Wooden Infant Toys Recalled by Habermaass Corp. Due to Choking Hazard

Name of Product: Wooden Puzzles, Infant Rattles, Pacifier Holders, and Stroller Toys

Manufacturer: Habermaass Corp. Inc., of Skaneateles, N.Y.

Hazard: The wooden rattles, pacifier holders, and stroller toys contain small pieces including glued on mirrors and/or prisms that can detach, posing a choking hazard to infants. The head of the ladybug puzzle pieces also poses a choking hazard.

Incidents/Injuries: Habermaass Corp. has received 15 reports of incidents involving the recalled puzzles, rattles, and pacifier holders including pieces of the rattles detaching and being mouthed by young children.

Description: The puzzles, rattle toys, pacifier holders, and stroller toys are wooden and are described in the chart below. “HABA” is printed on them.

Sold at: Specialty toy stores nationwide and at specialty online retailers from January 2002 through August 2008 for between $10 and $35.

Description: The recalled garments were sold in children’s sizes small, medium and large in the following colors: white, ivory, red, black, navy, brown, green, charcoal, blue, and pink. “BAC” is printed on the sewn-in tag on some of the garments. “Breckenridge”, “Keystone”, “Jackson Hole”, “Vail”, “Steamboat”, and “Mackinac Island” is printed on the front of the garments.

Hazard: The stroller’s hinge locking mechanism poses a laceration hazard to the user folding and unfolding the strollers.

Incidents/Injuries: Regal Lager has received nine reports from consumers who injured their fingers in the stroller’s hinge locking mechanism.

Description: The recalled single strollers all have metal frames with three wheels, a cloth seat and canopy. The twin strollers have a metal frame with four wheels, side-by-side cloth seats and double sun canopies. The strollers were sold in a variety of colors including red, orange, green, black, charcoal and navy. The Phil & Teds logo is located on the crotch piece of the harness.

Hazard: The internal heating element could overheat and drop molten metal through ventilation openings in the water dispenser’s base onto the countertop, posing a fire hazard. The internal heating element also poses a shock hazard to consumers who touch it through the vents.
Incidents/Injuries: Greenway has received one report of a fire involving the recalled countertop water dispenser outside the United States that resulted in property damage. No injuries have been reported.

Six Retailers Agree To Stop Sale and Recall Simplicity Bassinets Due To Strangulation Hazard

The U.S. Consumer Product Safety Commission (CPSC) is urging retailers across the country to protect the health and safety of babies by stopping sale and recalling nearly 900,000 dangerous Simplicity bassinets.

Yesterday, CPSC warned parents, caregivers and consumers to immediately stop using convertible "close-sleeper/bedside sleeper" bassinets manufactured by Simplicity Inc., of Reading, Pa., due to a serious safety risk. The fabric is folded down when the bassinet is converted into a bed-side co-sleeping position. If the velcro is not properly re-secured when the flap is adjusted, an infant can slip through the opening and become entrapped in the metal bars and suffocate. This warning does not cover bassinets produced in recent months that have fabric permanently attached over the lower bar.

Infants Strangled to Death in Simplicity Bassinets: CPSC Urges Consumers To Stop Using Simplicity 3-in-1 and 4-in-1 Convertible “Close-Sleeper” Models

The U.S. Consumer Product Safety Commission (CPSC) is urging parents and caregivers to stop using convertible “close-sleeper/bedside sleeper” bassinets manufactured by Simplicity Inc., of Reading, Pa.

CPSC has learned that on August 21, 2008, a 5-month-old girl from Shawnee, Kan. was strangled to death when she became entrapped between the bassinet’s metal bars. This is the second strangulation death CPSC has learned of in the co-sleeper bassinets.

On September 29, 2007, a 4-month-old girl from Noel, Mo. became entrapped in the metal bars of the bassinet and died.

CPSC is issuing this safety alert because SFCA Inc., the company which purchased all of Simplicity Inc.’s assets at public auction in April 2008, has refused to cooperate with the government and recall the products. SFCA maintains that it is not responsible for products previously manufactured by Simplicity Inc.

The Simplicity 3-in-1 and 4-in-1 convertible bassinets contain metal bars spaced farther apart than 2 3/8 inches, which is the maximum distance allowed under the federal crib safety standard. The metal bars are covered by an adjustable fabric flap which is attached by velcro. The fabric is folded down when the bassinet is converted into a bed-side co-sleeping position. If the velcro is not properly re-secured when the flap is adjusted, an infant can slip through the opening and become entrapped in the metal bars and suffocate.

