Yamaha Motor Corp. Offers Free Repair For 450, 660, and 700 Model Rhino Vehicles
Yamaha Motor Corp. Offers Free Repair For 450, 660, and 700 Model Rhino Vehicles: CPSC advises consumers not to use the off-road vehicles until repaired
The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Yamaha Motor Corp. U.S.A., of Cypress, Calif., is announcing a free repair program to address safety issues with all Rhino 450, 660, and 700 model off-highway recreational vehicles. Yamaha has also agreed to voluntarily suspend sale of these models immediately until repaired. Consumers should immediately stop using these popular recreational vehicles until the repair is installed by a dealer.
CPSC staff has investigated more than 50 incidents involving 46 driver and passenger deaths in these two Rhino models. More than two-thirds of the cases involved rollovers and many involved unbelted occupants. Of the rollover-related deaths and hundreds of reported injuries, some of which were serious, many appear to involve turns at relatively low speeds and on level terrain.
About 120,000 of the 450 and 660 model Rhinos have been distributed nationwide since Fall 2003. Some units have been equipped by Yamaha with half doors and additional passenger handholds, either before or after sale.
Yamaha's repair includes the installation of a spacer on the rear wheels as well as the removal of the rear anti-sway bar to help reduce the chance of rollover and improve vehicle handling, and continued installation of half doors and additional passenger handholds where these features have not been previously installed to help keep occupants' arms and legs inside the vehicle during a rollover and reduce injuries. Owners of the affected Rhinos should stop using them and call their dealer to schedule an appointment to have repairs made once they are available and to take advantage of a free helmet offer.
Yamaha is also voluntarily implementing the same repair program and suspension of sale for the Rhino 700 model, in order to ensure customer satisfaction. Consumers should stop riding the 700 model until it is repaired. About 25,000 Rhino 700s are part of this repair program.
Once these repairs have been made to their vehicles, Rhino users should always wear their helmet and seatbelt and follow the safety instructions and warnings in the on-product labels, owner's manuals and other safety materials. The Rhino is only recommended for operators 16 and older with a valid driver's license. All passengers must be tall enough to place both feet on the floorboard with their back against the seat back.

Caraco Pharmaceutical Laboratories, Ltd. Announces a Nationwide Voluntary Recall of All Lots of Digoxin Tablets Due to Size Variability
Caraco Pharmaceutical Laboratories, Ltd. (NYSE AMEX: CPD), a generic pharmaceutical company, announced today that all tablets of Caraco brand Digoxin, USP, 0.125 mg, and Digoxin, USP, 0.25 mg, distributed prior to March 31, 2009, which are not expired and are within the expiration date of September, 2011, are being voluntarily recalled to the consumer level. The tablets are being recalled because they may differ in size and therefore could have more or less of the active ingredient, digoxin. The recalled tablets were manufactured by Caraco Pharmaceutical Laboratories, Ltd. This recall is being conducted with the knowledge of the Food and Drug Administration.
Digoxin is a drug product used to treat heart failure and abnormal heart rhythms. It has a narrow therapeutic index and the existence of higher than labeled dose may pose a risk of digoxin toxicity in patients with renal failure. Digoxin toxicity can cause nausea, vomiting, dizziness, low blood pressure, cardiac instability, and bradycardia. Death can also result from excessive digoxin intake. A lower than labeled dose may pose a risk of lack of efficacy potentially resulting in cardiac instability. Consequently, as a precautionary measure, Caraco is recalling these tablets to the consumer level to minimize any potential risk to patients.
Consumers with the products with the following NDC codes that are within expiration should return these products to their pharmacy or place of purchase.

Hazard: The Funnel Pre-Filters can create a flash when they are removed from the fuel tank posing a fire hazard.
Incidents/Injuries: Outerwears received three reports of flashing when the funnel is removed from the fuel tank following a fueling event. No injuries have been reported.
Description: These filters are used to fuel-up off-road and racing vehicles. The recall involves Outerwears Funnel Pre-Filters, or fuel funnel pre-filters, with the model numbers 20-2465-03, 20-2466-03, 20-2467-03, and 20-2474-03. The red fabric mesh filters were available in two sizes, small and large, and in two different shapes to accommodate various sizes of fueling funnels. The name of the manufacturer, Outerwears, is the only marking on the product.

