Hazard: The flame guards on the recalled replacement torch fuel containers can malfunction and cause the containers to melt while the torch is in use, posing a fire hazard to consumers.
Incidents/Injuries: Lamplight has received 35 reports of the fuel containers melting while the torch was lit including one report of a minor burn injury.
Description: The recall involves Clear-Vu(r) Torch Fuel Containers sold as replacements. "RC" is stamped on the underside of the flame guard. Clear-Vu(r) torch fuel containers that came with torches are not included in this recall.
Sold at: Discount, hardware, and home improvement stores nationwide from January 2008 through August 2009 for between $1 and $3.

Consumer Alert - Undeclared Sulfites in "Ziolopex Bakaliada Morele Suszone (Dried Apricots)"
New York State Agriculture Commissioner Patrick Hooker today (September 24) alerted consumers that Adamba Imports International, Inc., located at 585 Meserole Street, Brooklyn, NY is recalling "Ziolopex Bakaliada Morele Suszone (Dried Apricots)" due to the presence of undeclared sulfites. People who have severe sensitivity to sulfites may run the risk of serious or life-threatening reactions if they consume this product.
The recalled "Ziolopex Bakaliada Morele Suszone (Dried Apricots)" are packaged in a 4.64 ounce (130 gram), plastic bag and coded 06 2010, 081215V1C in black ink on the back of the package. The product was distributed nationwide and is a product of Poland.
Routine sampling by New York State Department of Agriculture and Markets Food Inspectors and subsequent analysis of the product by Food Laboratory personnel revealed the product contained high levels of sulfites which were not declared on the label. The consumption of 10 milligrams of sulfites per serving has been reported to elicit severe reactions in some asthmatics. Anaphylactic shock could occur in certain sulfite sensitive individuals upon ingesting 10 milligrams or more of sulfites.
No illnesses have been reported to date to this Department in connection with this product. Consumers who have purchased "Ziolopex Bakaliada Morele Suszone (Dried Apricots)" should contact Adamba Imports at 718-628-9700.

Hazard: The hammock's supporting hooks can fail, causing occupants to fall and suffer injuries.
Incidents/Injuries: The firm has received nine incidents of hooks failing, resulting in bumps, bruises and/or scrapes.
Description: The recalled hammocks were sold in single and double versions and packaged in a nylon sack. The Grand Trunk logo is printed on the sack. The following models and colors are included in the recall: SH-01 (Single Olive/Khaki), SH-02 (Single Blue/Navy Blue), SH-06 (Single Royal/Khaki), DH-01 (Double Olive/Khaki), DH-02 (Double Lt. Blue/Yellow), DH-03(Double Navy/Silver), DH-04 (Double Navy/Red). The model number can be located on the product's hang tag.

Incidents/Injuries: Big Lots has received 14 reports of the recalled bunk beds' support slats cracking or breaking, including four reports of minor injuries.
Description: This recall includes wooden bunk beds with a three step ladder. Model numbers WP-9108-1 and WP-9108-2 are included in this recall. The model number is located on a sticker on the bunk bed's interior panel of the headboard or the footboard.
Sold exclusively at: Big Lots stores nationwide from May 2008 through February 2009 for about $300.
Manufactured in: Vietnam

Garden-Fresh Foods Issues Allergy Alert On Undeclared Milk in 16-Ounce Archer Farms Potato Salad
Garden-Fresh Foods, Inc. of 726 South 12th Street, Milwaukee, Wisconsin is recalling 16-ounce containers of Archer Farms Potato Salad with an expiration date of 10/04/2009 because they may contain undeclared milk allergens. People who have allergies to milk run the risk of serious or life-threatening allergic reaction if they consume this undeclared product.
The recalled 16-ounce Archer Farms Potato Salad were distributed nationwide at SuperTarget retail stores.
The product is sold in a 16-ounce square plastic container. The product is marked with UPC number 0 85239 40228 3 and an expiration of 10/04/2009. Only this product marked with this code date is being recalled. No illness has been reported to date in connection with this problem.
The recall was initiated after it was discovered that the product was distributed in packaging that did not reveal the presence of milk. Subsequent investigation indicates the problem was caused by a breakdown in Garden-Fresh Food’s production and packaging process.

