Name of Product: Gerber(r) Gator(r) Combo Axe
Units: About 103,000
Importer: Gerber Legendary Blades, of Portland, Ore., a division of Fiskars Brands Inc., of Madison, Wis.
Hazard: The knife in the axe handle can come out when the axe is used for chopping or hammering, posing a laceration hazard to the user.
Incidents/Injuries: Gerber has received five reports of laceration injuries. All required stitches.
Description: This recall involves the Gerber Gator Combo Axe, which has a 7-inch knife in the hollow axe handle. The axe is approximately 8.75 inches long with an axe head that is 4.75 inches x 2.7 inches. The axe handle is marked with the "Gerber" trademark. The Gerber Combo Axe II, which holds a saw in a longer axe handle, is not included in this recall.
Sold at: Retail stores nationwide, including The Sportsman's Guide, Dick's Sporting Goods and Bass Pro Shops/American Rod & Gun and online since March 2005 for around $28 - $60.
Manufactured in: Taiwan
Remedy: Consumers should immediately remove the knife from the axe handle and contact Gerber to receive a free handle cap, which will hold the knife in the axe handle during transport and storage, instructions for the use of the handle cap, and a warning label to affix on the axe head.

Ocean Technology Systems Recalls Guardian Full-Face Diving Masks Due to Drowning Hazard

Name of Product: Guardian full-face diving masks
Units: About 1,700 in the U.S. and 80 in Canada
Manufacturer: Undersea Systems International Inc. dba Ocean Technology Systems, of Santa Ana, Calif.
Hazard: The purge assembly on the diving mask can disengage from the regulator, resulting in loss of air to the diver. This poses a drowning hazard to the consumer.
Incidents/Injuries: The firm has received one report of a disengaged assembly. No injuries have been reported.
Description: This recall involves Guardian full-face diving masks with serial numbers 9051284 through 100070954. The serial number is printed on the main regulator body. The diving masks were sold in black with blue accent colors. The Ocean Technology Systems' logo is affixed to the front of the mask.
Sold by: Diving equipment retailers and direct sales nationwide from September 2010 through November 2010 for about $800.
Manufactured in: Taiwan
Remedy: Consumers should immediately stop using the diving masks and contact Ocean Technology Systems for instructions on conducting a test of the regulator and returning the masks for a free repair.

Vitalabs, Inc. Issues Voluntary Recall for Specific Lots of Whey Protein Isolate
Vitalabs, Inc., Jonesboro, GA is voluntarily recalling four specific lot numbers of Whey-26 Protein Powder due to concerns over potential Salmonella contamination.
Salmonella is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (e.g., infected aneurysms), endocarditis and arthritis.
This recall extends to various parts of the United States including Alabama, Arizona, California, Colorado, Georgia, Florida, Illinois, Kentucky, Louisiana, Maryland, Missouri, New Mexico, New York, North Carolina, Ohio, Oklahoma, Pennsylvania, South Carolina, Tennessee, Texas, Utah, and Washington and outside the United States in Aruba, Ontario and Poland. The recalled product was offered through a variety of distribution channels including health food stores, clinics, fitness centers and internet based companies.

Consumer complaint leads to recall of O.N.E. Coconut Water
Georgia Agriculture Commissioner Gary W. Black alerts consumers to the recall of some coconut water following a consumer complaint received by the Georgia Department of Agriculture.
Georgia Department of Agriculture Compliance Specialists collected samples of the product, O.N.E. Coconut Water, for laboratory analysis after being alerted of potential mold in the product by the consumer. It was discovered that potential oxidation of the product may permit the growth of mold causing the coconut water to be discolored and have an off flavor.
Upon this discovery, the Department began working cooperatively with the company and the U.S. Food and Drug Administration. Out of an abundance of caution, the company issued a recall of the product.
Georgia Department of Agriculture Compliance Specialists will continue to monitor facilities for the presence of the recalled product.

FOR IMMEDIATE RELEASE – ONE World Enterprises, LLC is recalling O.N.E. Coconut Water, packaged in 11.2 oz Tetra Pak cartons with Lot code 12E1004, UPC # 894991001104 and Sell-by date of May 1, 2011, due to the potential oxidation of the product that may permit the growth of mold. The affected product is isolated to a single lot number and the company is currently utilizing Corrective And Preventative Action (CAPA) procedures to identify why this problem exists, which was discovered after receiving a small number of consumer complaints.
The affected product was distributed to various retail outlets in Florida, Georgia, Indiana, New York, South Carolina, and Tennessee.