Torres Hillsdale Country Cheese LLC Announces the Recall of Asadero and Oaxaca Cheeses due to Possible Listeria Contamination
Torres Hillsdale Country Cheese of Reading, Michigan announces the recall of Asadero and Oaxaca soft Mexican-style cheeses due to potential Listeria contamination.
Consumption of food contaminated with Listeria monocytogenes can cause Listeriosis, an uncommon but potentially fatal disease. Listeriosis can cause high fever, severe headache, neck stiffness and nausea. Listeriosis can also cause miscarriages and stillbirths. The very young, the pregnant, the elderly and persons with compromised immune systems are the most susceptible to infection. People experiencing these problems should seek immediate medical attention.
A routine sample of Asadero cheese, taken by an inspector from the Michigan Department of Agriculture’s Food and Dairy Division on February 23, 2009, was subsequently tested by the Michigan Department of Agriculture Laboratory and discovered to be contaminated with Listeria monocytogenes.
Products were distributed to the following states: Illinois, Indiana, Michigan, Georgia, Tennessee, Kentucky, North Carolina, South Carolina, Ohio and Wisconsin.
The recalled Asadero and Oaxaca cheese products were distributed to retails stores and delis in 10 lb balls, 16- and 12 oz plastic packages, sold under the name "Aguas Calientes". Product was also sold in 6 lb blocks shrink-wrapped in clear plastic, under the name "El Jaliciense". Packaging may also include a label with the name "Torres Hillsdale Country Cheese LLC"
The recall encompasses all productions dates between August 1, 2008 (expiration date 10/29/2008) and February 27, 2009 (expiration date 5/10/2009).
No illnesses have been reported to date in connection with the recalled products.
Consumers and retailers are urged to return all unused recalled product to the manufacturer.

Some Chicken Bouillon Recalled Due to Foreign Inspection Not Equal to USA Standards
Georgia Agriculture Commissioner Tommy Irvin alerts consumers to the recall of some granulated chicken bouillon that does not meet U.S. standards for inspection of food products. The product, imported from an unapproved source, is being withdrawn from sale and distribution in this country.
The Kim Seng Company, Commerce City, Calif., is recalling 17,674 pounds of granulated chicken bouillon products that were ineligible for import to America. The product does not meet requirements for import into the United States.

Watson Announces a Nationwide Voluntary Recall of Propafenone HCL Tablets Due to Oversized Tablets
Watson Pharmaceuticals, Inc. (NYSE: WPI), a leading specialty pharmaceutical company, announced today that one lot of Propafenone HCL 225 mg Tablets sold in 100 count bottles in the United States is being voluntarily recalled to the consumer level as a precautionary measure. The lot is being recalled because some tablets may contain slightly higher levels of the active ingredient than specified. The recalled tablets were manufactured by Watson Laboratories, Inc. The affected lot of Propafenone HCL tablets was shipped to customers between October 15, 2008 and November 26, 2008. No other strengths or lots were affected and the Company does not anticipate any product shortages as a result of this recall. The Company has notified the U.S. Food and Drug Administration (FDA) of the recall.
Propafenone is a drug product used to treat cardiac arrhythmias (irregular heartbeats). It has a narrow therapeutic index, so some patients who are particularly sensitive to small variations in dose may experience potentially serious side effects, including arrhythmias (irregular heartbeat) or low blood pressure. Consequently, as a precautionary measure, Watson is recalling this one lot to the consumer level to minimize any potential risk to patients.
Anyone who has 225 mg Propafenone HCL tablets should check the bottle for the lot number and expiration date to see if they have tablets being recalled. Anyone with 225 mg Propafenone HCL tablets being recalled should call (888) 352-9616, Monday through Friday, 8:00 a.m.-5:00 p.m. EDT, for instructions on how to return affected product.
Patients using 225 mg Propafenone HCL tablets who have medical questions should contact their health-care provider.

