Alpine Slicing and Cheese Conversion Company in Wisconsin is voluntarily recalling “HEB Queso Cotija (Grated)”, “El Viajero Queso Quesadilla (Shredded)”, “El Viajero Queso Cotija (Grated)”, and “BMF Queso Quesadilla (Shredded)” because it has the potential to be contaminated with Listeria monocytogenes, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, Listeria infection can cause miscarriages and stillbirths among pregnant women.

No illnesses have been reported.

“HEB Queso Cotija (Grated)” – 8 oz. packages, Distributed in Texas. UPC code 0 4122024846 9 and sell-by dates of 08FEB12 and 09MAR12, which are located on the back of the package. Product was distributed between October 27, 2011 and December 23, 2011.
“El Viajero Queso Quesadilla (Shredded)” -- 8 oz packages, Distributed in Wisconsin, Illinois, Tennessee and Florida UPC code 0 7282014045 7 (located on the bottom left of front label) and sell by dates of 24FEB12, 09MAR12 or 15MAR12, which are located on the back of the package. Product was distributed between November 1, 2011 and January 3, 2012.
“El Viajero Queso Cotija (Grated)” -- 4 lb. packages, Distributed in Indiana and Minnesota. Use-by code 09MAR12, located on the back of the package. Product was distributed between November 11, 2011 and January 3, 2012.
“BMF Queso Quesadilla (Shredded)” -- 5 lb. packages, Distributed in New York. Sell by date of 16FEB12, 01MAR12 or 15MAR12, on product cases. Product was distributed between October 21, 2011 and January 3, 2012.

Alpine Slicing and Cheese Conversion discovered the issue during testing of its products by an independent laboratory. Laboratory testing has confirmed that certain products processed in the shred line contained Listeria monocytogenes. Therefore, other products produced on the same line, even if they have been tested and found to be negative, have the potential to be contaminated.

Alpine Slicing and Cheese Conversion has contacted the FDA and issued the recall. Alpine Slicing and Cheese Conversion has ceased the production and distribution of the products as FDA and Alpine Slicing continue their investigation into the matter. All remaining inventory has been quarantined.

Green Valley Food Corp. is recalling approximately 35,159 cases of a variety of products because they have the potential to be contaminated with Listeria monocytogenes. Random samples tested positive for Listeria monocytogenes, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, Listeria infection can cause miscarriages and stillbirths among pregnant women.

Any customer who received any of the products on 12/07/2011 to 1/1/2012 are affected in this recall and/or if the items have a use by date ranging from 12/22/2011 to 1/17/2012. The original recall was initiated on 12/23/11 and 12/24/11, we are adding the additional items mentioned in this recall.

The sprouts affected in this recall were distributed via truck deliveries to all customers in Texas. Our customers consist on grocery store distribution centers and food service customers

Till this present day there has bee n no related i l l nesses CONFIRMED because of this recall . This i s a cautionary measure taken by G reen Vall ey Food C orp. to assure safe and quali ty products are being distributed by our facility.

On 12/12/2011 a random sample was taken from a customer the product tested positive for Salmonella. Green Valley Food Corp. voluntarily recalled the alfalfa-based products when we were notified of the positive result on 12/23/2011 and 12/24/2011. On 12/21/2011 random samples were taken of our facility, several samples had a positive result for Listeria monocytogenes. To assure our customers of safe products, Green Valley Food Corp., is voluntarily recalling the additional items listed, which consist of “Let’s Grow Healthy Together”, “Green Valley Food Corp.” and “BroccoSprouts” branded products. This is a voluntary cautionary measure taken by Green Valley Food Corp. to assure all customers and consumers of any issues with our products. We are voluntarily requesting this recall and are working closely with the FDA to assure all consumers our products are safe.

If you are a direct customer affected by this recall you will be receiving a letter asking for the quantities you were shipped and their whereabouts. Consumers who purchased these product at your local grocery store should dispose of the products immediately in a trash receptacle.

HP Expands Recall of Notebook Computer Batteries Due to Fire Hazard Consumers urged to recheck notebook models and batteries

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Lithium-ion batteries used in HP and Compaq notebook computers

Units: About 162,600 additional batteries (54,000 and 70,000 batteries were previously recalled in May 2010 and May 2009, respectively)

Importer: Hewlett-Packard Company, of Palo Alto, Calif.

Hazard: The recalled lithium-ion batteries can overheat and rupture, posing fire and burn hazards to consumers.

Incidents/Injuries: Since the May 2010 recall expansion, HP has received 40 additional reports of batteries that overheated and ruptured, resulting in seven burn injuries, one smoke inhalation injury, and 36 instances of property damage.

Description: The recalled lithium-ion rechargeable batteries are used with various model series of HP and Compaq notebook computers and include batteries that consumers were informed were not included in previous recalls. The chart below includes all notebook model numbers associated with batteries recalled to date. The computer model number is located at the top of the service label on the bottom of the notebook computer. Not all batteries matching the bar codes are being recalled.

After removing the recalled battery from their notebook computer, consumers may use the AC adapter to power the computer until a replacement battery arrives.

Additional Fingertip Amputations and Lacerations Prompt Reannouncement of November 2009 Recall of Strollers by Maclaren USA

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today reannounced a voluntary recall of the following consumer product. Consumers who have not yet obtained the repair should do so immediately. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Maclaren strollers (sold prior to November 2009)

Units: About one million (this product was recalled in November 2009)

Distributor: Maclaren USA, Inc., of South Norwalk, Conn.

Hazard: The stroller's hinge mechanism poses a fingertip amputation and laceration hazard to the child when the consumer is unfolding/opening the stroller.

Incidents/Injuries: Maclaren has received a total of 149 reported incidents with the strollers, including 37 reported injuries that occurred after the stroller was recalled in November 2009. These reported injuries include five additional fingertip amputations, 16 additional lacerations and 16 additional fingertip entrapments/bruising. At the time of the original recall, there were 15 incidents, including 12 reports of fingertip amputations in the United States.

Description: This reannouncement involves all Maclaren single and double umbrella strollers sold prior to November 2009. The word "Maclaren" is printed on the stroller. Maclaren strollers sold after May 2010 have a different hinge design and are not affected by this announcement.

