Hazard: The tires on the portable garden hose reel can explode while being inflated, posing an injury hazard to consumers.

Incidents/Injuries: Rapid Reel(r) has received five reports of tires exploding while being inflated, resulting in lacerations to fingers, arms, and legs from flying pieces of plastic.

Description: This recall involves the Rapid Reel(r) portable garden hose reel carts and wagons. The cart has two tires and the wagon has four. The tires have a white plastic rim and a steel ball bearing hub or a black plastic bearing hub.

Sold by: Home repair retailers and distributors such as Ace Hardware, Amazon.com, and True Value Hardware nationwide from April 2005 through March 2006 for between $190 and $250.

Incidents/Injuries: There has been one report of a glass reflector falling. No injuries have been reported.

Description: The recall involves the Hi-Bay Indoor High-Intensity Discharge (HID) Fixtures, model numbers PG16A and PG21A. The fixture is intended for high-mounting heights (at least 20 feet above floor level) and is used in retail, warehouse and heavy manufacturing locations. The fixture's housing is made of die-cast aluminum.

Description: This recall involves the Taggies(tm) Strollin' Along stroller activity bars. The multicolored, fabric, and plastic activity bars feature a yellow giraffe, a purple hippo and a blue elephant attached to a 12" long elliptical base with straps that attach to a stroller. The Taggies(tm) and Earlyears(r) logos are located on woven and satin labels sewn into the seam on the lower left front of the activity bar.

Sold at: Specialty toy stores nationwide and on the Internet from February 2007 through July 2008 for about $23.

Cervélo SA Recalls Bicycle Forks Due to Fall Hazard

Hazard: The forks steerer can break during normal use, causing the rider to lose control, fall and suffer serious injuries.

Incidents/Injuries: Cervelo has received 12 reports of forks cracking or breaking, resulting in one consumer suffering a broken wrist and another suffering minor abrasions.

Description: The recalled forks have a clear coating over black painted carbon fiber, with the words "Wolf Superlite" and related logo just below the crown on each fork leg, and the letters "SL" on each leg above the fork blade dropouts. There is a True Temper CRT(tm) logo on the inside of both fork legs. The recalled forks could have been included on the following bicycle models: R3, R3 SL, Soloist Carbon, Soloist Carbon SL, and certain P3 Carbon framesets and complete bicycles.

Sold by: Independent bicycle retailers nationwide from November 2005 through July 2007 for about $475.

Electrical Wire Splices Recalled Due to Shock and Fire Hazards

Hazard: The splice can fail to hold the wires adequately together, posing a shock and fire hazard to consumers.

Incidents/Injuries: Gardner Bender has received one report of a recalled butt splice failing to hold wires together. No injuries have been reported.

Description: The recalled butt splices are used to connect electrical wires to one another. They are typically used for wiring small electrical appliances, like audio equipment, or in automotive applications. The splices are yellow insulated vinyl and measure about one inch long and ¼ inch wide. They were intended for use with 12-10 AWG wire. 12-10 is stamped on the side of the splices. Model numbers 10-126, or 21-126, and Gardner Bender are printed on the product's packaging. They were sold in packages of 8 or 50.

Sold at: Electrical distributors, hardware stores, and home centers nationwide from June 2005 through April 2008 for between $1 and $5.

Hobbico Inc. Recalls Batteries Used In Radio-Controlled Helicopters Due to Fire Hazard

Name of Product: Batteries Used In Radio-Controlled Helicopter Kits

Units: About 11,000

Importer/Distributor: Hobbico Inc., of Champaign, Ill.

Hazard: The rechargeable battery can overheat and catch fire while charging, igniting nearby combustible materials. This poses a fire hazard to consumers.

Incidents/Injuries: Hobbico has received 28 reports of batteries overheating and/or melting, including 27 reports of flames and minor property damage. No injuries have been reported.

Description: This recall involves the Electrifly lithium-polymer batteries (part #GPMP0401) sold individually or with the Heli-Max Axe CP-L radio-controlled helicopter kits. The kit includes a 23-inch long plastic helicopter, a battery, a radio transmitter, a balance charger, a car adapter, an AC wall adapter, decals that read "HMX", "AXE" and "Heli-Max" and training gear. "Electrifly" and "GPMP0401" are printed on the battery.

Sold at: Hobby Stores nationwide and online from November 2007 through March 2008 for about $35. The helicopter kits were sold for $200.

Manufacturer for Publix Brand Vanilla Wafers Issues a Recall for Undeclared Soy

The manufacturer of Publix Vanilla Wafers is recalling a limited number of 12oz boxes of Vanilla Wafers with a code date of 030409A as they may contain undeclared soy. Persons who have an allergy to soy run the risk of a serious or possible life-threatening allergic reaction if they consume the affected product. The UPC number may be found on the side label below the Nutrition Facts panel:

Publix Vanilla Wafers, UPC: 41415-00609

The product was distributed throughout the chain to include stores in Florida, Georgia, South Carolina, Alabama and Tennessee.