Neco Foods Recalls Atlantis Brand Smoked Fish Dip Because of Possible Health Risk
Neco Foods, LLC of Lantana, Florida is recalling 231 cases of 7 ounce, 32 ounce and 5 pound packages of Atlantis Brand SMOKED FISH DIP, because it has the potential to be contaminated with Listeria monocytogenes, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, Listeria infection can cause miscarriages and stillbirths among pregnant women.
The recalled “Atlantis brand Smoked Fish Dip” was distributed in Florida in 7 ounce containers through retail stores, 32 ounce containers through club stores, and in 5 pound packages through foodservice distributors. The product is distributed in the following packages:
* 7 ounce, Atlantis brand, Smoked Fish Dip in a clear plastic container marked with Lot # 048 on bottom, with an expiration date of April 23, 2009. UPC code: 0 3469611124 5

No illnesses have been reported to date in connection with this problem.
Neco Foods has notified all Customers and confirmed that all affected product remaining in the customers inventory has been removed from store shelves and destroyed.
The potential for contamination was noted after routine testing by the company revealed the presence of Listeria monocytogenes in a returned heat abused shipment of 5 pound containers of the product.
No other products have been found to be contaminated.

Bodee LLC, Issues A Voluntary Nationwide Recall of Zencore Plus, a Product Marketed as a Dietary Supplement
Bodee LLC, 2222 Avenue of the Stars, 702E, Century City, CA 90067, announced today that it is conducting a nationwide voluntary recall of all the company's supplement product sold under the name Zencore Plus.
Bodee LLC is conducting this recall after being informed by representatives of the Food and Drug Administration (FDA) that lab analysis by FDA of Zencore Plus samples found the product contains benzamidenafil which is a newly discovered PDE5 inhibitor. Although different in structure, benzamidenafil is likely to have the same pharmacological properties as the other three PDE5 inhibitors, sildenafil, tadalafil and vardenafil, which are the active ingredients of three FDA approved drugs for Erectile Dysfunction (ED). Zencore Plus does not contain any of these three active ingredients. The use of Zencore Plus by an unsuspecting user of organic nitrates may pose a life-threatening risk of sudden and profound drop of blood pressure due to potential interaction between benzamidenafil and organic nitrates. The probability of this occurrence is unknown; therefore, it may cause a public health risk when used as a dietary supplement.
Zencore Plus is sold in health food stores and by mail order on internet nationwide. The Zencore Plus product is sold as a 2-capsule blister pack packaged in a retail booklet with five booklets in a box or as a 10-capsule blister pack in a retail box. The capsules were supplied by Hi-Tech Pharmaceuticals, Inc., Norcross, GA.
Customers who have this product in their possession should stop using it immediately. Peregrina Cheese Corporation Recalls Queso Fresco Because of Possible Health Risk
Peregrina Cheese Corporation of Brooklyn, New York, is recalling one code of its 14 ounce packages of Queso Fresco Fresh Cheese Mexican style soft cheese because they have the potential to be contaminated with Listeria monocytogenes, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, Listeria infection can cause miscarriages and stillbirths among pregnant women.
The recall cheese was distributed to retail stores in New York City (Brooklyn, Queens, Bronx and Manhattan) and Pennsylvania (Scranton and Hazelton).
The Queso Fresco Fresh Cheese comes in a 14 ounce foil wrapped package marked with code 4483, UPC 8 17424 00024 6, and bearing Plant # 36-8431. The cheese was produced on 2/18/09.
No illnesses have been reported to date.
This recall is the result of sampling and analysis by the U.S. Food and Drug Administration (FDA) which revealed that finished product contained the bacteria.
Peregrina Cheese is contacting the customers today and instructing them to remove the product from sale and segregate it. The firm’s driver will visit the retail stores and pick up the recalled product as soon as possible.
This recall action is being conducted with the knowledge of the FDA.

Wall-Mounted Radiators Recalled by Rettig Belgium Due to Crush Hazard; Child's Death Reported
The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of product: Radson Wall-Mounted Radiators
Units: About 36 in the United States, 113,000 worldwide
Manufacturer: Rettig Belgium NV, of Belgium
Hazard: The radiator can come loose from the wall, and fall on people near it.
Incidents/Injuries: None reported in the U.S. There have been 52 incidents reported outside the U.S., including one that involved the death of a child in Belgium.
Description: The recalled radiators are used to heat a home and are mounted vertically to the wall. The recall involves the Radson Vertical 20 and 21, Radson Kos V21, and Radson Faro V21.
Sold by: Plumbing and heating supply companies from October 2007 through January 2008 for between $300 and $700.
Manufactured in: Belgium
Remedy: Consumers should avoid standing or sitting near the recalled radiators and contact their plumbing or heating supplier or Rettig for a free replacement suspension system.

