The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.
The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Graco Children's Products Inc., of Atlanta, Ga., is announcing the recall of about 2 million Graco strollers due to risk of entrapment and strangulation. CPSC and Graco have received four reports of infant strangulations that occurred in these strollers between 2003 and 2005. In addition, CPSC is aware of five reports of infants becoming entrapped, resulting in cuts and bruises, and one report of an infant having difficulty breathing.
Entrapment and strangulation can occur, especially to infants younger than 12 months of age, when a child is not harnessed. An infant can pass through the opening between the stroller tray and seat bottom, but his/her head and neck can become entrapped by the tray. Infants who become entrapped at the neck are at risk of strangulation.
The recall involves older versions of the Graco Quattro Tour(tm) and MetroLite(tm) strollers and travel systems manufactured prior to the existence of the January 2008 voluntary industry standard which addresses the height of the opening between the stroller's tray and the seat bottom. This voluntary standard requires larger stroller openings that prevent infant entrapment and strangulation hazards.
This recall involves Graco Quattro Tour(tm) strollers and travel systems manufactured prior to November 2006 and MetroLite(tm) strollers and travel systems manufactured prior to July 2007. The strollers and travel systems were distributed between November 2000 and December 2007. The model numbers are printed on a label at the lower portion of the rear frame, just above the rear wheels or underneath the stroller. The name "Graco" appears on a label on the stroller tray and the headrest. Models included in the recall are:

NOTE: Quattro and MetroLite strollers ending with the number 3 are NOT affected by this recall.
The strollers were sold at AAFES, Babies R Us, Burlington Coat Factory, Fred Meyer, Kmart, Meijers, Navy Exchange, Sears, Target, Walmart and other stores nationwide between November 2000 and December 2007 for between $90 and $190 for the strollers, and between $190 and $250 for travel systems.
The strollers were manufactured in China.
Consumers should immediately stop using the recalled strollers and contact Graco for a free repair kit. To order a repair kit, contact Graco toll-free at (877) 828-4046 anytime, or visit the firm's website at www.gracobaby.com. Consumers can continue use of the stroller as a "travel system." When the stroller is used with the infant car seat, the entrapment and strangulation hazards posed by the space gap are not present.

Graco(r)-Branded Drop Side Cribs Made by LaJobi Recalled Due to Entrapment and Suffocation Hazards
The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Product: Graco(r)-branded drop side cribs made by LaJobi
Units: About 217,000
Manufacturer: LaJobi Inc., of Cranbury, N.J.
Hazard: The drop side hardware can break or fail, allowing the drop side to detach from the crib. When the drop side detaches, a hazardous gap is created between the drop side and the crib mattress in which infants and toddlers can become wedged or entrapped, posing a risk of suffocation and strangulation. In addition, children can fall from the cribs when the drop side detaches or fails to lock.
Incidents/Injuries: CPSC and LaJobi have received a total of 99 reports of drop side incidents, including hardware breakage and drop side detachment. There were two incidents in which children became entrapped in the gap created by the detached drop side. Both children were freed by their caregivers. There were six reports of children falling due to drop side failure, including one report of a mild concussion.
Description: This recall involves LaJobi-manufactured Graco(r) wood cribs. The full size cribs were sold in cherry, espresso, natural and white finishes. The production date, item number, purchase order number and finish name is printed on a label affixed to the footboard or headboard. "LaJobi" and the crib model name are printed on a product sticker located on the stabilizer bar or bottom rail of the crib. Affected models are listed below.

Graco Recalls Strollers Due to Fingertip Amputation and Laceration Hazards

Hazard: The hinges on the stroller's canopy pose a fingertip amputation and laceration hazard to the child when the consumer is opening or closing the canopy.
Incidents/Injuries: Graco has received seven reports of children placing their fingers in the stroller's canopy hinge mechanism while the canopy was being opened or closed, resulting in five fingertip amputations and two fingertip lacerations.

Graco manufactured two different styles of hinge mechanisms for these stroller models. Only strollers or travel systems with a plastic, jointed hinge mechanism that has indented canopy positioning notches (see photo below) are included in this recall. The recalled strollers were manufactured between October 2004 and February 2008. The model number and manufacture date are located on the lower inside portion of the rear frame, just above the rear wheels.