To date all internal testing at the manufacturing plant indicates no contamination. Therefore, the company is investigating possible issues that may have occurred during transport thereby causing potential oxidation.

Importer: Southern Enterprises Inc. (SEI), of Coppell, Texas
Hazard: Heat from the operating unit causes the plastic mounting screws to deform causing the unit to fall from the wall, posing a fall and fire hazard.
Incidents/Injuries: SEI has received reports of 21 incidents of the product detaching from wall and falling, heat damage, and/or fire. Two reports of personal injuries, including a knee injury and broken toes.
Description: This recall involves Colin Cowie dual-positioning, wood wall-mount, gel-fuel fireplace with item No. 955-074. The wooden wall mount fireplace has an espresso-colored finish with copper, silver or antique gold finished metal trim. It may be hung in a horizontal or vertical position. This recall involves units manufactured in July 2010. Lot number SEI/07/001 can be found on a label on the rear of the unit in the upper right hand corner when horizontal.
Sold at: Home Shopping Network between October and November 2010 for about $250.

Sunjoy Industries Recalls Outdoor Wood Burning Fireplaces Sold Exclusively at Lowe's Stores Due to Fire Hazard

Name of Product: Freestanding Steel Outdoor Fireplaces
Units: About 20,000 in the United States and 400 in Canada
Manufacturer: Sunjoy Industries Group Limited, of Steubenville, Ohio
Hazard: The decorative bronze powder coat finish on the fireplace chimney can ignite during use, posing a fire hazard to consumers.
Incidents/Injuries: Sunjoy has received 14 reports of the chimney's decorative powder coat finish catching fire, resulting in one report of melted siding. No injuries have been reported.
Description: This recall involves "Garden Treasures Living" steel outdoor fireplaces with a bronze finish chimney and slate colored accents. The wood burning fireplace is approximately 24 inches deep by 35 inches wide and 57 inches tall, has two glass doors and a tile back inside. Item number 0027705 and model number L-OF082PST-3 are printed on the front page of the product's instruction manual. No other "Garden Treasures Living" brand outdoor fireplaces are included in this recall.

REMEDYTEAS Voluntarily Recalls Peppermint Organic Herbal Tea And Organic Herbal Tea Blend Because Of Possible Health Risk
RemedyTeas announced a voluntary recall of 20 lbs of Peppermint Organic Herbal Tea produced by Aromatics Inc., Basin City, WA, because it has the potential to be contaminated with Salmonella. Salmonella is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting, and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis, and arthritis. The recall was as the result of a batch sample testing program by the vendor after it received a notification of possible contamination which revealed that the finished products contained the bacteria. The vendor has ceased the production and distribution of the product as the FDA and the vendor continue their investigation as to the origin of the contamination. RemedyTeas has not caused the contamination of Salmonella and RemedyTeas has ceased distribution of products affected by the vendor’s recall.
The #137 Peppermint Organic Herbal Tea or #120 Tranquility Herbal Tea were distributed in the RemedyTeas retail store and through mail and internet orders. #137 Peppermint Organic Herbal Tea or #120 Tranquility Herbal Tea is sold measured to customers orders in retail stores and sold in 2 oz pre-packaged pouches by mail order or internet. There are no lot/batch identifying markings on the store or ecommerce packaging. Retail stock of #137 Peppermint Organic Herbal Tea was sold between January 18, 2011 and March 4, 2011. Retail stock of #120 Tranquility Herbal Tea was sold between February 22, 2011 and March 4, 2011. Any purchases not within those dates are not affected by this recall.
While no illness related to this voluntary recall has been reported to date, any potential health risk is significantly reduced when following the printed brewing instructions on the package or available online, i.e. using 205 F water temperature.
Consumers who have purchased #137 Peppermint Organic Herbal Tea or #120 Tranquility Herbal Tea are urged to return it to the place of purchase for a full refund. Consumers may also contact RemedyTeas Customer Service at 206 323-4832 (M – F from 7.00 A.M. until 11:00 P.M., PST) for instructions on how to return the product.