This warning does not cover bassinets produced in recent months that have fabric permanently attached over the lower bar.

Due to the serious hazard these bassinets pose to babies, CPSC urges all consumers to share this safety warning with day care centers, consignment stores, family and friends to ensure that no child is placed to sleep in a Simplicity convertible bassinet covered by this warning.

The Commission voted, using its new authorities in the Consumer Product Safety Improvement Act, to release this warning upon making a finding that the health and safety of the public require immediate notice.

"Fresh from the Deli" American Sub Sandwich Recalled by Landshire, Inc. Because of Possible Health Risk

Landshire, Inc. is recalling its American Sub sandwich because it has the potential to be contaminated with Listeria Monocytogenes, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, Listeria infection can cause miscarriages and stillbirths among pregnant women.

The American Sub sandwiches were distributed in the Midwestern States of: Indiana, Kentucky, Ohio and Pennsylvania. The product was sold to Circle K Convenience Stores exclusively. There were 812 sandwiches produced on lot number 21882.

The sandwich label identifies it as: "Fresh from the Deli" American Sub. This is a pre-packaged, individually wrapped (clear plastic) with a black tray inside of package. Sandwich weight is 9.75ounces (276grams). UPC code is 9748800540. The sandwiches involved have a lot number of 21882. The lot number is printed in black ink on the side of the package.

No illnesses have been reported to date.

The recall was the result of a routine sampling taken by the Ohio Department of Agriculture for retail food establishments. Landshire has ceased the production and distribution of the product as the Company continues its investigation as to the source of the problem.

Trans Ocean Products Recalls "Cracked Pepper Style Smoked Salmon" LOT No 54933-2 Because of Possible Health Risks

Trans-Ocean Products, Inc. of Bellingham, Washington is recalling its 4 ounce Cracked Pepper Style Smoked Salmon Lot No 54933-2 because it has the potential to be contaminated with Listeria monocytogenes, an organism, which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, Listeria infection can cause miscarriages and stillbirths among pregnant women.

Cracked Pepper Style Smoked Salmon, 4 ounce packages were distributed to three supermarket chains in seven states: Brookshire Brothers in Texas and Louisiana; Price Choppers in New York, Pennsylvania, Massachusetts and Vermont; and Giant Eagle in Ohio and Pennsylvania.

The product is sliced cold-smoked salmon with black pepper. It is vacuum packaged in a resealable purple plastic pouch and bears the brand name Trans Ocean Products.The lot number is ink jetted on the back panel of the package, just right of center.

No illnesses have been reported to date in connection with this problem.

The potential for contamination was noted after routine testing by Trans-Ocean Products revealed the presence of Listeria monocytogenes in one 4 ounce package of Cracked Pepper Style Smoked Salmon.

Distribution of the product has been suspended while the FDA and Trans-Ocean Products investigate the source of the problem.

Consumers that have purchased Trans Ocean ProductsCracked Pepper Style Smoked Salmon are urged to return them to the place of purchase for a full refund.

Name of Product: Indoor Lighting Fixtures

Manufacturer/Distributor: Lithonia Lighting, of Conyers, Ga.

Hazard: The glass reflector can separate from the fixture and fall, posing a risk of injury to consumers below the fixture.

Incidents/Injuries: There has been one report of a glass reflector falling. No injuries have been reported.

Description: The recall involves the Hi-Bay Indoor High-Intensity Discharge (HID) Fixtures, model numbers PG16A and PG21A. The fixture is intended for high-mounting heights (at least 20 feet above floor level) and is used in retail, warehouse and heavy manufacturing locations. The fixture's housing is made of die-cast aluminum.

Sold by: Electrical distributors and sales representatives nationwide from January 2007 through May 2008 for between $280 and $350.

Name of Product: Car Chargers Used with Power System Plus 3 Million Candlepower Spotlights

Hazard: The car charger is incompatible with the spotlight's battery, which can cause it to overcharge inside of a vehicle and pose a fire or burn hazard to consumers.

Incidents/Injuries: IMS has received two reports of incidents of spotlights overheating while being charged with the car charger. No injuries have been reported.

Description: The recalled 12V DC car charger was sold with the Power Systems Plus/UST 3 Million Candlepower Spotlight with model number HSLR30S. The spotlight is black and blue and has a sticker on each side that reads "3 Million Candle Power." "HSLR30S" is printed on the instruction manual and on the packaging.