Ippolito International, LP Voluntarily Recalls Bunch Spinach Because ofPossible Health Risk Nationwide and Canada
Ippolito International, LP of Salinas, California is voluntarily recalling 1,715 cartons of bunched spinach, because it has the potential to be contaminated with Salmonella, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis.
The voluntary recall comes after routine random testing of bunched spinach in Michigan by the USDA Microbial Data Program, collected and analyzed by the Michigan Department of Agriculture, tested positive.
No illnesses have been reported to date.
The voluntary recall is limited to 1,715 cartons. One thousand, five hundred fifteen of the cartons were packed in the "Queen Victoria" brand. This product was distributed to Alabama, Arizona, California, Florida, Georgia, Illinois, Indiana, Kentucky, Michigan, Minnesota, New Jersey, New York and British Columbia, Ontario and Manitoba, Canada. It was distributed to retail, foodservice and wholesale buyers.
Two hundred of the cartons were packed in the "Tubby" label and were distributed in California and New York to retail and wholesale customers.
The "Queen Victoria" product was packed in cardboard cartons with "Spinach" printed on the side panel, in wire bound crates or Reusable Plastic Containers. The product was harvested September 1 through September 3.
The 12 and 24 count spinach bunches were bound with a twist tie which says "Queen Victoria Spinach Produce of USA PLU 4090 UPC 33383 65200."
The cartons were labeled with the following Code Date Stickers which can be located on the outside edge panel of the carton. The stickers read as follows:

The 24 count spinach bunches packed in the "Tubby" label were distributed in California and New York. They were bound with a generic bunched spinach twist tie which says "Spinach Produce of USA PLU 4090 UPC 33383 65200."
Consumers should understand the numbers provided: PLU 4060 and UPC 33383 65200
are located on the majority of bunched spinach twist ties packed by the majority of spinach shippers in the United States. Those numbers do not necessarily reference the single lot to which the recall is limited. The only numbers that reference the recalled lot would be found on the carton.
Consumers should check with where they purchased the product to see if it was indeed from a carton with the specific Code Date Sticker referenced above.
Ippolito International immediately identified the specific harvest lot and all buyers who received the product. The buyers were contacted and instructed to destroy any product remaining in inventory. No other Ippolito International products or packs are involved in this recall.
"Our primary concern and commitment is for the well-being of the consumer. This voluntary recall is precautionary to reduce risk to the lowest level," said Butch Corda, Ippolito International General Manager.

People who have an allergy or severe sensitivity to almonds or tree nuts may run the risk of a serious allergic reaction if they consume these products.

LIFEPAK CR Plus Automated External Defibrillators (Physio-Control, Inc)
FDA notified healthcare professionals of a Class I recall of certain LIFEPAK CR Plus Automated External Defibrillators (AED) manufactured and distributed from July 9, 2008 through August 19, 2008. An extremely humid environment may cause the affected devices to improperly analyze the heart rhythm and may cause the device to delay or fail to deliver therapy.

Arbonne International, LLC Announces Voluntary Recall of One Lot of Seasource Detox Spa® Foaming Sea Salt Scrub
Arbonne International, LLC ("Arbonne") has voluntarily recalled one lot of its Seasource Detox Spa® Foaming Sea Salt Scrub. The recalled products were manufactured by a third party and distributed nationwide through Arbonne Independent Consultants. The product comes in a 6.8 fluid oz white tube and is identified with a lot number on the crimping at the top of the tube. The Seasource Detox Spa Foaming Sea Salt Scrub products affected by this voluntary recall are ONLY from the following lot number (with shipping dates ranging from March 28, 2008 to July 15, 2009):