Sold at: Juvenile product and mass merchandise retailers nationwide from 1999 through November 2009 for between $100 and $360.

Remedy: Consumers who have not installed the hinge covers should immediately contact Maclaren USA to receive the free repair kit.

Six L's Voluntarily Recalls Grape Tomatoes Because of Possible Health Risk

Six L's of Immokalee, Fla. is voluntarily recalling a single lot of grape tomatoes, because they have the potential to be contaminated with Salmonella, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. The strain of Salmonella has not been determined and no illnesses have been reported.

The specific lot was packed on April 11 and was comprised of grape tomatoes that can be identified by Cherry Berry lot code DW-H in either in clam shells or 20 lbs. cardboard containers. The product was distributed to North Carolina, South Carolina, Alabama, Florida, New Jersey, New York, Michigan, Pennsylvania, California, Georgia and Canada, and reached consumers through retail stores and restaurant distribution.

The contamination was detected through a random sample obtained by the USDA at a distributor in New York. The product is from a farm in Estero, Fla. which has since ceased production of that commodity.

The recall does not include any other tomatoes or produce distributed by Six L's. Consumers who have purchased the specific lot of grape tomatoes mentioned above are urged to return it to the place of purchase for a full refund. Consumers with questions may contact Six L's at 877-606-1821, ext. 3955, 24 hours a day 7 days a week.

Taylor Farms Pacific, Inc. of Tracy, CA has been notified by grower Six L’s that a specific lot of grape tomatoes supplied to Taylor Farms Pacific may be contaminated with Salmonella. This product has been recalled by Six L's.

This lot of grape tomatoes was used in the following products made by Taylor Farms Pacific for Albertsons, Raley's, Safeway, Savemart, Sam's Club, & Walmart and is being voluntarily recalled as a precautionary measure. No illnesses have been reported.

Salmonella is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy people may experience fever, nausea, vomiting, diarrhea (which may be bloody), and abdominal pain. Defibtech Announces a Voluntary Recall of DDU-100 series AEDs

Defibtech, LLC is initiating a worldwide voluntary recall of certain DDU-100 series semi-automatic external defibrillators (AEDs) sold under the Lifeline AED and ReviveR AED brand names, including 65,885 AEDs distributed in the United States. This recall affects only DDU-100 Series AEDs shipped with 2.004 software or earlier. This corrective action addresses two possible conditions, which in rare cases may cause an affected AED to cancel shock during the charging process and not provide therapy which may result in failure to resuscitate the patient. Both conditions are not detectable by the periodic self test.

Condition 1: In rare instances, the AED may cancel charge in preparation for a shock. Based on field data, the odds of an affected AED having this happen are less than a 1 in 400,000 chance per month for any given AED.

A subset of AEDs (less than 11%) that are affected by Condition 1 are also affected by Condition 2: In rare instances, the AED may cancel charge in preparation for a shock in very high humidity conditions. The only reported cases were in environments of greater than 95% relative humidity or condensing conditions. Based on field data, the odds of an affected AED having this happen are less than a 1 in 250,000 chance per month for any given affected AED.

Defibtech will provide customers with a free software upgrade to address these issues. The correction to the AED will be able to be performed at the location where the AED is deployed.

Because both of these conditions occur very rarely, it is recommended that customers keep their AEDs in service until they have performed the software upgrade. Full instructions and recommendations are being mailed to affected customers. Organic Food Bar, Inc. of Fullerton, CA is issuing a voluntary recall on the specific listed lots of RAW Organic Food Bars listed below, because they may contain undeclared peanut proteins from cashew butter supplied by one of the vendors. These bars are being recalled because they are made with the same lot of cashew butter that was used in previously tested chocolatey chocolate chips bars. People who have an allergy or severe sensitivity to peanuts should return these lots to the place of purchase.

Quinault Tribal Enterprise Recalls All Canned Seafood Products Because of Possible Health Risk

Quinault Tribal Enterprise of Taholah, WA is recalling all canned seafood products including all Salmons, smoked and non-smoked; Tuna, smoked and non-smoked; Smoked Sturgeon; Minced Razor Clams; smoked Razor Clams; and smoked Steelhead. The canned seafood products are being recalled because they were not adequately processed according to the U.S. Food and Drug Administration's low-acid canned food regulations. Canned seafood that is not processed following those food safety requirements may be contaminated with harmful microorganisms that can cause serious and possibly life-threatening illness. Consumers are warned not to use the products even if it does not look or smell spoiled.

No illnesses or injuries have been reported to date.

The recalled products were distributed nationwide through distributors and retail stores.

The products are packaged in metal cans in various sizes (5oz, 6oz, 6.5oz, and 7oz) and labeled under the Quinault Pride or Quinault Tribal Enterprises brand. All manufacturing codes are subject to this recall.

This issue was found during FDA inspection at Quinault Tribal Enterprises and Quinault Tribal Enterprises is cooperating with FDA in conducting this voluntary recall.

Hazard: The carburetors on both products develop fuel leaks and can ignite when exposed to an ignition source, posing a fire or burn hazard.

Incidents/Injuries: There have been about 500 reports of carburetor leaks. There were no reports of fire or injury.

Doctor's CarbRite Diet Issues Allergy Alert on Certain Chocolate Brownie Flavored Bars

Doctor’s CarbRite Diet of New Brunswick, NJ is voluntarily recalling all lots of the 2 oz. Chocolate Brownie Doctor’s CarbRite Diet Bar that are missing the allergen statement, “Allergy Alert: Made on equipment that also processes soy, milk, egg, peanuts and tree nuts”, due to possible peanut allergen cross-contamination. This allergy statement would be found at the end of the Ingredients list. People who have an allergy or severe sensitivity to a specific type of allergen (e.g., peanuts, tree nuts {chestnuts, brazil nuts, walnuts, hazelnuts, pecans, pine nuts, cashews}, eggs, and sulfites) run the risk of serious or life-threatening allergic reaction if they consume these products.

The recalled lots of the Chocolate Brownie Doctor’s CarbRite Diet Bar extend to all fifty states and the bar was offered through a variety of distribution channels including health food stores, gyms, and internet-based e-commerce sites.