McCormick & Company Recalls Golden Dipt® Tempura Seafood Batter Mix Due To Unlabeled Milk Ingredient
McCormick & Company, Incorporated (NYSE:MKC) has announced a voluntary recall of Golden Dipt® Tempura Seafood Batter Mix 8 OZ, with UPC Code 4123470132 and “BEST BY” date of OCT 08 12 H. A very limited number of packages of Golden Dipt® Tempura Seafood Batter Mix with this date code contain an undeclared milk ingredient. People who have an allergy to milk run the risk of serious or life threatening allergic reactions if they consume this product.
The recalled cartons of Golden Dipt® Tempura Seafood Batter Mix were distributed to grocery stores in Florida, Georgia, Kentucky, Massachusetts, Maryland, Maine, Michigan, North Carolina, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Tennessee, and Wisconsin as well as internationally to the Cayman Islands, beginning on October 21, 2010. The affected product is packaged in 8 OZ cartons, and the date code is found printed in white ink on the bottom of the carton.
The recall was initiated after the discovery of a packaging error that resulted in a pouch of Cajun Style Fish Fry mix (which contains a milk ingredient) being packaged in the Golden Dipt® Tempura Seafood Batter Mix carton. The ingredient statement on the package does not list milk as an ingredient.
The “BEST BY” date of OCT 08 12 H is the only date code affected by the recall. No illnesses or allergic reactions have been reported to date and no other McCormick products are involved in this recall.
All grocery outlets that sell Golden Dipt® Tempura Seafood Batter Mix are being notified to remove the affected product (UPC Code 4123470132 and date code BEST BY OCT 08 12 H) from their shelves immediately. Consumers do not need to return the product to the store where it was purchased. Instead, consumers are urged to contact McCormick’s Consumer Affairs team at 1-800-632-5847, weekdays from 9:30 AM to 9:00 PM, or weekends from 11:00 AM to 7:00 PM (Eastern Time), for a replacement or full refund, as well as instructions on what to do with the product.
This recall is being made with the knowledge of the Food and Drug Administration. The Company is also issuing an alert through the Food Allergy & Anaphylaxis Network.

Manhattan Group Recalls Parents Wooden Activity Toys Due to Choking Hazard

Hazard: Wooden pegs on the xylophone activity can come loose, posing a choking hazard to young children.
Incidents/Injuries: CPSC has received one report of a child putting a peg in his mouth. No injuries have been reported.
Description: The Parents(r) Busy Time Activity Centers(tm) is a wooden activity center cube-shaped toy that has a multi-colored bead run on the top. Activities on each side include a 4-tone xylophone and rasp, butterfly and caterpillar spinners, zig-zag drum, castanets, and pockets with "alligator," "bear," and "cat" pillow characters. Only items with wooden pegs bearing date code 400090GC are involved in this recall. The date code can be found on the bottom of the product packaging near the UPC code and on the tags on the bear character.
Sold at: Specialty and gift stores nationwide and through mail order catalogs and online and in Canada from December 2010 through February 2011 for about $90.

Hazard: Pressure from the metal pot holder against the coffee/tea maker can cause the glass to break unexpectedly, posing burn and laceration hazards.
Incidents/Injuries: IKEA has received one report in the U.S. of a glass coffee/tea maker breaking. No injury was reported. IKEA has received 19 additional reports, outside of the U.S., of the glass coffee/tea makers breaking, resulting in 12 reports of burn injuries from spilt coffee/tea and one report of a laceration injury.
Description: This recall involves FORSTA coffee/tea makers sold in sizes 0.4 and 1 liters. The coffee/tea maker is a press pot comprising a handmade clear glass pot, metal holder and black plastic top and handle. Coffee/tea makers included in this recall have supplier number 20325, "IKEA" and "Made in China" etched on the bottom of the metal holder.
Sold exclusively at: IKEA stores nationwide from January 2010 through December 2010 for between $6 and $10.

Incidents/Injuries: Rhino Toys has received one report of a broken C-link. No injuries have been reported.
Description: This recall involves infant rattles called OBall Links & Mini Rattles™. The soft plastic OBall is 2 1/2 inches in diameter and has beads inside a small plastic bubble that make the rattle sound. The OBall is attached to a 7 1/2 inch soft plastic chain by C-shaped plastic links on both ends. "OBall" is stamped into the top of the plastic chain.
Sold at: Toys R Us and other specialty toy stores nationwide from April 2010 through December 2010 for about $8.