This voluntary recall was initiated by Arbonne as a result of discovering the presence of Pseudomonas aeruginosa bacteria in the recalled lot. No other lots are affected. The organism Pseudomonas aeruginosa may cause dermatitis, soft tissue infections, bacteremia, and a variety of system infections, particularly with users who are immunosuppressed. Because the Foaming Sea Salt Scrub is used to exfoliate the skin’s surface, there is a possibility that inadvertent introduction of the tainted product directly into any skin abrasions could result in infection.
No illnesses have been reported in connection with this product.
Purchasers of the recalled product lot are advised not to use it. Arbonne will, at the purchaser's choice, replace or exchange the product or refund money paid for the recalled product. In order to replace or exchange product or receive a refund, Arbonne is requesting purchasers of the recalled product lot to contact Arbonne at 1-800-ARBONNE between the hours of 6:00 a.m. and 6:00 p.m. Pacific Time weekdays and between the hours of 6:00 a.m. and 3:00 p.m. Pacific Time on Saturdays, and have the product with the recalled lot number available to return to Arbonne in order to complete the transaction. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. The recalled chocolate-covered peanuts were sold in Kilwin's retail stores located in the following states: Michigan, Florida, Illinois, Wisconsin, Colorado, Rhode Island, Georgia, Maryland, Pennsylvania, Tennessee, North Carolina, and South Carolina.
The product comes in a 7 ounce, gold foiled plastic package with a clear center section, and is labeled "Milk Chocolate Peanuts" and "Fresh dry roasted peanuts covered in creamy milk chocolate", Kilwin's Quality Confections, Petosky, MI 49770. The barcode on the package is 001615. Additionally, the product was sold in bulk bins, by weight, at Kilwin's retail stores.
No illnesses have been reported to us in connection with this problem.
The potential for contamination was noted after our peanuts supplier voluntarily recalled their product because these peanuts were subject to the PCA Texas facility recall.
Any of these products purchased at our stores on or after April 1, 2009 are not subject to this recall.

PREMIER ORGANICS Issues Nation Wide Recall For ARTISANA RAW TAHINI Because of Possible Health Risk
Premier Organics of Berkeley, CA is recalling 840 Cases of Artisana Raw Tahini of Lot 90811, in 16oz glass jars because it has the potential to be contaminated with Salmonella, an organism which can cause serious infections. Healthy persons infected with Salmonella often experience fever, diarrhea, nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections, endocarditis and arthritis.
The affected lot (lot number 90811) has been distributed nationwide through retail locations and online stores. No other lots have been affected.
It is important to note that there have been NO REPORTED ILLNESSES to date.
Of the numerous samples taken by both Premier Organics and the FDA of lot 90811, only one sample came back positive for salmonella. Premier Organics has ceased distribution of Artisana Tahini lot 90811 as they continue their investigation with the FDA into what caused the problem.

Bicycle Chains and Chain Connector Links Recalled by SRAM Due to Fall Hazard
The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Product: 10 Speed SRAM Bicycle Chains with PowerLock connector links
Units: About 24,000
Manufacturer: SRAM LLC, of Chicago, Ill.
Hazard: The recalled PowerLock connector links, used on bicycle chains, are brittle and can crack, allowing the chain to separate from the bicycle and posing a fall hazard to the rider.
Incidents/Injuries: One incident has been reported in the United States and three outside. No injuries have been reported.
Description: The recall involves SRAM PowerLock connector links, which are sold individually, on 10-speed bicycle chains and as original equipment on some bicycles. The recalled PowerLock connector links are identified by a date code of M or N, which is located on the side of the PowerLock connector link. 10-speed SRAM chains that were equipped with recalled PowerLock connector links include the following chain model numbers: PC-1030, PC-1050, PC-1070, PC-1090 and PC-1090R. The recalled chains were also installed on some Guru, Surly, Salsa, BMC, Serotta, Seven, and Ridley brand complete bicycles. The individual PowerLock connectors and bike chains with these connectors were sold from January 2009 through August 2009; bicycles with affected chains were sold from April 2009 through August 2009.
Sold at: Specialty bicycle retailer stores nationwide from January 2009 through August 2009. PowerLock connector links were sold separately for about $5 or as part of 10-speed chains for between $35 and $85.
Manufactured in: Portugal
Remedy: Consumers should immediately stop using the recalled PowerLock connectors, chains and bicycles with these connectors and contact their SRAM retailer for a free replacement PowerLock connector link.

Children's Animal Masks and Pendants Recalled by Team Work Trading Due to Risk of Lead Exposure
The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Product: Children's Animal Masks and Pendants
Units: About 1,400 pieces
Manufacturer: Team Work Trading, of Los Angeles, Calif.
Hazard: The recalled children's animal masks and pendants contain high levels of lead. Lead is toxic if ingested by young children and can cause adverse health effects.
Incidents/Injuries: None reported.
Description: This recall involves a children's animal mask and four styles of metal pendants. The mask resembles the face of a cat. The pendants have various animated symbols including a dog tag with fire symbol (Bleach), knife and lion symbol (Final Fantasy), Mickey Mouse symbol (Kingdom of Hearts), and a burning sun symbol (Naruto). The pendants were sold with silver-colored chains.
Sold at: Gift shop and modeling stores nationwide, and the Team Work Trading store from November 2008 through March 2009 for between $4 and $8.
Manufactured in: China
Remedy: Consumer should immediately stop using the recalled items and contact Team Work Trading for a refund or exchange.