This recall affects only the Chocolate Brownie flavor for Doctor’s CarbRite Diet Bars, those marked with the UPC # 0 39442 08112 4.

Chocolate Brownie Doctor’s CarbRite Diet Bars have been produced for over 10 years for sale without any serious adverse events or allergic reactions reported in the United States.

The recall was initiated after it was discovered that the certain Chocolate Brownie flavored bars were distributed in packaging that omitted the appropriate allergen warnings: “Allergy Alert: Made on equipment that also processes soy, milk, egg, peanuts and tree nuts.” Examination of current lots of Chocolate Brownie revealed that the correct allergen warnings do appear.

This recall is being conducted with the knowledge of the Food and Drug Administration.

June Deadline Set for End to Globe Fire Sprinkler Model J Sprinkler Recall Property Owners Should Act Now To Request Replacement of Sprinkler Heads

The U.S. Consumer Product Safety Commission (CPSC) and Globe Fire Sprinkler Corp. are urging residential and commercial property owners who have model J dry-type series fire sprinklers covered under the previously-announced recall and have not yet filed a claim to replace them, to contact Globe Fire Sprinkler immediately. Home and commercial property owners should immediately submit their claim, as the program for receiving replacement heads will end on June 1, 2011.

The voluntary replacement program, which was first announced in June 2007, included about 300,000 model J series dry fire sprinklers manufactured by Globe Fire Sprinkler due to a concern that these sprinklers can deteriorate over time and can fail to operate in a fire situation.

To participate in the replacement sprinkler program, property owners must submit "Proof of Claim" and "Waiver and Release of Claims" forms to Globe Fire Sprinkler Corporation postmarked by June 1, 2011. Valid claims received prior to the deadline will qualify for replacement sprinkler heads.

Consumers can obtain the claim forms, additional information on identifying the sprinklers covered by this recall and instructions on filing a claim by visiting www.globesprinkler.com or by calling Globe Fire Sprinkler Corporation at (800) 248-0278 between 8 a.m. and 5 p.m. ET Monday through Friday.

This deadline relates only to the Globe model J dry type sprinklers recalled on June 12, 2007. It does not involve additional models of sprinklers other than those named in the June 2007 recall announcement. Building owners need not take any further action if they have: (1) had replacement sprinklers installed by Globe Fire Sprinkler; (2) submitted a claim to Globe Fire Sprinkler that has been deemed complete and installation of the replacement heads is pending; or (3) confirmed that their sprinkler heads are not covered by the Globe Fire Sprinkler model J dry type recall. Building owners who have previously submitted an incomplete claim will be sent a reminder notice reminding them to send in any missing information or materials by the June 1, 2011 deadline or else their claim will be void.

Name of Product: Marineland Stealth and Stealth Pro Aquarium Heaters

Units: About 1.2 million

Importer: United Pet Group, of Cincinnati, Ohio

Hazard: A wiring problem can cause the aquarium heaters to overheat or break during normal use, damaging the aquarium and posing fire and laceration hazards to consumers. Overheating can cause the heater to shatter or the acquarium glass to break.

Incidents/Injuries: United Pet Group has received 38 reports of fires resulting in property damage and 45 reports of broken aquarium glass. United Pet Group has received one report of a consumer who suffered an eye injury when the aquarium heater forcefully broke while he held it.

Description: The recall involves the following Marineland Stealth and Stealth Pro acquarium heaters. The heaters are black plastic tubes and have a temperature adjustment knob at the top. The model name "Stealth" (in white letters) or "Stealth Pro" (in red letters) is printed on the side of the heater. The model number and the wattage rating are printed below the model name. Stealth Pro heaters were also sold as part of aquarium starter kits.

Name of Product: Hot chocolate pots

Units: About 28,000 units in the United States and 700 in Canada

Importer: ICI USA, LLC, of Seattle, Wash.

Hazard: The handle of the hot chocolate pot can break off during use, posing burn and laceration hazards.

Incidents/Injuries: The firm has received 28 reports of handles breaking off of the pots, including eight reports of injuries involving minor burns or cuts.

Description: This recall involves the Whirly Whip hot chocolate pots sold individually as item number 2981454 or 4986535, and as part of the Whirly Whip hot chocolate pot gift set item number 3021714. The item number can be found on the product's box, below the bar code. The pot is made of white porcelain and has a red handle. The lid has a red knob and a frother attached to the underside of the lid knob.

Ocean Technology Systems Recalls Guardian Full-Face Diving Masks Due to Drowning Hazard

The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Guardian full-face diving masks

Units: About 1,700 in the U.S. and 80 in Canada

Manufacturer: Undersea Systems International Inc. dba Ocean Technology Systems, of Santa Ana, Calif.

Hazard: The purge assembly on the diving mask can disengage from the regulator, resulting in loss of air to the diver. This poses a drowning hazard to the consumer.

Incidents/Injuries: The firm has received one report of a disengaged assembly. No injuries have been reported.

Description: This recall involves Guardian full-face diving masks with serial numbers 9051284 through 100070954. The serial number is printed on the main regulator body. The diving masks were sold in black with blue accent colors. The Ocean Technology Systems' logo is affixed to the front of the mask.

Sold by: Diving equipment retailers and direct sales nationwide from September 2010 through November 2010 for about $800.

Manufactured in: Taiwan

Remedy: Consumers should immediately stop using the diving masks and contact Ocean Technology Systems for instructions on conducting a test of the regulator and returning the masks for a free repair.

Vitalabs, Inc., Jonesboro, GA is voluntarily recalling four specific lot numbers of Whey-26 Protein Powder due to concerns over potential Salmonella contamination.

Salmonella is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (e.g., infected aneurysms), endocarditis and arthritis.

This recall extends to various parts of the United States including Alabama, Arizona, California, Colorado, Georgia, Florida, Illinois, Kentucky, Louisiana, Maryland, Missouri, New Mexico, New York, North Carolina, Ohio, Oklahoma, Pennsylvania, South Carolina, Tennessee, Texas, Utah, and Washington and outside the United States in Aruba, Ontario and Poland. The recalled product was offered through a variety of distribution channels including health food stores, clinics, fitness centers and internet based companies.