Burlington Coat Factory Recalls Slow Cookers Due to Fire Hazard

Hazard: The slow cooker's control panel can overheat and melt, posing a fire hazard.
Incidents/Injuries: The manufacturer has received 60 reports of the control panels smoking, melting and sparking, and three reports of panels catching fire. Fourteen incidents resulted in minor damage to countertops. No injuries have been reported.
Description: This recall involves Bella Kitchen 5-quart programmable slow cookers. The slow cookers are black with "Bella Kitchen" printed on the control panel. Only slow cookers with model number WJ-5000DE and date codes 0907 or 0909 are included in this recall. The model number and the four-digit date code are printed on a label on the underside of the product.
Sold at: Burlington Coat Factory stores from June 2010 through December 2010 for $20.

FAB/Starpoint Recalls Circo Beaded Door Curtains Due to Risk of Strangulation; Sold Exclusively at Target
The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Name of Product: Circo beaded door curtains
Units: 79,000
Manufacturer: FAB/Starpoint LLC, of New York, N.Y.
Retailer: Target Corp., of Minneapolis, Minn.
Hazard: Strangulations can occur when a child plays with the beaded strands by wrapping them around their necks or by creating loops in which they can insert their heads. Also, children can get entangled in the strands, which are prone to entangle, just by running through the doorway.
Incidents/Injuries: The firm has received three reports of entanglement. In January 2009, a nine-year old girl was entangled while passing through the beads and nearly choked as she continued walking through the curtain. In January 2010, a six-year old girl in Reno, Nev. became entangled in the bead strands as she passed through them and suffered slight lacerations to her neck. In April 2010, a girl became entangled as the beads swung back behind her father who had walked through the beads before her. She was pulled off her feet and momentarily suspended before she regained her footing and she received scratches to her neck.
Description: The Circo pink door curtain is a set of two beaded door curtains, each of which is 12 inches wide and 72 inches long. The warnings on the packaging include: "Not for use in areas with children under 5 years of age. Plastic ornaments may pose strangulation or entanglement hazard. Not for use near cribs or playpens...". The photograph on the packaging shows an installed door curtain at the doorway of a girl's bedroom with pink wall color, child-themed lamp and clock. There is a CAUTION statement included in the installation instructions that states: "Do not tie bottom to form loops."
Sold exclusively at: Target stores nationwide and target.com from January 2009 through May 2010 for between $13 (at target.com) and $15 in stores.

Remedy: Consumers should immediately take down the curtains and contact FAB/Starpoint for a full refund. FAB/Starpoint will provide a postage-free package to consumers to return the door curtain.

Lennox Hearth Products Recalls Natural Gas Log Set Burner Assemblies Due to Risk of Carbon Monoxide Poisoning

Name of Product: Lennox Shadowdance Natural Gas Log Set Burner Assemblies
Units: About 3,200
Manufacturer: Lennox Hearth Products LLC, of Nashville, Tenn.
Hazard: A crack can develop at the gas valve connection allowing natural gas to leak while the burner is in use, posing a risk of carbon monoxide poisoning.
Incidents/Injuries: Lennox has received 20 reports of cracks at the gas valve connection. No injuries have been reported.
Description: This recall involves gas burners sold with Lennox brand Shadowdance natural gas log set burner assemblies. The product is used in a wood-burning fireplace or a ventless firebox enclosure. The assemblies were sold under model numbers LSVFSD-18, LSVFSD-24 and LSVFSD-30, and include a burner and ceramic-fiber log set. Replacement gas burners also were sold separately under model number LSVFSD-NG. Burners included in this recall have serial numbers that begin with 6407, 6408, 6409 and 6410A through 6410G. The burner's model and serial numbers are printed on a metal identification plate attached to the burner.
Sold by: Fireplace and HVAC retailers and installers nationwide from August 2007 through December 2010 for between $700 and $850.
Manufactured in: USA
Remedy: Consumers should immediately stop using the recalled natural gas log set burner assemblies. Consumers can contact Lennox to arrange for a free replacement of the burner.