Interior Alaska Fish Processors, Inc. Recalls Santa's Smokehouse Brand Smoked Keta Salmon

Interior Alaska Fish Processors, Inc. of Fairbanks, AK is recalling some of its Santa's Smokehouse brand smoked keta salmon because it has the potential to be contaminated with Listeria monocytogenes, an organism which can cause serious infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as fever, headache, stiffness, nausea, abdominal, or diarrhea, listeria infection can cause miscarriages and stillbirths among pregnant women.

The recalled product was sold in random weight portions and packaged in clear plastic vacuum sealed bags with the Santa's Smokehouse "teriyaki style smoked keta salmon" label on it. The only batch subject to the recall will be labeled with the batch number '637', found on the Santa's Smokehouse label. Approximately 150 pounds of this product was distributed through the company's retail store in Fairbanks, Alaska and through gift packs sold on the internet between mid December 2007 and July, 2008. The recall is the result of survey sampling by the Alaska Department of Environmental Conservation which revealed that the sampled product in question contained the bacteria Listeria monocytogenes. No complaints or instances of illness have been reported from consumption of this product.

The splices are yellow insulated vinyl and measure about one inch long and ¼ inch wide. They were intended for use with 12-10 AWG wire. 12-10 is stamped on the side of the splices. Model numbers 10-126, or 21-126, and Gardner Bender are printed on the product's packaging. They were sold in packages of 8 or 50.

Electra Bicycle Company Expands Recall of Amsterdam Bicycles; Chain Can Derail and Pose Injury Risk to Riders

Units: About 1,500 (about 9,500 bicycles were recalled on July 22, 2008)

Hazard: The interior alignment tabs of the bicycle's chainguard can be pushed against the chain causing it to derail, which poses a risk of injury to riders.

Incidents/Injuries: Electra has received four reports of the chainguard derailing the chain, including one report of minor cuts and abrasion from a fall.

Description: This recall involves some Amsterdam model bicycles in the Classic 3, Original 3, Royal 8, and Sport 3 styles. The bicycles have an enclosed chainguard. "Electra" and "Amsterdam" are printed on the bicycle's frame and/or chainguard. Bicycles with frame numbers beginning with "EL" or bicycles with chainguards marked "06/08" are not included in this recall. The frame number is located on the underside of the bottom bracket.

Manufacturer for Publix Brand Vanilla Wafers Issues a Recall for Undeclared Soy

The manufacturer of Publix Vanilla Wafers is recalling a limited number of 12oz boxes of Vanilla Wafers with a code date of 030409A as they may contain undeclared soy. Persons who have an allergy to soy run the risk of a serious or possible life-threatening allergic reaction if they consume the affected product. The UPC number may be found on the side label below the Nutrition Facts panel:

The product was distributed throughout the chain to include stores in Florida, Georgia, South Carolina, Alabama and Tennessee.

The vanilla wafers are packaged in 12 oz. paperboard boxes.

“The recall was initiated after the discovery of gingersnap cookies co-packaged with vanilla wafers. This resulted in the undeclared soy ingredient.” said Roy Peterson, Toufayan Bakery general manager. "As part of Publix commitment to food safety, they are asking customers to return the product for a full refund or replacement. There have been no reported cases of illness. FDA Warns Consumers Not to Eat Certain Mussel Products from Bantry Bay Seafood

The U.S. Food and Drug Administration (FDA) is warning consumers against eating certain frozen cooked mussel products made by Bantry Bay Seafoods, imported from Ireland, because they may be contaminated with azaspiracid toxins, a group of naturally occurring marine toxins known to cause nausea, vomiting, diarrhea, and stomach cramps.

Azaspiracid toxins are odorless, tasteless, and cannot be destroyed or neutralized by freezing or cooking, including boiling. Individuals who have experienced gastrointestinal symptoms such as those noted above after eating any of the products listed below should consult their health care professional. Symptoms typically occur within hours of consumption and persist for two to three days.

In July, two people in Washington state became ill after eating the company's "Mussels in a Garlic Butter Sauce." FDA tested unopened product from the same production lot and found that it contained the azaspiracid toxins.