Consumer complaint leads to recall of O.N.E. Coconut Water

Georgia Agriculture Commissioner Gary W. Black alerts consumers to the recall of some coconut water following a consumer complaint received by the Georgia Department of Agriculture.

Georgia Department of Agriculture Compliance Specialists collected samples of the product, O.N.E. Coconut Water, for laboratory analysis after being alerted of potential mold in the product by the consumer. It was discovered that potential oxidation of the product may permit the growth of mold causing the coconut water to be discolored and have an off flavor.

Upon this discovery, the Department began working cooperatively with the company and the U.S. Food and Drug Administration. Out of an abundance of caution, the company issued a recall of the product.

Georgia Department of Agriculture Compliance Specialists will continue to monitor facilities for the presence of the recalled product.

FOR IMMEDIATE RELEASE – ONE World Enterprises, LLC is recalling O.N.E. Coconut Water, packaged in 11.2 oz Tetra Pak cartons with Lot code 12E1004, UPC # 894991001104 and Sell-by date of May 1, 2011, due to the potential oxidation of the product that may permit the growth of mold. The affected product is isolated to a single lot number and the company is currently utilizing Corrective And Preventative Action (CAPA) procedures to identify why this problem exists, which was discovered after receiving a small number of consumer complaints.

The affected product was distributed to various retail outlets in Florida, Georgia, Indiana, New York, South Carolina, and Tennessee.

To date all internal testing at the manufacturing plant indicates no contamination. Therefore, the company is investigating possible issues that may have occurred during transport thereby causing potential oxidation.

Hazard: The decorative bronze powder coat finish on the fireplace chimney can ignite during use, posing a fire hazard to consumers.

Incidents/Injuries: Sunjoy has received 14 reports of the chimney's decorative powder coat finish catching fire, resulting in one report of melted siding. No injuries have been reported.

Description: This recall involves "Garden Treasures Living" steel outdoor fireplaces with a bronze finish chimney and slate colored accents. The wood burning fireplace is approximately 24 inches deep by 35 inches wide and 57 inches tall, has two glass doors and a tile back inside. Item number 0027705 and model number L-OF082PST-3 are printed on the front page of the product's instruction manual. No other "Garden Treasures Living" brand outdoor fireplaces are included in this recall.

Sold at: Lowe's retail stores nationwide from March 2010 through November 2010 for about $300.

REMEDYTEAS Voluntarily Recalls Peppermint Organic Herbal Tea And Organic Herbal Tea Blend Because Of Possible Health Risk

RemedyTeas announced a voluntary recall of 20 lbs of Peppermint Organic Herbal Tea produced by Aromatics Inc., Basin City, WA, because it has the potential to be contaminated with Salmonella. Salmonella is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting, and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis, and arthritis. The recall was as the result of a batch sample testing program by the vendor after it received a notification of possible contamination which revealed that the finished products contained the bacteria. The vendor has ceased the production and distribution of the product as the FDA and the vendor continue their investigation as to the origin of the contamination. RemedyTeas has not caused the contamination of Salmonella and RemedyTeas has ceased distribution of products affected by the vendor’s recall.

The #137 Peppermint Organic Herbal Tea or #120 Tranquility Herbal Tea were distributed in the RemedyTeas retail store and through mail and internet orders. #137 Peppermint Organic Herbal Tea or #120 Tranquility Herbal Tea is sold measured to customers orders in retail stores and sold in 2 oz pre-packaged pouches by mail order or internet. There are no lot/batch identifying markings on the store or ecommerce packaging. Retail stock of #137 Peppermint Organic Herbal Tea was sold between January 18, 2011 and March 4, 2011. Retail stock of #120 Tranquility Herbal Tea was sold between February 22, 2011 and March 4, 2011. Any purchases not within those dates are not affected by this recall.

While no illness related to this voluntary recall has been reported to date, any potential health risk is significantly reduced when following the printed brewing instructions on the package or available online, i.e. using 205 F water temperature.

Consumers who have purchased #137 Peppermint Organic Herbal Tea or #120 Tranquility Herbal Tea are urged to return it to the place of purchase for a full refund. McCormick & Company, Incorporated (NYSE:MKC) has announced a voluntary recall of Golden Dipt® Tempura Seafood Batter Mix 8 OZ, with UPC Code 4123470132 and “BEST BY” date of OCT 08 12 H. A very limited number of packages of Golden Dipt® Tempura Seafood Batter Mix with this date code contain an undeclared milk ingredient. People who have an allergy to milk run the risk of serious or life threatening allergic reactions if they consume this product.

The recalled cartons of Golden Dipt® Tempura Seafood Batter Mix were distributed to grocery stores in Florida, Georgia, Kentucky, Massachusetts, Maryland, Maine, Michigan, North Carolina, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Tennessee, and Wisconsin as well as internationally to the Cayman Islands, beginning on October 21, 2010. The affected product is packaged in 8 OZ cartons, and the date code is found printed in white ink on the bottom of the carton.

The recall was initiated after the discovery of a packaging error that resulted in a pouch of Cajun Style Fish Fry mix (which contains a milk ingredient) being packaged in the Golden Dipt® Tempura Seafood Batter Mix carton. The ingredient statement on the package does not list milk as an ingredient.

The “BEST BY” date of OCT 08 12 H is the only date code affected by the recall. No illnesses or allergic reactions have been reported to date and no other McCormick products are involved in this recall.

All grocery outlets that sell Golden Dipt® Tempura Seafood Batter Mix are being notified to remove the affected product (UPC Code 4123470132 and date code BEST BY OCT 08 12 H) from their shelves immediately. Consumers do not need to return the product to the store where it was purchased. Instead, consumers are urged to contact McCormick’s Consumer Affairs team at 1-800-632-5847, weekdays from 9:30 AM to 9:00 PM, or weekends from 11:00 AM to 7:00 PM (Eastern Time), for a replacement or full refund, as well as instructions on what to do with the product.

This recall is being made with the knowledge of the Food and Drug Administration. The Company is also issuing an alert through the Food Allergy & Anaphylaxis Network.

Description: The Parents(r) Busy Time Activity Centers(tm) is a wooden activity center cube-shaped toy that has a multi-colored bead run on the top. Activities on each side include a 4-tone xylophone and rasp, butterfly and caterpillar spinners, zig-zag drum, castanets, and pockets with "alligator," "bear," and "cat" pillow characters. Only items with wooden pegs bearing date code 400090GC are involved in this recall. The date code can be found on the bottom of the product packaging near the UPC code and on the tags on the bear character.