Importer: Wiseco Performance Products, of Mentor, Ohio
Hazard: Engine stress could cause the connecting rods to crack and the engine to lock up, posing a crash hazard.
Incidents/Injuries: None reported.
Description: This recall involves the following connecting rods and crankshaft assemblies used in off-road dirt bike engines. Wiseco's identification number can be found on the neck of the connecting rods between the two circular ends. The part number and date code can be found on the product's packaging.

Nautilus Recalls Bowflex(r) Home Gyms Due to Risk of Injury
The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of product: Bowflex(r) Ultimate 2 Home Gyms
Units: About 78,000 (about 68,000 home gyms were recalled on December 6, 2007)
Importer: Nautilus Inc., of Vancouver, Wash.
Hazard: The home gym's horizontal seat rail is designed to be latched in a vertical position for storage. If the seat rail is not manually latched, it can fall unexpectedly on the user or a bystander, posing a risk of serious injury.
Incidents/Injuries: Nautilus has received 18 reports of injuries caused by unlatched seat rails that have fallen, resulting in head and shoulder lacerations, some requiring stitches or staples.
Description: This recall involves the Bowflex(r) Ultimate 2 Home Gyms. The home gyms are equipped with pulleys, resistance rods, and other equipment, which allow the user to perform a variety of exercises. "Bowflex Ultimate 2" is written on the vertical part of the frame.
Sold at: Specialty fitness retailers nationwide and through direct Nautilus sales from June 2005 through January 2009 for about $2,300.

Remedy: Consumers may continue to use their home gyms for exercising, but should not place the seat rail in the vertical (storage) position until the repair is performed. Registered owners are being sent a free repair kit. Owners who have not received a repair kit by March 10, 2009 should contact the firm.

Hazard: Decorative paint on the sole of the flip flops can contain levels of lead in excess of the federal standard.

Description: Flip flops of the Havaianas brand containing decorative paint were sold under the following model names: Baby Estampas, Baby Pets, Kids Apple, Kids Fairy, Kids Flores, Kids Lighthouse, Kids Monsters, Kids Surf, Baby Letrinhas, Kids Sports, Kids Candies, Kids Fun, Kids Love, Kids Sereias, Kids Speed, Kids Lucky Bug, Kids Pets, Kids Rock, Kids Slim, Kids Wonder Woman, Kids Small Flowers and Kids Tropical w/Kit. A photo of a representative flip flop is provided below. Havaianas flip flops without decorative paint are not being recalled.
Sold by: Department and specialty stores nationwide from November 2006 through February 2009 for about $15 to $24 a pair.
Manufactured in: Brazil
Remedy: Consumers should immediately take the recalled flip flops away from children and return them to Alpargatas USA, Inc. to receive a replacement.

Goya Foods, Inc. Announces Voluntary Recall of Dried Yellow Potato (Papa Seca Amarilla)
Goya Foods, Inc. of Secaucus, NJ, is recalling its 12 ounce packages of Dried Yellow Potato (Papa Seca Amarilla), UPC 041331050562, because it contains undeclared sulfites. Consumers who have sensitivity to sulfites run the risk of serious or life-threatening allergic reactions if they consume this product.
The product is packaged in 12 ounce plastic bags, all production lot codes are affected and the product has been distributed nationwide. It is a product of Peru.
The recall was initiated after routine sampling by New York State Department of Agriculture and Markets Food Inspectors and subsequent analysis of the product by Food Laboratory personnel revealed the presence of sulfites in Goya brand Dried Yellow Potato (Papa Seca Amarilla) which were not declared on the label. The consumption of 10 milligrams of sulfites per serving has been reported to elicit severe reactions in some asthmatics. Anaphylactic shock could occur in certain sulfite sensitive individuals upon ingesting 10 milligrams or more of sulfites.
No illnesses have been reported to date in connection with this problem.
Consumers who have purchased Goya brand Dried Yellow Potato (Papa Seca Amarilla) are urged to contact Goya Foods, Inc. for a full refund. Consumers with questions may contact the company Monday through Friday between the hours of 8AM to 5PM, Eastern Standard Time at 1-800-275-4692.

Marron Foods Recalls Instant Non Fat Dry Milk Boxes That May Contain Foreign Material
Marron Foods, a Durand, WI. company is voluntarily recalling approximately 140 cases of Non Fat Dry Milk that may contain a piece of metal that came off the packaging equipment which was discovered as part of regular maintenance.
Swallowing fragments of metal may constitute a choking hazard.
Product was distributed in Safeway Food stores nationally.