Consumers should throw out the following Bantry Bay Seafood frozen cooked products with "Best before end" dates ranging from January 23, 2009, to November 15, 2009:

  • Mussels in a Garlic Butter Sauce
  • Mussels in White Wine Sauce
  • Mussels in Tomato and Garlic Sauce

The "Best before end" dates are displayed on the side of the box in the following format: MM:DD:YY. Products to be thrown out are marked with dates 01:23:09 through 11:15:09.

These products are sold frozen in 1 pound cardboard packages in stores throughout the United States.

The FDA also recommends that retailers and foodservice operators remove these products, and any food in which these products were used as an ingredient, from sale or service.

Azaspiracid toxins were an unknown marine toxin until 1995, when they were identified and linked to an outbreak of foodborne illnesses associated with consumption of Irish shellfish. The toxins have since been identified in other shellfish from the west coast of Europe. They have never been detected in shellfish harvested from U.S. waters.

Peppers Unlimited, Inc. Recalls La Torre Jalapeno Nacho Slices

Peppers Unlimited, Inc. of Fontana California is voluntarily recalling 580 cases of La Torre Jalapeno Nacho Slices because of possible product contamination caused by a chemical reaction between the product and the can potentially resulting in a swollen and/or ruptured container and significant off flavor.

The recalled products were sold nationwide through food distributors and food retailers.

The Jalapeno Nacho Slices were sold in 425 gram cans and bear the following description:

Yellow, red, white and black label with a picture of Jalapeno Slices.
The La Torre logo is in black and the "Jalapeno Nacho Slices" is in white.

The affected lot numbers are: AU4D6; AU4E6; AU515; AU516; AU520; AU526; AU5H6; AU5L6; AU5M6; AU8W6; AU955; AU9V5; AU9W5; AU9X5; AU9Y5; AU9Z5 (In some cases these lots may have a letter "A" or "B" proceeding the lot #, for example AU4D6A OR AU4D6B). Some cans may bear no lot number. The lot code is located on the bottom of the can.

To date no illnesses have been reported regarding this issue, however, this recall is being initiated as a precautionary measure and consumers should not consume these affected products even if the cans appear to be normal. Consumers who have the opened cans of the affected products or who have used them in recipes should immediately throw the cans and food away. Unopened cans may be returned to the place of purchase for a refund.

Mars Petcare US Issues Voluntary Recall of Limited Bags of PEDIGREE® Complete Nutrition Small Crunchy Bites at Albertsons in Southern California and L

Mars Petcare US today (August 8, 2008) announced a voluntary recall of limited bags of PEDIGREE® Complete Nutrition Small Crunchy Bites sold in Albertsons stores in Southern California and Las Vegas, Nevada. The pet food is being voluntarily recalled because of potential contamination with Salmonella. There have been no complaints or reports of injury resulting from consumption or handling of the recalled product.

The product should not be sold or fed to pets. Pet owners should dispose of product in a safe manner (example, a securely covered trash receptacle) and return the empty bag to the store where purchased for a full refund.

Salmonella can cause serious infections in dogs and cats, and, if there is cross contamination caused by handling of the pet food, in people as well, especially children, the aged, and people with compromised immune systems. Healthy people potentially infected with Salmonella should monitor themselves for some or all of the following symptoms: nausea, vomiting, diarrhea or bloody diarrhea, abdominal cramping and fever. On rare occasions, Salmonella can result in more serious ailments, including arterial infections, endocarditis, arthritis, muscle pain, eye irritation, and urinary tract symptoms. Consumers exhibiting these signs after having contact with this product should contact their healthcare providers.

Pets with Salmonella infections may be lethargic and have diarrhea or bloody diarrhea, fever, and vomiting. Some pets will have only decreased appetite, fever and abdominal pain. Animals can be carriers with no visible symptoms and potentially infect other animals or humans. The popcorn in these packages was incorrectly flavored with a white cheddar seasoning which includes milk ingredients; the seasoning may not be visibly apparent. Home Made Brand Foods Recalls Tuna Salad Because of Possible Health Risk

Home Made Brand Foods, 2 Opportunity Way, Newburyport MA 01950 is voluntarily recalling approximately 4890 pounds of "Ready-to-Eat" Tuna Salad with an expiration date of 8/19/08. This product has the potential to be contaminated with Listeria monocytogenes, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, Listeria infection can cause miscarriages and stillbirths among pregnant women.