Sold at: Specialty and gift stores nationwide and through mail order catalogs and online and in Canada from December 2010 through February 2011 for about $90.

Hazard: Strangulations can occur when a child plays with the beaded strands by wrapping them around their necks or by creating loops in which they can insert their heads. Also, children can get entangled in the strands, which are prone to entangle, just by running through the doorway.

Incidents/Injuries: The firm has received three reports of entanglement. In January 2009, a nine-year old girl was entangled while passing through the beads and nearly choked as she continued walking through the curtain. In January 2010, a six-year old girl in Reno, Nev. became entangled in the bead strands as she passed through them and suffered slight lacerations to her neck. In April 2010, a girl became entangled as the beads swung back behind her father who had walked through the beads before her. She was pulled off her feet and momentarily suspended before she regained her footing and she received scratches to her neck.

Description: The Circo pink door curtain is a set of two beaded door curtains, each of which is 12 inches wide and 72 inches long. The warnings on the packaging include: "Not for use in areas with children under 5 years of age. Plastic ornaments may pose strangulation or entanglement hazard. Not for use near cribs or playpens...". The photograph on the packaging shows an installed door curtain at the doorway of a girl's bedroom with pink wall color, child-themed lamp and clock. There is a CAUTION statement included in the installation instructions that states: "Do not tie bottom to form loops."

Sold exclusively at: Target stores nationwide and target.com from January 2009 through May 2010 for between $13 (at target.com) and $15 in stores.

Remedy: Consumers should immediately take down the curtains and contact FAB/Starpoint for a full refund. FAB/Starpoint will provide a postage-free package to consumers to return the door curtain.

Haddon House of Medford, New Jersey is recalling Asian Gourmet Cheese Rice Crackers (UPC 076606-710889), because it may contain undeclared milk and food coloring additives used to create the cheese powder listed on the label. People who have an allergy or severe sensitivity to milk run the risk of serious or life-threatening allergic reaction if they consume these products.

The Asian Gourmet Cheese Rice Crackers was distributed in the following states and was distributed by Haddon House Food Products to numerous retailers: AL, CT, DE, FL, GA, IL, IN, KY, ME, MD, MA, MI, MS, NH, NJ, NY, NC, OH, PA, RI, SC, TN, VT, VA, Washington, DC, WV, WI.

Specific information on how the product can be identified is as follows. This recall includes all lot numbers. As of this date, no illnesses have been reported due to this recall. Upsher-Smith Laboratories, Inc., of Maple Grove, Minnesota is voluntarily expanding its previously announced recall of Jantoven® Warfarin Sodium, USP, 3 mg Tablets to include additional products that were packaged on the same packaging line between May 17, 2010 and November 17, 2010. The company is initiating the recall as a precautionary measure after a bottle labeled as Jantoven® Warfarin Sodium, USP, 3 mg Tablets was found by a retail pharmacy to contain tablets at a higher, 10 mg strength.

At Upsher-Smith, patient safety is of foremost concern. The substitution of warfarin, or any other product, may lead to a change in the therapeutic effect of the intended drug.

Consistent, continuous dosing of any product is necessary for optimal care for many ill patients. Patients should check with their health care provider regarding the appropriateness of their current therapy prior to making any change.

Both the video monitor and receiver components come with A/C adapters but only the receiver unit contains a rechargeable battery. The batteries are unmarked or marked with letters MP or BK on the lower right corner of the battery. Batteries that are marked TCL are not included in this recall.

Two Strangulation Deaths Prompt Summer Infant to Recall Video Baby Monitors with Cords; Firm to Provide New On-Product Label & Instructions

/PRNewswire/ -- The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Summer Infant Inc., of Woonsocket, R.I., is announcing the voluntary recall to provide new on-product label and instructions for about 1.7 million video baby monitors with electrical cords. The cords can present a strangulation hazard to infants and toddlers if placed too close to a crib. Because of this serious strangulation risk, parents and caregivers should never place these and other corded cameras within three feet of a crib.

Over the past year, CPSC and the firm have received reports of two strangulation deaths of infants with the electrical cords of Summer Infant video baby monitors. In March 2010, a 10-month-old girl from Washington, D.C. strangled in her crib in the electrical cord of a Summer Infant video monitor. The monitor camera had been placed on top of the crib rail.

In November 2010, CPSC received a report of a six-month-old boy from Conway, S.C., who strangled in the electrical cord of a baby monitor placed on the changing table attached to the crib. In January 2011, CPSC learned the product involved was a Summer Infant video baby monitor.

CPSC and the firm are also aware of a near strangulation incident in which a 20-month-old boy from Pittsburg, Pa. was found in his crib with the camera cord wrapped around his neck. The Summer Infant monitor camera was mounted on the wall, but the child was still able to reach the cord. He was freed from the cord without serious injury.

Summer Infant has initiated a campaign to provide new on-product labels for electric cords and instructions to consumers with the recalled video monitors distributed between January 2003 and February 2011. The baby monitors were sold at major retailers, mass merchandisers, and juvenile products stores nationwide for between $60 and $300. They were sold in more than 40 different models, including handheld, digital, and color video monitors. All video monitors include both the camera (placed in the baby's room) and the hand held device (some models have 2 hand-held devices) which enable the caregiver to see and/or hear the baby from a specific distance. The brand "Summer" is found on the product.

The product was manufactured in China.

CPSC and Summer Infant urge parents to immediately check the location of the video monitors, including cameras mounted on the wall, and all electric cords to make sure the cords are out of arm's reach of their child. Name of Product: Rosebud drop-side cribs

Units: About 300

Manufacturer: Status Furniture, of Quebec, Canada (out of business)

Distributor/Retailer: The Land of Nod, of Northbrook, Ill.

Hazard: The drop-side rail hardware on the cribs can break or fail, allowing the drop side to detach from the crib. When the drop side detaches, a hazardous gap is created between the drop-side rail and the crib mattress in which infants and toddlers can become wedged or entrapped, posing risks of suffocation and strangulation. In addition, children can fall out of the crib when the drop-side rail falls unexpectedly or detaches from the crib. Drop-side rail failures can also occur due to incorrect assembly or with age-related wear and tear.

