Safety 1st SmartLight Stair Gate with model number 42111, which is printed on a sticker under the handle panel. The gates, which were manufactured before December 20, 2008, are white metal with a gray handle and a motion sensor nightlight that illuminates on approach. The manufacture date is located below the gate's handle as a date wheel imprinted in the plastic; an arrow indicates the month, and the year is stamped inside the circle. The manufacture date is also printed on a sticker inside the battery door.

Increasing Injuries Prompt Black & Decker to Reannounce Recall of Trimmers/Edgers Due to Laceration and Burn Hazards

Name of Product: Black & Decker GH1000 Grasshog XP String Trimmer/Edgers

Manufacturer: Black & Decker (U.S.) Inc., of Towson, Md.
Hazard: The trimmer/edger's spool, spool cap and pieces of trimmer string can come loose during use and become airborne projectiles, posing a serious laceration hazard to the user, as well as bystanders. The trimmer/edgers can also overheat, posing a burn hazard to consumers.
Incidents/Injuries: At the time of the original announcement in July 2007, Black & Decker had received more than 700 reports of incidents, including 58 reports of injuries. The firm has received over 100 additional injury reports; ten that required medical attention and minor injuries such as bruises and lacerations. There were also reports of minor property damage, including broken windows.
Description: The Black & Decker GH1000 Grasshog XP String Trimmers/Edgers are electric-powered. Trimmer/edgers with date codes 200546 through 200645 (representing manufacture dates of November 14, 2005 through November 6, 2006) are included in this recall. The date code is located on the underside of the trimmer/edger's handle. Only trimmers with black spool caps are included in the recall. Those with orange spool caps are not included in the recall.