The vanilla wafers are packaged in 12 oz. paperboard boxes.

“The recall was initiated after the discovery of gingersnap cookies co-packaged with vanilla wafers. This resulted in the undeclared soy ingredient.” said Roy Peterson, Toufayan Bakery general manager. "As part of Publix commitment to food safety, they are asking customers to return the product for a full refund or replacement. There have been no reported cases of illness. Consumers with questions may contact Publix at 1-800-242-1227."

Peppers Unlimited, Inc. Recalls La Torre Jalapeno Nacho Slices

Peppers Unlimited, Inc. of Fontana California is voluntarily recalling 580 cases of La Torre Jalapeno Nacho Slices because of possible product contamination caused by a chemical reaction between the product and the can potentially resulting in a swollen and/or ruptured container and significant off flavor.

The recalled products were sold nationwide through food distributors and food retailers.

The Jalapeno Nacho Slices were sold in 425 gram cans and bear the following description:

Yellow, red, white and black label with a picture of Jalapeno Slices.
The La Torre logo is in black and the "Jalapeno Nacho Slices" is in white.

The affected lot numbers are: AU4D6; AU4E6; AU515; AU516; AU520; AU526; AU5H6; AU5L6; AU5M6; AU8W6; AU955; AU9V5; AU9W5; AU9X5; AU9Y5; AU9Z5 (In some cases these lots may have a letter "A" or "B" proceeding the lot #, for example AU4D6A OR AU4D6B). Some cans may bear no lot number. The lot code is located on the bottom of the can.

To date no illnesses have been reported regarding this issue, however, this recall is being initiated as a precautionary measure and consumers should not consume these affected products even if the cans appear to be normal. Consumers who have the opened cans of the affected products or who have used them in recipes should immediately throw the cans and food away. Unopened cans may be returned to the place of purchase for a refund.

Mars Petcare US Issues Voluntary Recall of Limited Bags of PEDIGREE® Complete Nutrition Small Crunchy Bites at Albertsons in Southern California and L

Mars Petcare US today (August 8, 2008) announced a voluntary recall of limited bags of PEDIGREE® Complete Nutrition Small Crunchy Bites sold in Albertsons stores in Southern California and Las Vegas, Nevada. The pet food is being voluntarily recalled because of potential contamination with Salmonella. There have been no complaints or reports of injury resulting from consumption or handling of the recalled product.

The product should not be sold or fed to pets. Pet owners should dispose of product in a safe manner (example, a securely covered trash receptacle) and return the empty bag to the store where purchased for a full refund.

Salmonella can cause serious infections in dogs and cats, and, if there is cross contamination caused by handling of the pet food, in people as well, especially children, the aged, and people with compromised immune systems. Healthy people potentially infected with Salmonella should monitor themselves for some or all of the following symptoms: nausea, vomiting, diarrhea or bloody diarrhea, abdominal cramping and fever. On rare occasions, Salmonella can result in more serious ailments, including arterial infections, endocarditis, arthritis, muscle pain, eye irritation, and urinary tract symptoms. Consumers exhibiting these signs after having contact with this product should contact their healthcare providers.

Pets with Salmonella infections may be lethargic and have diarrhea or bloody diarrhea, fever, and vomiting. Some pets will have only decreased appetite, fever and abdominal pain. Animals can be carriers with no visible symptoms and potentially infect other animals or humans. If your pet has consumed the recalled product and has these symptoms, please contact your veterinarian.

The recall was initiated after it was discovered that the Spaghetti Sauce Mix containing milk was distributed in packaging that did not declare the presence of milk. Subsequent investigation indicates the problem was caused by an oversight in the company's development process.

Actavis Totowa Announces Voluntary Recall at the Retail Level of All Drug Products Manufactured at its Little Falls, New Jersey Facility

Actavis Totowa LLC, a generic drug manufacturer, is announcing a voluntary recall, to the retail level, of all drug products manufactured at its Little Falls, New Jersey facility. This is a precautionary, voluntary action by Actavis following an inspection conducted by the Food and Drug Administration earlier this year.

The inspection at Little Falls revealed operations which did not meet the FDA's or Actavis' standards for good manufacturing practices. Actavis Totowa is voluntarily recalling these products from the pharmacy/retail level, which includes wholesalers and hospitals. The company has informed the FDA regarding this action.

This action is not prompted by product complaints or health hazards associated with the products, which are all prescription medications. Patients who may have these medicines in their possession should continue to take them in accordance with their prescriptions, as the risk of suddenly stopping needed medication may place patients at risk.