Georgia Agriculture Department Finds Listeria in Sample of Eggo Buttermilk Waffles at Atlanta Plant
A laboratory test by the Georgia Department of Agriculture (GDA) found Listeria monocytogenes in a sample of Kellogg's Buttermilk Eggo Waffles manufactured at an Atlanta plant.
The sample was taken as part of a routine inspection by GDA of the Kellogg's plant at 5601 Bucknell Dr. SW, Atlanta. This product was not shipped into the marketplace.
While no illnesses have been reported, out of an abundance of caution, Kellogg is voluntarily recalling a limited number of the following products produced in the same facility:
Kellogg's® Eggo® Cinnamon Toast waffles, 10-count package, UPC code 3800040440 with Best If Used Before" dates beginning with:NOV22 10 EA, NOV23 10 EA and NOV24 10 EA.
Kellogg's® Eggo® Toaster Swirlz™ Cinnamon Roll Minis eight-count package, UPC code 3800023370 with a "Best If Used Before" date beginning with NOV15 10 EA.
Kellogg voluntarily ceased production at the plant, began an investigation to determine a possible cause of contamination and began a regimen of cleaning and sanitizing. Kellogg will execute its hygienic restoration plan under GDA before it resumes production at the plant.
Consumption of food contaminated with Listeria monocytogenes can cause listeriosis, an uncommon but potentially serious disease. The most common manifestation of listeriosis is meningitis, which has symptoms of high fever, severe headache, neck stiffness and nausea. Listeriosis can also cause miscarriages and stillbirths, as well as serious and sometimes fatal infections to infants, the elderly and those with weakened immune systems such as persons with chronic diseases or taking chemotherapy for cancer.

OfficeMax Recalls Office Chairs Due to Fall Hazard
The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Product: Office Max Task Chairs
Units: About 216,000
Importer: OfficeMax Inc., of Naperville, Ill.
Hazard: The back and the base post of the chair can break while in use, posing a fall hazard to consumers.
Incidents/Injuries: OfficeMax has received about 35 reports of the chair backs or posts breaking, including 15 reports of injuries involving lacerations, muscle strains, contusions and concussions.
Description: The recall involves OfficeMax Task Chairs with model numbers OM182 and OM96614. The model number is located under the chair's seat on a white UPC label. The chairs are charcoal or dark charcoal in color and have black plastic arms and a rolling plastic and metal base.
Sold at: OfficeMax stores nationwide, on the firm's Web site www.officemax.com, in OfficeMax catalogs, and through direct commercial sales to businesses from September 2003 through July 2008 for between $40 and $65.
Manufactured in: China
Remedy: Consumers should immediately stop using the chairs and return them to any OfficeMax store for a full refund or a $55 gift card if the consumer does not have a receipt.

Wal-Mart Announces Recall Expansion of Durabrand DVD Players Due to Fire Hazard
/PRNewswire/ -- The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of product: Durabrand DVD Players. (The August 20 recall of the silver Durabrand DVD player has been expanded to include the pink- and purple-colored versions of the same DVD player.)
Units: About 4.2 million (1.5 million were previously announced #09-316 link)
Importer: Wal-Mart Stores Inc., of Bentonville, Ark.
Hazard: The DVD player's circuit board can overheat, posing a fire and burn hazard to consumers.
Incidents/Injuries: Wal-Mart has received 14 reports of DVD players overheating, seven of which have resulted in fires that caused property damage. No injuries have been reported.
Description: This recall involves a single DVD player with a remote control. The device was sold in three colors -- silver, pink and purple and has a U-shaped opening at the top to insert the DVD. The DVD players were sold under the following UPC codes and model numbers:
Silver: UPC 1799901002, Model No. 1002
Pink: UPC 1799934100, Model No. 1002 PINK
Purple: UPC 1799932100, Model No. 1002 PUR
Sold exclusively at: Wal-Mart stores nationwide from January 2006 through July 2009 for about $29.

Remedy: Consumers should immediately stop using the product and return it to the nearest Wal-Mart for a full refund.