Incidents/Injuries: CPSC and The Land of Nod have received 13 reports involving drop-side hardware that has malfunctioned or failed. No injuries have been reported.

Description: This recall includes "Rosebud" cribs manufactured by Status Furniture. The cribs are white or antique white and have plastic drop-side hardware. "Status Furniture" appears on crib labeling on the lower portion of the headboard. Model number "910" appears on the assembly instructions.

Sold at: The Land of Nod retail stores and online at www.landofnod.com from January 2003 through September 2004 for about $600.

Manufactured in: Canada

Remedy: Consumers should stop using these cribs immediately and contact The Land of Nod to receive instructions on how to receive a merchandise credit for the full purchase price of the crib ($599). The Land of Nod is undertaking this recall for its customers because Status Furniture is out of business. In the meantime, parents are urged to find an alternate, safe sleeping environment for the child, such as a bassinet, play yard or toddler bed depending on the child's age.

Important Message from CPSC: CPSC reminds parents not to use any crib with missing, broken or loose parts. Make sure to tighten hardware from time to time to keep the crib sturdy. When using a drop-side crib, parents should check to make sure the drop side or any other moving part operates smoothly. Always check all sides and corners of the crib for parts separating. Disengagements can create a gap and entrap a child. In addition, do not try to repair any side of the crib. Babies have died in cribs where repairs were attempted by caregivers. Crib age is a factor in safety. At a minimum, CPSC staff recommends that you not use a crib that is older than 10 years. Many older cribs may not meet current voluntary standards and can have numerous safety problems.

Incidents/Injuries: phil&teds has received three reports of incidents resulting in injuries to the adult users including a finger tip amputation and two reports of lacerations.

Description: This recall involves sport v2 and classic v1 model single-seat jogging strollers. The three-wheel strollers have a metal frame, cloth seat and a canopy. The sport v2 model stroller was sold in red, orange, green, black, charcoal, navy and in graffiti print. Sport v2 serial numbers included in the recall are 0308/0001 to 0510/0840. The classic v1 model strollers were only sold in red. Serial numbers for the classic v1 are 0308/0001 to 0510/0906. The first four digits of the serial number is a month/year date code and the last four digits are for the individual stroller. Serial numbers are printed on the inside of the folding hinge. The phil&teds logo is located on the crotch piece of the harness on both models.

Sold by: Specialty juvenile stores nationwide from May 2008 through July 2010 for between $350 and $450.

Name of Product: Xantrex Grid Tie Solar Inverters

Units: About 25,000

Manufacturer: Xantrex Technology Inc. - a subsidiary of Schneider Electric, of Livermore, Calif.

Hazard: A component of the inverter can degrade, causing out gassing within the wiring compartment of the inverter. When arcing occurs, gasses could build and force the compartment cover to be blown off. If the cover is blown off with sufficient force it can injure the user or person, or cause damage to property in close proximity to the inverter.

Incidents/Injuries: Schneider Electric has received five reports of wiring compartment covers being blown off. No injuries or property damage have been reported.

Description: The recalled inverter converts solar photovoltaic voltages into utility grid voltages; allowing the owner to feed power into the electrical grid. The recalled units were manufactured between September 2005 and August 2010. These products were sold under the Xantrex, Sunpower, and General Electric brands. The brand name is printed on the front of the unit.

Candy Dynamics Recalls Toxic Waste® brand Nuclear Sludge® Chew Bars all flavors -- Net wt. 0.7 oz (20 g) package

Circle City Marketing and Distributing doing business as Candy Dynamics, Indianapolis, IN, is issuing a voluntary recall of all Toxic Waste® brand Nuclear Sludge® Chew Bars, all flavors, Net wt. 0.7 oz (20 g) package. The product is imported from Pakistan.

A recent test performed by the California Department of Public Health has indicated that a lot (#8288A) of the cherry flavor of the above-listed product contains elevated levels of lead (0.24 parts per million; the U.S. FDA tolerance is 0.1 ppm) that potentially could cause health problems, particularly for infants, small children, and pregnant women.

Out of an abundance of caution, the company has determined to recall all lots and all flavors of the product distributed from the product's inception in 2007 through January 2011.

The products are identified as: Toxic Waste® Nuclear Sludge® Cherry Chew Bar (UPC 0 89894 81430 6), Toxic Waste® Nuclear Sludge® Sour Apple Chew Bar (UPC 0 10684 81410 7), and Toxic Waste® Nuclear Sludge® Blue Raspberry Chew Bar (UPC 0 89894 81420 7). Each chew bar has a net wt. of 0.7 oz (20 g).

No other "Toxic Waste®" brand product is affected by this recall.

No illnesses have been reported to date in connection with this problem.

The recalled Nuclear Sludge™ Chew Bars were distributed nationwide in retail stores and through mail orders.

The company has discontinued distribution of the cherry flavor and had previously discontinued distribution of the other two flavors.

Triad Group Issues a Voluntary Nationwide Recall of All Lots of Alcohol Prep Pads, Alcohol Swabs, and Alcohol Swabsticks Due to Potential Microbial Contamination

Triad Group, a manufacturer of over-the-counter products has initiated a voluntary product recall involving ALL LOTS of ALCOHOL PREP PADS, ALCOHOL SWABS, and ALCOHOL SWABSTICKS manufactured by Triad Group but which are private labeled for many accounts to the consumer level. This recall involves those products marked as STERILE as well as non-sterile products. This recall has been initiated due to concerns from a customer about potential contamination of the products with an objectionable organism, namely Bacillus cereus. We are, out of an abundance of caution, recalling these lots to ensure that we are not the source of these contamination issues.

Use of contaminated Alcohol Prep Pads, Alcohol Swabs or Alcohol Swabsticks could lead to life-threatening infections, especially in at risk populations, including immune suppressed and surgical patients. To date we have received one report of a non-life-threatening skin infection.