The following products are subject to recall:

  • 99/ 5 lbs. units of "Home Made Brand Foods Tuna Salad" dated 8/19/08 expiration
  • 412/10 lb. units of "Stop and Shop Tuna Salad" dated 8/19/08 expiration
  • 366/12 oz. units of "Stop and Shop Tuna Salad" dated 8/19/08 expiration

The problem was discovered after routine sampling at the customer level and subsequent analysis of the product by an independent laboratory which found the product to be positive for Listeria monocytogenes. All product lots produced prior to and after the above listed affected lot were found to be negative for Listeria monocytogenes through independent laboratory analysis.

No illnesses have been reported to date in connection with this problem.

Description: This recall involves Learning Pots and Pans(tm) toy sets with stackable pots and pans, a lid and shape-sorting blocks. The toys have light and sound features that operate on three AA batteries. Model number G6685 is located on the bottom of the blue pan.

Description: The recalled bicycle pedals are black and were sold separately from bicycles. Pedal models include KĂ©O Classic, KĂ©O Sprint, KĂ©O HM and KĂ©O Carbon. The model name is printed in white on the side of the pedal. Date codes between January 2004 and December 2005 are included in this recall. The date code for the KĂ©O Classic, Sprint and Carbon pedals is on a dial stamped onto the pedal. The date code for the KĂ©O HM is on the bottom of the pedal, with the letters A through L corresponding to the month, and the numbers 4 and 5 indicating 2004 or 2005.

Newer Davenport cribs with model numbers beginning with 244-xxxx-xxxx that have one mattress support are not included in the recall.

C.F. Sauer Foods Issues Allergy Alert on Undeclared Milk in one lot of Gold Medal Spaghetti Sauce Mix – 1.37 ounce pouch

CF Sauer Foods Inc, of Richmond, VA is recalling all packages of Gold Medal Spaghetti Sauce Mix with the code date as follows: 8119R. People who have an allergy or severe sensitivity to milkrun the risk of serious or life threatening allergic reaction if they consume these products.

GOLD MEDAL SPAGHETTI SAUCE MIX was distributed through retail stores in Mississippi, Kentucky, and Alabama.

The Spaghetti Sauce Mix comes in 1.37 ounce pouch and was sold under the name Gold Medal displaying a UPC code of 0-52500-02104-7.

The recall was initiated after it was discovered that the Spaghetti Sauce Mix containing milk was distributed in packaging that did not declare the presence of milk. Subsequent investigation indicates the problem was caused by an oversight in the company's development process.

Actavis Totowa Announces Voluntary Recall at the Retail Level of All Drug Products Manufactured at its Little Falls, New Jersey Facility

Actavis Totowa LLC, a generic drug manufacturer, is announcing a voluntary recall, to the retail level, of all drug products manufactured at its Little Falls, New Jersey facility. This is a precautionary, voluntary action by Actavis following an inspection conducted by the Food and Drug Administration earlier this year.

The inspection at Little Falls revealed operations which did not meet the FDA's or Actavis' standards for good manufacturing practices. Actavis Totowa is voluntarily recalling these products from the pharmacy/retail level, which includes wholesalers and hospitals. The company has informed the FDA regarding this action.

This action is not prompted by product complaints or health hazards associated with the products, which are all prescription medications. Manufacturer for Wegmans Brand Sorbet Issues an Allergen Alert for Undeclared Milk on a Limited Number of Wegmans Brand Sorbets

The manufacturer of Wegmans Sorbet is recalling a limited number of 16 oz. Wegmans brand Sorbets because they may contain undeclared milk. People who have an allergy to milk run the risk of a serious or possible life-threatening allergic reaction if they consume the affected products listed below.

The products were distributed throughout Wegmans Food Markets located in New York, Pennsylvania, New Jersey, Virginia, and Maryland.

The sorbets are packaged in 16 oz. paperboard containers and sold in the supermarkets’ ice cream section. All four sorbet flavors: Lemon, Raspberry, Green Apple, and Pink Grapefruit, are affected by this recall. The affected products contain the following "BEST USED BY" date codes on the bottom of the container:

A review of records discovered that product containing milk was packaged in cartons that were not labeled for the presence of milk. Subsequent investigation indicates the cross contamination was caused by a breakdown in the company's production process, which has been corrected.

Consumers who have purchased Wegmans Sorbet with the affected code dates and flavors may return it to Wegmans for a full refund. Consumers with questions may contact the manufacturer at 1-800-873-7797 between the hours of 9:00am -5:00pm Monday thru Friday EST.