Two Additional Infant Deaths Prompt Re-announcement of Simplicity "Close-Sleeper/Bedside Sleeper" Bassinets
Consumers urged to immediately stop using the 3-in-1 and 4-in-1 bassinets, including those with Graco logo and "Winnie the Pooh" motifs
The U.S. Consumer Product Safety Commission (CPSC) is once again urging all parents and caregivers to immediately stop using convertible "close-sleeper/bedside sleeper" bassinets manufactured by Simplicity Inc., of Reading, Pa. CPSC has learned of two additional infant deaths since the August 2008 safety alert and recall announcements that notified consumers of two infants who had strangled in their Simplicity bassinets. To date, CPSC is aware of four infants who have died in the close-sleeper bassinets.
CPSC learned that in September 2008, a two-month old female in Demorest, Georgia suffocated when she rolled into the adjustable fabric siding of the bassinet, pressing her face into a "pocket" which formed near the Velcro© fasteners. In another incident in January 2009, a six-month old female in Fort Worth, Texas appears to have been fatally entrapped in the bassinet's bar opening created when the Velcro© fasteners were not secured. An investigation into this infant's death is ongoing.
In addition, CPSC has received two separate reports involving a 10-week old and a 3-month old whose heads became entrapped between the lower bassinet bar and the mattress support. The infants were freed by their caregivers without injury.
The Simplicity 3-in-1 and 4-in-1 convertible bassinets, including bassinets with the Graco logo and "Winnie the Pooh" motif licensed by Disney Consumer Products, contain metal bars that are covered by an adjustable fabric flap. This fabric flap is secured by Velcro© fasteners, which are intended to be partially removed when the bassinet is converted into the bedside/close-sleeper mode. If the Velcro© fasteners are not properly re-secured when the fabric flap is adjusted, or if the consumer entirely removes the fabric flap exposing the metal bars, an infant can slip through the opening between the lower metal bar and the mattress support and become entrapped and suffocate or strangle. Consumers are urged to check their bassinets to ensure they are not using one of the recalled Simplicity bassinets.
Parents and caregivers are advised to return these bassinets to the retail store where they were purchased. All of the recalled bassinets can pose a danger to infants.
To determine whether your bassinet has been recalled, please check for the model number and the manufacture date code pictured below. Bassinets manufactured prior to 21st week of 2008, "2108" (before May 18) are recalled. The model numbers for recalled bassinets manufactured prior to "2108" are:

Jump 'n Jive Doorway Jumpers Recalled by Graco Due to Choking Hazard
The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Product: Jump 'n Jive(tm) Doorway Jumpers
Units: About 2,400
Manufacturer: Graco Children's Products Inc., of Atlanta, Ga.
Hazard: The recalled doorway jumpers include detachable toys that are attached to the jumper straps with strips of hook and loop fabric. The strips of fabric are not permanently attached to the toys and can become detached during use, posing a choking hazard.
Incidents/Injuries: The company has received one report of a child removing the fabric strip and placing it in his/her mouth. No injuries have been reported.
Description: The recalled doorway jumper includes an interactive musical dance mat and two detachable toys. The model number, 1755544, appears on the carton UPC label and on the underside of the jumper tray. The doorway jumpers were sold from April 2009 through July 2009.
Sold at: Babies R Us and other children's speciality retail stores nationwide and online from Amazon.com for about $55.
Manufactured in: China
Remedy: Consumers should immediately stop using the recalled product and remove and discard the detachable toys and the attaching fabric strips and call the manufacturer for a replacement set of toys. After the toys and fabric strips are removed, consumers can continue to use the jumper and interactive musical dance mat.

Some Recalled Simplicity Bassinets Include Graco and "Winnie the Pooh" Brand
The U.S. Consumer Product Safety Commission (CPSC) is aware that some bassinets in the Simplicity recall announced August 28, 2008 include the Graco logo and "Winnie the Pooh" motif licensed by Disney Consumer Products. Consumers are urged to check their bassinets to ensure they are not using one of the recalled Simplicity models, including those with the Graco logo or "Winnie the Pooh" motif.
The Simplicity bassinets with the Graco logo were sold between 2001 and 2004. The Simplicity bassinets with the "Winnie the Pooh" motif were sold between 2002 and 2008.

CPSC learned that on August 21, 2008, a 6 ½-month-old girl from Shawnee, Kan. was strangled to death when she became entrapped between the bassinet's metal bars. This is the second strangulation death CPSC has learned of involving the co-sleeper bassinets. On September 29, 2007, a 4-month-old girl from Noel, Mo. became entrapped in the metal bars of the bassinet and died.
The Simplicity 3-in-1 and 4-in-1 convertible bassinets contain metal bars that are covered by an adjustable fabric flap which is attached by velcro. The fabric is folded down when the bassinet is converted into a bed-side co-sleeping position. If the velcro is not properly re-secured when the flap is adjusted, an infant can slip through the opening and become entrapped in the metal bars and suffocate. This warning does not cover bassinets produced in recent months that have fabric permanently attached over the lower bar.
Due to the serious hazard these bassinets pose to babies, CPSC urges all consumers to share this safety warning with day care centers, consignment stores, family and friends to ensure that no child is placed to sleep in a Simplicity convertible bassinet covered by this warning.