Alcohol Prep Pads, Alcohol Swabs and Alcohol Swabsticks are used to disinfect prior to an injection. They were distributed nationwide to retail pharmacies and are packaged in individual packets and sold in retail pharmacies in a box of 100 packets. The affected Alcohol Prep Pads, Alcohol Swabs and Alcohol Swabsticks can be identified by either “Triad Group,” listed as the manufacturer, or the products are manufactured for a third party and use the names listed below in their packaging:

Brand Castle, LLC is recalling its licensed Rice Krispies Treats Holiday Village Kit. This recall was initiated because the icing component of the kit contains undeclared egg; no other components of the kit are affected. People who have allergies to egg run the risk of serious or life-threatening allergic reactions if they consume this product.

Approximately 20,000 of the recalled Holiday Village Kits were distributed to Michaels retail stores in the U.S. and Canada. This Holiday Village Kit is not produced by the Kellogg Company, although the product does contain Kellogg’s Rice Krispies. There is no labeling issue with the Rice Krispies.

The product comes in a 23.5-ounce cardboard package, marked with a UPC Code of 8-44527-02106-9 and is identified with the following expiration dates: 15 Oct 2012; 16 Oct 2012; and 02 Nov 2012.

One (1) allergic reaction has been reported to date.

The recall was initiated after it was discovered that an incorrect icing pouch was packaged with the Holiday Village Kits. This recall is limited to the Holiday Village Kit and does not include the Train, Snowman or Holiday House kits.

The products are being recalled due to an undeclared milk ingredient present in a small amount in the product. People who have an allergy or severe sensitivity to milk run the risk of serious or life-threatening allergic reaction if they consume these products.

“The packaging error was detected during a routine quality and label review by Publix,” said Rick Bernardo of South Florida Bakery. “There have been no reported cases of illness. FDA announces recall of Abbott glucose test strips

The U.S. Food and Drug Administration today (December 22) announced the agency is working with Abbott Diabetes Care to recall 359 different lots of glucose test strips marketed under the following brand names:

* Precision Xceed Pro;
* Precision Xtra;
* Medisense Optium;
* Optium;
* OptiumEZ; and
* ReliOn Ultima

These strips are used with Abbott’s Precision Xtra, Precision Xceed Pro, MediSense Optium, Optium, Optium EZ and ReliOn Ultima blood glucose monitoring systems. As many as 359 million strips may be affected by the recall. The blood glucose monitoring systems are not affected by this recall.

The recall pertains to certain lots of these test strips distributed in the United States and Puerto Rico. Other Abbott Diabetes Care products are not affected by the recall.

The test strips being recalled may give falsely low blood glucose results. False results may lead patients to try to raise their blood glucose unnecessarily, or they may fail to treat elevated blood glucose because of a false, low reading. Both scenarios pose risks to a patient’s health.

The recall is related to the test strips’ inability to absorb enough blood for monitoring. Strips exposed to warm weather or prolonged storage may be more likely to provide a false result.

The test strips were manufactured between January and September 2010 and are sold both in retail and online settings directly to consumers, but are also used in health care facilities.

The FDA has provided recommendations for consumers and health care professionals below that explain how to determine whether a particular lot is affected, how to order free replacement strips and how to use recalled strips to reduce the likelihood a false result.

“Patients with diabetes should be aware of the recalled glucose test strips and take steps to prevent them from affecting their health,” said Alberto Gutierrez, Ph.D., director for the Office of In Vitro Diagnostics in FDA’s Center for Devices and Radiological Health. “FDA and Abbott are reviewing the cause of the manufacturing defect to avoid this problem in the future.”

To determine if you have product being recalled:

* Call Abbott Diabetes Care customer service at 1-800-448-5234 (English) and 1-800-709-7010 (Spanish) to speak with a customer service representative.
* Visit www.precisionoptiuminfo.com to look up test strip lot numbers.

Consumers should report serious adverse events (side effects) with the device to the FDA's MedWatch Adverse Event Reporting program either online, by regular mail, fax, or phone.

Recommendations for Consumers Who Purchased the Precision Xtra, Optium, OptiumEZ, and ReliOn Ultima Blood Glucose Blood Glucose Test Strips

For consumers who purchased test strips in retail stories or online, FDA recommends:

* Calling Abbott for a replacement of the affected strips. Abbott will send you unaffected Precision Xtra, Optium, OptiumEZ, and ReliOn Ultima Blood Glucose Test Strips at no charge.
* While waiting for the replacement strips to arrive, use an alternate method to measure blood glucose (such as a different test system) or purchase at least two weeks worth of new, unaffected strips while waiting for replacement strips.
* If you purchase Precision Xtra, Optium, OptiumEZ, and ReliOn Ultima Blood Glucose Test Strips in a store or online, check to be sure these are from unaffected lots. Ask a pharmacist to help you.
* If the only test strips available to you are from affected lots, do not stop testing your blood glucose. But do take the following two precautions to reduce the chance of erroneous reading:

Precaution 1: Check the amount of time it takes for your blood glucose meter to start the “countdown” after you first apply blood to the test strip. Start timing immediately after blood first makes contact with the test strip. If your meter takes longer than five (5) seconds to start the countdown that test strip is defective and the result should not be used.

Check the time for each test strip you use because all of the strips in a package may not be affected to the same degree.

Precaution 2: If any reading from a strip appears lower than you would expect or does not seem to match the way you are feeling, you should contact your health care provider.

Pay special attention to signs and symptoms of high blood sugar (hyperglycemia) and low blood sugar (hypoglycemia).

* Symptoms of high blood sugar include excessive thirst, excessive urination, blurred vision, weakness, nausea, vomiting and abdominal pain. If you are experiencing any of these symptoms or are not feeling well, contact your health care professional immediately.
* Symptoms of low blood sugar may include trembling, excessive sweating, weakness, hunger, confusion, and headache. Some individuals may have no symptoms at all before they develop unconsciousness or seizures. It is important to treat low blood sugars promptly to avoid loss of consciousness or a seizure. If you are unable to obtain unaffected strips, you should contact your health care provider for advice on how to treat these symptoms before they occur.

Recommendations for Health care Professionals using Precision Xceed Pro Blood Glucose System

If you have affected strips, FDA recommends the following:

* If available, immediately switch to using test strips from unaffected lots.
* If your facility does not have any test strips from unaffected lots, and you have immediate access to an alternate Point of Care blood glucose testing system within your healthcare facility, discontinue use of the Precision Xceed Pro Blood Glucose Test System and use the alternate method until you can obtain unaffected Precision Xceed Pro Blood Glucose Test Strip lots.
* If your facility does not have test strips from unaffected lots, and you do NOT have immediate access to an alternate Point of Care blood glucose testing system, FDA recommends the following procedures:

1. Verify any critical glucose test results (e.g., test results that may be used to adjust insulin therapy in vulnerable patient populations) generated on the Precision Xceed Pro Blood Glucose Test System using a central laboratory blood glucose method. Medical judgment should be applied when deciding whether to act on results prior to verification.
2. Verify any Precision Xceed Pro Blood Glucose Test System results that do not match a patient’s symptoms, or seem unexpected for the patient’s clinical status, using a central laboratory blood glucose method.
3. When using the Precision Xceed Pro Blood Glucose Test System, take precautions to reduce the chance of an erroneous reading. Limited evidence suggests that results may be accurate using strips from affected lots if fill time does not exceed five seconds. Monitor the amount of time it takes for the Precision Xceed Blood Glucose Meter to start the “countdown” after blood is first applied to the test strip. If the amount of time exceeds five (5) seconds, discard that test strip immediately because the blood glucose result may be erroneously low. In addition, if this occurs:
i. Note the specific lot number of that test strip;
ii. Notify Abbott Diabetes Care to report the observed problem with that lot by calling 1-877-529-7185;
iii. Make sure to check fill time on each individual strip during use and do not assume if one strip in a package/lot appears to be unaffected, that all strips in that package/lot are unaffected.

Pfizer To Recall One Lot Of Lipitor In The U.S.

Pfizer has announced that it intends to recall one lot—approximately 19,000 bottles—of Lipitor 40 mg tablets (atorvastatin calcium) distributed in the U.S. The recall stems from one customer report of an uncharacteristic odor related to the bottles in which these lots of Lipitor were packaged. The bottles were supplied by a third-party bottle manufacturer.

A medical assessment found that the risk of health consequences to patients appears to be minimal.

The market action is the result of Pfizer’s increased surveillance of odor-related issues after other reports in the industry. This increased surveillance also led to three earlier recalls of Lipitor, implemented in August, October and November of 2010, in response to infrequent complaints of uncharacteristic odor.

The odor is consistent with the presence of 2, 4, 6 tribromoanisole (TBA), which was found at a very low level in a complaint sample bottle during the investigation leading to the first product recall. Research indicates that a major source of TBA appears to be 2, 4, 6-tribromonophenol (TBP), a chemical used as a wood preservative. Although TBP often is applied to pallets used to transport and store a variety of products, Pfizer prohibits the utilization of TBP-treated wood in the shipment of its medicines.

The company has taken quick action to ensure its product continues to meet the company’s high quality and patient safety standards. We have identified the source of the odor, and we are enacting rigorous measures to prevent odor-related issues going forward. The lot that will be recalled was packaged and shipped before these changes went into effect in August of this year. LIPITOR (atorvastatin calcium) is a prescription medicine that is used along with a low-fat diet. It lowers the LDL ("bad") cholesterol and triglycerides in your blood. It can raise your HDL ("good") cholesterol as well. LIPITOR can lower the risk for heart attack, stroke, certain types of heart surgery, and chest pain in patients who have heart disease or risk factors for heart disease such as age, smoking, high blood pressure, low HDL, or family history of early heart disease.

LIPITOR can lower the risk for heart attack or stroke in patients with diabetes and risk factors such as diabetic eye or kidney problems, smoking, or high blood pressure.

IMPORTANT SAFETY INFORMATION

IMPF Voluntarily Recalls Dips & Spreads because of Possible Health Risk

Bright Water Seafood, Tucker, GA is voluntarily recalling its 7 ounce packages of Buffalo Krab Dip and 7 ounce packages of Southwest Krab Dip because they have the potential to be contaminated with Listeria monocytogenes, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, Listeria infection can cause miscarriages and stillbirths among pregnant women.

The recalled Dips & Spreads were distributed in retail stores in North Carolina, South Carolina, Tennessee, North Georgia and Virginia.

The Bright Water Buffalo Krab Dip product comes in a 7 ounce, clear plastic container marked with an UPC code 8 1971100001 4 and with an expiration date of 12/21/2010 stamped on the side. The Bright Water Southwest Krab Dip comes in a 7 ounce, clear plastic container marked with an UPC code 8 1971100002 1 and with an expiration date of 12/29/2010 stamped on the side.

No illnesses have been reported to date in connection with this problem.

The potential for contamination was noted after routine testing by the company revealed the presence of Listeria monocytogenes in 7 ounce container.

The production of the product has been suspended while the Company, GA Department of Agriculture and US FDA continue to investigate the source of the problem.

Reese Pharmaceutical Company of Cleveland, Ohio has voluntarily recalled lot# 091612 only in 60-count size bottles identified under four different brand names, listed in the table below, because cold decongestant tablets (containing Acetaminophen 325 mg, Phenylephrine 5 mg & Chlorpheniramine Maleate 2 mg) were mislabeled as containing only 200mg Guaifenesin tablets. This mislabeling could cause a consumer to ingest the product and unknowingly be exposed to serious side effects of acetaminophen, phenylephrine or chlorpheniramine.

The mislabeled product does not warn consumers that Acetaminophen may cause liver damage. Ingesting high doses of Acetaminophen can potentially cause severe liver damage. The likelihood of acute liver damage is higher among consumers with pre-existing liver disease and those who drink three or more alcoholic drinks per day. Overdose may specially occur if consumers are also taking other cold/cough products that contain Acetaminophen in addition to the mislabeled product. Contraindications for Phenylephrine are high-blood pressure, poor blood flow to the extremities, and patients on antidepressants known as MAO Inhibitors. Furthermore, products that contain Phenylephrine should be used with caution in patients with high blood pressure, diabetes, heart disease, increased intraocular pressure, hyperthyroidism, or enlarged prostate. Consumers who are allergic to any of the ingredients or who have narrow angle glaucoma, or are pregnant or nursing mothers, also should not take the product. However, to date, there have been no reports of adverse events from its use.
